About this file
This solicitation document represents a Request for Proposal issued by the State of Utah's Division of Facilities Construction and Management (DFCM) to prequalified code inspection firms for the provision of code and special inspection services on the Convergence Hall project, identified as Project #24279300. The Convergence Hall project involves the construction of a six-story building with approximately 163,000 square feet, incorporating diverse room typologies including office space, maker space, retail areas, event spaces, and housing units. All special inspections contractors, material testing laboratories, and special inspectors engaged on this DFCM project must be listed on the DFCM Approved Special Inspection Firms, Material Testing Labs, and Special Inspectors list, regardless of whether they work directly or through a prequalified code inspection firm. The solicitation establishes specific requirements and protocols for the inspection and certification services necessary to ensure code compliance throughout the construction process.
The estimated budget allocated for inspections services is $650,000.00. The procurement is restricted to DFCM prequalified code inspection firms, which narrows the competitive field to vendors who have already completed DFCM's prequalification process. This set-aside requirement ensures that only firms meeting DFCM's established standards and criteria can participate in the bidding process. Prospective bidders must verify that all personnel assigned to the project comply with the agency's approval requirements for special inspectors and material testing personnel, establishing a rigorous quality assurance framework for the inspection services to be performed on this institutional construction project.

The importance of considering the teams distance to the project site relates to response time as inspections are requested.
D. Past Performance References | 20 Points DFCM Performance Ratings DFCM rates the performance of each Contractor on a scale of 1 (low) to 5 (high). Ratings for completed DFCM projects over the past five years will be averaged and used in the selection scoring matrix.

DFCM VBS Code & Special Inspections - Request for Proposals, 3.30.2026 8
DFCM performance ratings are available for review upon request by contacting the DFCM VBS Coordinator via email at dfcm_vbs@utah.gov.
Non-DFCM Performance References If a Contractor does not have a minimum of three active DFCM performance ratings, they are required to submit references for non-DFCM projects of comparable type, scope, size, and completed within the past five years that will supplement any of the Contractor’s DFCM performance ratings. All references must be for projects in which the Contractor was the prime Contractor.
Complete and accurate information for non-DFCM references must be provided on the DFCM Performance References Form. The form requires a minimum of five references. Incomplete reference information or the inability for DFCM to acquire a rating from a provided reference may result in the Contractor being disqualification from the selection process.
How does DFCM rate non-DFCM projects?
To obtain a minimum of three collective performance ratings for each Contractor, references provided for non- DFCM projects will be contacted at random and asked a series of predetermined questions. The reference will be asked to rate the Contractor on a scale of 1 (low) to 5 (high) for each question. The ratings will be averaged and result in a performance rating for the corresponding project. DFCM will only contact references until a collective total of three performance ratings for non-DFCM and DFCM projects are obtained. The three performance ratings will be averaged and used in the selection scoring matrix. Contractors that fail to meet this requirement or a minimum of three collective ratings may be disqualified from the selection process.
Additional Requirements The following documents are not part of the Selection Criteria but are requirements that, if not met, may disqualify the Contractor from the selection process or award of the Contract if not submitted.
E. DFCM Termination or Debarment Form The Contractor must sign and submit the provided DFCM Termination or Debarment Form certifying that neither it nor its principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from soliciting work by any government agency. This form also certifies that neither they nor their principals have been terminated during the performance of a contract or withdrew from a contract to avoid termination. If the Contractor cannot certify these two statements, the form provides a section for a written explanation of the circumstances to be provided for review by DFCM. Although this form is not part of the Selection Criteria, it is a required proposal document. Proposals submitted without this form may lead to Contractor disqualification.
F. Health Insurance Statement of Compliance Form (applicable to the awarded Contractor only) The Health Insurance Statement of Compliance Form applies to: (i) a prime Contractor (and design professional) if the prime contract (or design professional contract) is in the aggregate amount of $2,000,000 or greater; and

If applicable to this project, the awarded Contractor must submit the Health Insurance Statement of Compliance Form to DFCM via email at dfcmcontracts@utah.gov before the Construction Contract Notice to Proceed will be issued.
