About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in New Mexico for the Finch Street Extension project. The form serves as a pre-award certification requirement for bidders and contractors, establishing eligibility criteria and compliance standards prior to contract award. The certification applies to primary covered transactions and must be completed by the offeror/bidder's authorized representative with signature, date, and title. The form requires submission of company name, solicitation number, and solicitation title, with no specific response deadline or award date identified in the document itself.

The certification requires bidders to attest that neither they nor their principals are currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from covered transactions by any federal, state, or local department or agency. Bidders must also affirm they have not been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records within the preceding three years; have not violated contract provisions or ethical standards; are not indicted or criminally charged; are not current or former County employees or immediate family members of County employees or public officials; and have not had public contracts terminated for cause or default within three years. If unable to certify any statement, bidders must provide written explanation. The form stipulates that falsification of any certification statement constitutes grounds for non-consideration or contract award rescission, and bidders must confirm all proposed subcontractors are not suspended or debarred from conducting business with any governmental entity.
