About this file
This is an Experience Form template (Attachment 1) used to evaluate contractors' past performance and relevant experience for a plaza renovation construction project at the Mid-Atlantic Social Security Center in Philadelphia. The form requires contractors to document previous projects with characteristics listed in descending order of importance: construction projects valued at $15 million or higher, major plaza renovations/modernizations, structural steel replacement in existing buildings, hardscaping/landscaping projects, large stormwater management facility installations, projects completed within the last 10 years, phased construction in occupied buildings, and federal clientele.
The form includes sections for basic project information (contractor name, solicitation number, contract details), magnitude (total awarded/final prices, square footage), complexity factors (CMc Method, GMP utilization, multiple subcontractors, open book accounting), and detailed project descriptions. Contractors must provide points of contact for both building owners and contracting officers, along with geographic location details. The form emphasizes that offerors demonstrating experience meeting all listed characteristics will receive the highest evaluation scores, particularly for projects involving similar scope and complexity to the current solicitation.
