About this file

This document is an attachment to a Request for Proposal (RFP) issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services for locum tenens staffing services for pharmacists. The RFP seeks qualified vendors to provide licensed and experienced pharmacist staffing on an as-needed basis, with services to be delivered in-person as requested by DHHS. The anticipated contract structure includes multiple awards to vendors meeting specified performance thresholds. The contract term is five years, with vendors required to guarantee pricing for a minimum of one year.

Vendors must satisfy all mandatory minimum requirements to advance in the evaluation process, including maintaining current Utah licensure, board certification or board eligibility, valid DEA and Controlled Substance licenses, and the ability to provide 24/7 emergency staffing support with qualified candidates meeting DHHS submission deadlines. Technical evaluation will be scored on a 0-5 scale across five criteria addressing staffing qualification capability, credentialing and verification processes, experience in correctional or similar healthcare environments, response timeline performance and staff coverage reliability, and 24/7 emergency support capabilities. Contract awards will be granted to vendors achieving a minimum technical score of 420 points and a combined technical and cost score of at least 600 points.
