About this file
Exhibit A is an Offeror IFB (Invitation for Bid) Checklist from the Colorado Department of Corrections, specifically for the Adult Parole division's Community Re-Entry program. The document outlines the required submission documents for bidders interested in supplying pre-filled hygiene backpacks, which will be distributed quarterly to newly released offenders and parolees experiencing housing instability or transition barriers. The contract requires the successful offeror to deliver 720 pre-filled backpacks on the closest business day to July 15, October 15, January 15, and April 15 each year of the awarded term.
The checklist details four specific exhibits that must be submitted with the bid: Exhibit B (Required Offeror Information) which must be signed, Exhibit C (W-9 Request for Taxpayer ID Number) which must be completed and returned, Exhibit D (Price Bid), and Exhibit E (Offeror Disclosure Statement). All bid documents and attachments must be submitted through bidnetdirect.com, and bidders are instructed to include the IFB number and title on their submission. The document prompts potential offerors to review all exhibits and attachments carefully before submitting their bid.
