About this file
This is a sworn statement form required pursuant to Kentucky Revised Statute (KRS) 45A.395 for the City of Owensboro's Purchasing Department, located at 101 E. 4th Street, P.O. Box 10003, Owensboro, KY 42302-9003. The form serves as a mandatory prerequisite for any bidder or offeror to be considered a responsible bidder in city procurement processes. The document requires completion and submission in 2025, though no specific due dates, project details, or contract terms are specified as this is a general compliance form used across multiple procurement opportunities.
The statement requires bidders to swear under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated Kentucky's campaign finance laws, and that awarding them a contract would not violate these laws. The form includes spaces for the bidder's company name, authorized signatory information, title, signature, and date of execution. This compliance document appears to be a standard requirement for all city contracting opportunities rather than being tied to any specific procurement, with no pricing terms, set-asides, or funding information included.
