About this file

This document is an Exhibit E Offeror Disclosure Statement from the Colorado Department of Corrections, designed to capture information about contract performance locations for potential vendors. The form requires prospective vendors to disclose the geographic locations where contract services, including subcontracts, will be performed, with specific attention to services conducted outside of the United States or Colorado. The disclosure is mandatory for contract solicitations, with explicit exceptions for certain healthcare-related contracts and construction contracts.

The form provides structured fields for vendors to document the solicitation number, vendor name, specific counties and states where services will be performed, and a required explanation if services are anticipated to be performed outside of the United States or Colorado. The document is part of the state's contract management process, ensuring transparency about service delivery locations. It requires a signature, printed name, and title from the vendor representative, indicating an official commitment to the disclosed information. This disclosure statement appears to be a standard component of the Colorado Department of Corrections' procurement process, aimed at maintaining accountability and understanding the geographic scope of potential service delivery.
