Reference Questionnaire Summary

This is a Reference Questionnaire document issued by the Nevada State Purchasing Division, Department of Administration, for solicitation number 40DHHS-S3910 related to food services provision. The solicitation pertains to contracting for food services at multiple Nevada facilities including Desert Regional Center, Northern Nevada Adult Mental Health Services, Lakes's Crossing Center, Southern Nevada Adult Mental Health Services, and both Northern and Southern Nevada Department of Child and Family Services locations. The questionnaire serves as a formal evaluation tool requiring proposing vendors and subcontractors to provide client references demonstrating experience with projects of similar size and scope. The completed questionnaire must be submitted to Annette Morfin, Purchasing Officer II, at amorfin@admin.nv.gov no later than October 21, 2026, with the solicitation number included in the email subject line.

The reference questionnaire requests comprehensive evaluation ratings across twelve performance categories using a 1-10 scale, with additional narrative feedback required for selected items. Evaluation criteria include vendor knowledge and expertise, flexibility regarding scope and timeline changes, satisfaction with delivered materials and products, staff dynamics and interaction quality, schedule adherence and timeliness of deliverables, customer service responsiveness, billing accuracy, problem resolution capability, and likelihood of future recommendations. The State has designated completed questionnaires as confidential business information pursuant to Nevada Revised Statutes 333.020(5)(b), restricting public disclosure. Respondents are instructed to answer all questions completely, marking answers as "U/K" if unknown or "N/A" if not applicable, with provisions for additional pages if necessary to adequately address evaluation items.
