About this file

This document is a PREA (Prison Rape Elimination Act) Questionnaire form issued by the Nevada Department of Corrections (NDOC) to comply with federal requirements under 42 U.S.C. § 15601 et seq. and 28 C.F.R. § 115.17(a). The form must be completed by all contractors and volunteers who may have contact with offenders before they are authorized to enter any NDOC institutions, facilities, or work release centers. The questionnaire requires respondents to disclose whether they have engaged in sexual abuse in any detention facility, been convicted of sexual activity facilitated by force or coercion, been civilly or administratively adjudicated for such conduct, or engaged in sexual harassment in community or confinement settings. Respondents must provide detailed explanations for affirmative answers and sign the form affirming the truthfulness and completeness of their responses.

The form serves as a screening mechanism to identify and exclude individuals with histories of sexual misconduct from positions involving offender contact. Contractors and volunteers who provide false or misleading statements, including material omissions regarding such misconduct, are subject to application denial. This questionnaire is a mandatory prerequisite for participation in vending services contracts at Nevada correctional facilities, ensuring compliance with federal PREA standards and protecting the safety and welfare of offenders within the correctional system.
