About this file
This file is a Vendor Disclosure Statement form required by the Colorado Department of Corrections for prospective vendors seeking state contracts. The form mandates that vendors disclose information regarding contract performance locations and any work conducted outside the United States or State of Colorado prior to contract award. The specific solicitation referenced is NPSS1 #2026000110, related to a quote request for green twill fabric for the CCi Garment Factory. Vendors must identify all counties and states where services or subcontracted work will be performed and provide justification if any portion of the contract work is anticipated to occur outside the United States or Colorado.
The disclosure statement applies to most state contracts but specifically excludes Medicare, Colorado Medical Assistance Act contracts, the Children Basic Health Plan Act, the Colorado Indigent Care Program, and construction contracts. Vendors are required to complete the form and return it to the contracting state agency before or concurrent with solicitation submission. The form requires vendor name, signature, printed name, title, and date of completion. This document serves as a contractual requirement to ensure transparency regarding geographic performance of contracted services and to support the state's preference for in-state contract performance.
