About this file

This file is a pathway overview document issued by the Utah Division of Professional Licensing (DOPL) under the Internationally Trained Applicant Credentialing Program (ITAC), in coordination with the State of Utah Department of Commerce. The document outlines the Limited Supervised Training Permit (LSTP) pathway, which is a pilot program designed to enable internationally trained physicians to practice in Utah under structured supervision while pursuing full physician licensure. The pathway grants a temporary three-year LSTP license to qualified applicants who meet specified educational, examination, and licensing requirements. To qualify, applicants must obtain ECFMG certification, pass the United States Medical Licensing Examination Steps 1 and 2, demonstrate English proficiency through the Occupational English Test, complete either a foreign residency program or postgraduate medical training or possess a minimum of five years of physician practice experience outside the U.S., maintain an active license in good standing from their home jurisdiction for at least one year within the preceding five years, and secure an employment offer from a Utah employer. Applicants must also enter into an LSTP Agreement with the Division that establishes supervision requirements, supervision levels, clinical competency evaluations, progress reporting obligations, and other governing conditions.

The LSTP Agreement mandates that applicants pass the United States Medical Licensing Examination Step 3 before the agreement expires, though candidates may attempt this examination at any time during the permit term. Full licensure eligibility is achieved upon the employer's submission of documentation confirming successful completion of all competencies outlined in the LSTP Agreement. The program is designed to address physician workforce needs while ensuring patient safety and clinical competency. Interested parties can obtain additional information by contacting ITAC at doplglobal@utah.gov. The related state and local contract opportunity references a Request for Statements of Qualifications for an Internationally Trained Physicians Evaluation Tool, issued by the State of Utah Division of Purchasing through the Department of Commerce, with a three-year contract term to be awarded under Utah Procurement Code 63G-6a Part 15 for Design Professional Service Providers and Consultants.
