About this file
This is an evaluation scoresheet for a Request for Statements of Qualifications (RFSQ) issued by the State of Utah Division of Purchasing and the Office of the Utah Attorney General for an Officer Wellness Program. The contract is intended as a five-year professional services engagement to provide mental health support and wellness services to the Attorney General's Office. The vendor selection process requires submission of qualifications only, with pricing to be negotiated separately with the highest-ranked vendor. The evaluation will be based on technical criteria scored on a zero-to-five scale, with points distributed across six primary evaluation categories: Statement of the Problem (10%), Impact/Outcomes and Evaluation (14.3%), Goals/Objectives (14.3%), Project/Program Design and Implementation (21.4%), Capabilities/Competencies and Key Staff (21.4%), and Experience Working with Law Enforcement and Trauma/Child Abuse/Sex Abuse (21.4%), totaling 700 possible points across the technical evaluation.
All vendors must meet four mandatory requirements to be considered: hold licensure as a Licensed Clinical Mental Health Counselor (LCMHC), Licensed Clinical Social Worker (LCSW), or equivalent credential; maintain active licensure in the State of Utah; demonstrate documented experience working in child sex abuse and trauma; and be an outside resource rather than a State of Utah employee, ensuring confidentiality and removing barriers to open communication. If the highest-ranked vendor cannot reach agreement on fair and reasonable compensation and contract terms, the state may proceed with negotiations with the next highest-ranked vendor. No pricing information should be included in the qualification submissions.
