About this file
This is a Statement of Assurances and Certifications document for the City of Bend, Oregon's Commercial Grade Concrete (CGC) solicitation numbered 26-3407. The City of Bend seeks to establish one or more price agreements for the supply of commercial grade concrete on an on-call basis. The initial contract term is one year with options to extend for additional periods, not to exceed a total of five years. The City estimates a total contract value not to exceed $900,000 over the full five-year term but does not guarantee any minimum quantity of work due to the on-call nature of the services.
Bidders must certify compliance with numerous regulatory and policy requirements, including Title VI of the Civil Rights Act of 1964, the Americans with Disabilities Act, Oregon Workers' Compensation Law, and prevailing wage requirements under ORS 279C.800 to 279C.870. Bidders must also certify that they have inspected all work sites, reviewed plans and specifications, are not sham bidders, and will complete work in compliance with all applicable City, State, and Federal regulations. Bidders must declare their resident bidder status as defined by ORS 279A.120, disclose any outstanding claims against their firm, and provide information regarding surety company bonds if required. Bidders must sign the contract within seven calendar days of award and submit a Performance and Payment Bond if required. The document requires bidders to acknowledge the City's non-discrimination policy and to include similar compliance requirements in all subcontracts.
