About this file

This is a Small Business Enterprise (SBE) Subcontractor Intent Form (BEH-FRM-610.SAW) issued by the City of Dallas Business Enterprise Hub (BEH) for use in conjunction with the SWWTP Energy Study and Biogas Beneficial Use project in Texas. The form documents a prime vendor's intent to subcontract with a certified SBE for specified goods, services, or work elements under designated NAICS codes. The form requires identification of the prime vendor, SBE subcontractor, project name, solicitation number, and a detailed description of the work scope to be performed by the SBE. Both parties must certify their commitment to the subcontracting arrangement and acknowledge that any reduction, substitution, or reallocation of the subcontractor's scope, value, or percentage participation requires prior BEH approval.

The form establishes SBE participation requirements, including contract-specific goals and the SBE vendor's participation amount as a percentage of the total prime contract value. The SBE subcontractor must maintain current certification throughout the entire contract duration and perform work within its certified NAICS code(s) under a Commercially Useful Function (CUF) using its own workforce and supervision. The SBE vendor's certification must be obtained from one of several authorized agencies, including the DFW Business Council, North Central Texas Regional Certification Agency, Women's Business Council SW, or other approved certification bodies. Failure to maintain current certification or comply with DRIVE policy requirements may result in adjustment of SBE participation credit or other remedial actions permitted under City of Dallas policy. The form requires signatures from authorized officers of both the prime vendor and SBE subcontractor attesting to the accuracy of the information provided.
