About this file

This is a Selection Criteria document issued by the State of Utah Department of Corrections for a Cognitive Behavioral Program Request for Statements of Qualifications (RFSQ). The procurement seeks a qualified vendor to implement evidence-based cognitive-behavioral interventions for individuals under the criminal justice system at moderate to high risk of reoffending. The contract term is five years, and vendors will be evaluated on a five-point scale across four primary criteria: Relevant Experience & Track Record, Train-the-Trainer Capability & Staff Certification, Implementation Plan & Methodology, and Theoretical Foundation & Evidence-Based Model. Each criterion is worth up to 5 points, with vendors expected to provide detailed, point-by-point responses addressing each selection area. The contract will be awarded under Administrative Rule R33-105 for Small Purchases of Professional Service Providers and Consultants, with the highest-ranked vendor selected to negotiate final contract terms.

Pricing information should not be included in qualification submissions, as compensation will be determined through negotiation with the selected vendor based on fair and reasonable rates. The small purchase threshold for this project is $100,000.00. If contract negotiations cannot be successfully completed with the highest-ranked vendor regarding compensation, contract requirements, and contract documents, the state reserves the right to proceed with negotiations with the next highest-ranked vendor in the scoring order.
