About this file

This document is a Vendor's Statement form required by Kentucky Revised Statute (KRS) 45A.343 for contractors seeking to do business with the City of Owensboro, Kentucky. The form serves as a compliance certification that vendors must complete and submit as part of their contracting process, requiring disclosure of any violations within the past five years and commitment to ongoing compliance with specified Kentucky statutes covering corporate taxes, sales and use taxes, income taxes, wage and hour laws, occupational safety, unemployment, and workers' compensation. The document includes signature fields for company representatives and is dated for 2025, though no specific contract details, response dates, or award timelines are provided.

The form establishes that vendors must certify they have not been finally determined to have violated any of the referenced Kentucky statutes within the five-year period preceding the statement and must acknowledge their obligation to remain compliant throughout the contract duration. Non-disclosure of violations or failure to maintain compliance can result in contract cancellation and disqualification from future City of Owensboro contracts for a two-year period. No pricing terms, funding sources, contract amounts, or set-aside provisions are specified in this compliance certification document, as it serves as a prerequisite legal requirement rather than a procurement solicitation.
