About this file
This is a Cost Proposal Form for Solicitation #UDOT260313MH issued by the Utah Department of Transportation (UDOT) for public relations and media management services supporting the Zero Fatalities Program. The program focuses on eliminating crashes, injuries, and fatalities through education and behavior change communications targeting the traveling public. The PR and Media team will collaborate with UDOT Communications, the Zero Fatalities program team, and other partners to develop and implement safety messaging campaigns. The team will also provide ongoing support to the Behavioral Safety Program and Communications Office as needed throughout the contract term. The solicitation is anticipated to result in a single contract award, though specific response dates, due dates, site visits, bidder conferences, award dates, and contract term details are not provided in this cost form document.
Offerors must submit costs using a blended hourly rate covering all labor costs for staff and profit, including account supervision, planning, consultation, and other labor expenses. In addition to the blended rate, offerors must provide position-specific labor rates for Senior Manager/Executive, Mid-Level Staff, and Support Staff positions. Cost points will be awarded based on a comparative scoring formula where the lowest proposed price receives 100% of available cost points, with all other offerors receiving a proportional percentage based on their price relative to the lowest bid; proposals exceeding 200% of the lowest price receive no cost points. Travel costs will be reimbursed at the current Utah State Travel Policy per diem rates. Offerors must strictly adhere to the provided cost form format, as any modifications or alternate formats may result in proposal disqualification. Contracts will authorize payment for labor at agreed-upon rates plus pre-authorized pass-through direct expenses accompanied by invoices.

Cost Proposal Solicitation #UDOT260313MH
To be submitted as a separate document from the Technical Response document
Offeror Name:
Cost is to be submitted based on the following:
(Any deviation from this format may result in disqualification of proposal.)
Provide a blended hourly rate for the tasks described in the RFP. The “blended” hourly rate is the maximum billable rate for all services and must include all labor costs for staff, and profit. Labor may include account supervision, planning, consultation, and other labor expenses required to accomplish the needed tasks.
Blended Hourly Rate:
$________ per/hr.
Note: The Blended Hourly Rate will be used as the basis for assigning Cost Points in the RFP evaluation. The Position specific labor rates are for contracting purposes.
Additionally, provide the following specific position labor rates:
Position
Associated Rate
1. Senior Manager/ Executive
$
2. Mid-Level Staff
$
3. Support Staff
$
Purchasing will use the following cost formula: The points assigned to each offeror’s cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. An offeror whose Proposed Price is more than double (200%) the Lowest Proposed Price will receive no points. The formula to compute the points is: Cost Points x (2- (Proposed Price/Lowest Proposed Price)).
IMPORTANT: Offerors may not change/modify the format of this cost form. The State relies on this cost sheet to accurately and fairly compare offeror costs. Cost Proposals received in a modified or alternate format may be disqualified.
*Costs of travel will be reimbursed at the then-current per diem rate set by the State of Utah Travel Policy, as found at: http://www.rules.utah.gov/publicat/code/r025/r025-007.htm
The resultant contracts will specifically authorize payment for time spent on work based on the agreed upon labor rates, it also is the intent of the contract to allow for the payment of pass-through direct expenses (accompanied with invoices) as pre-authorized by the project manager.
