About this file

This is a Proposer's General Questionnaire template issued by the University of Texas Health Science Center San Antonio (UTHSA) Department of Purchasing for RFP 745-26-P37, a Payment Card Program for clinical trial participants. The RFP seeks qualified suppliers to provide a secure, electronic payment card system to reimburse clinical trial participants for expenses including travel, meals, parking, lodging, and time-related burdens. UTHSA conducts clinical trials across diverse therapeutic areas and patient populations and aims to replace traditional paper-based reimbursement methods with a modern electronic payment solution that improves speed, accuracy, transparency, and compliance while reducing administrative overhead at investigative sites. The questionnaire requires proposers to provide detailed company information, financial documentation, customer references, key personnel resumes, service approach descriptions, work plans with milestones, quality assurance programs, and contingency plans. No specific contract term, award date, response deadline, or volume guarantee is identified in this questionnaire document.

The questionnaire mandates that proposers disclose ownership information, financial stability documentation, litigation history, default status on loans or financing agreements, and any relationships with UTHSA employees. Proposers must provide a minimum of three customer references from organizations for which they have provided similar services within the past five years, including contact information and project descriptions. The questionnaire explicitly requires proposers to address cloud computing service provisions and select from provided options regarding their use of such services. Additionally, proposers must confirm their ability to meet HIPAA compliance requirements and ensure financial integrity and auditability suitable for the highly regulated clinical research environment. UTHSA makes no guarantee of specific compensation amounts, service volumes, minimum or maximum service levels under this RFP or any resulting contract.
