About this file

This is a cost sheet attachment for a competitive sealed bid solicitation issued by Salt Lake City Corporation for surface treatment chip and slurry aggregate products under Bid No. SLCI26071. The solicitation requests qualified contractors to provide two required materials: Type II slurry sand and Grade A chip, each with an estimated annual usage of 1,250 tons. Bidders must submit pricing for both required materials in Category 1 to ensure bid responsiveness, with award based on the lowest total cost identified in the cost sheet's designated cell. The solicitation also includes an optional Category 2 for Type III slurry, though this alternate product will not be evaluated as part of the award determination. Specific response dates, due dates, site visit requirements, and bidder conferences are not detailed in this cost sheet attachment.

Pricing must be submitted on a per-ton basis for each material and delivery charges, with all costs including materials, supervision, labor, insurance, transportation, delivery, and fuel. The City will award a single contract from this solicitation to the lowest responsible and responsive bidder. Bidders must certify that chip seal and slurry seal aggregate materials are 100 percent crushed stone, slag, or other high-quality particle combinations and must provide location information, contact details, and standard operating hours. The bid submission requires bidder acknowledgment and compliance with Salt Lake City's ethics ordinance, conflict of interest standards, and gift prohibition policies, with violations potentially resulting in supplier suspension or debarment from future City procurement processes.
