Mandatory Requirements, Minimum Qualifications Checklist

Proposers must fill in the Mandatory Requirements/Minimum Qualifications and initial in attestation of compliance with the minimum qualifications and attach to your Proposal. This Attachment response is not part of the page count.

An outstanding violation, legal matter, or potential conflicts of interest does not automatically disqualify a team from participating in the procurement process. If indicated on this form or in 9C or 9D, the County will review a team’s information, may request additional information, but will decide as to the team’s ability to become compliant and be eligible for an award at the conclusion of the procurement process. Failure to indicate to the

County that a review is warranted may cause the District to disqualify a team as not responsive.
