About this file
This is a Cost Proposal Form (Attachment C) for the Utah Geological Survey's solicitation NC26-3 seeking U-Pb crystallization ages for igneous rocks services. The Utah Division of Purchasing is conducting this procurement on behalf of the Utah Geological Survey's Energy & Mineral Resources division to obtain U-Th-Pb geochronology services for determining crystallization ages of intrusive magmatic rocks. The services include mineral separation, imaging, dating, and technical report preparation with calculated ages for igneous rock samples, with an estimated annual quantity of 50 samples. The contract will be a 5-year term with vendors required to guarantee pricing for one year, and the procurement is expected to result in a single contract award to the highest-scoring responsive and responsible vendor.
The cost proposal form requires vendors to provide fully loaded costs per sample with all expenses included, and cost evaluation will be based on total proposed costs using a point system where the lowest bidder receives 100% of cost points and other bidders receive proportional points based on their price premium. Vendors must complete all yellow-highlighted cells, enter N/A for non-applicable items, and any costs not included in this form will not be permitted in the resulting contract. The form warns that deviations from the specified format may result in disqualification, and the budgeted annual volume of 50 samples is provided for evaluation purposes only and does not guarantee actual order quantities.

NC26-3 U-Pb Crystallization Ages for Igneous Rocks
Vendor Name:
Costing Considerations & Instructions
1. All cells in a yellow highlight require an input. All line items must be fully loaded and include all costs.
2. Cost will be evaluated based on the Total Proposed Costs provided below.
3. Technical Requirements and Cost Proposals must be reflective of one another. All costs necessary to implement and maintain the proposed solution over the contract's term must be included on this Cost Schedule form.
4. If zero is entered for any cell or if any cell is left blank, the State will assume that the related services are either provided at no cost or built into another cost item. If a cost category is not applicable, enter N/A.
6. Any cost not provided on this Cost Schedule, will not be allowed into the resulting contract. If you identify a cost that cannot be properly captured in this Cost Schedule, then provide suggestions for modifying this Cost Schedule to the Q&A board for this solicitation.
8. Any deviation from this format may result in disqualification of Offeror's proposal.

| The total cost that will be evaluated | $ - 0 |
*Values provided for the Budgeted Annual Volume are for cost evaluation purposes. Such values do not constitute a guaranteed volume.
* The State will use the following cost formula: The points assigned to each offerors cost proposal will be based on the lowest proposal price. The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. The formula to compute the points is: Cost Points x (Lowest Proposed Price/Proposed Price).
