About this file

This is a Safety Plan and Record proposal document submitted by the City of Fort Worth for the Bonds Ranch Road reconstruction project (CPN 103278). The project involves the reconstruction of an existing two-lane Bonds Ranch Road into a four-lane divided neighborhood connector, including intersection improvements with traffic signals at Wagley Robertson Road and Fossil Springs Drive, extension of Wagley Robertson Road north of Bonds Ranch Road, installation of approximately 5,800 linear feet of storm drain, 750 linear feet of water line (16" and 36"), and 350 linear feet of sanitary sewer (12"), major drainage channel grading at outfalls, street lighting, pavement markings, signing, and utility adjustments throughout the corridor.

The Safety Plan proposal table requires contractors to document field man-hours, recordable incidents with and without lost time accidents, total recordable incident rates (TRIR), fatalities, and published Experience Modification Rates (EMR) for the years 2022, 2023, and 2024. Contractors must submit supporting documentation for the EMR through either a letter from their insurance agent, carrier, or government agency on official letterhead, or copies of the Experience Rating Calculation Sheets provided annually by their insurance carrier. If the Total Recordable Incident Rate exceeds 2.0 for any of the past three years, contractors are required to provide OSHA 300 logs for each of those years. This documentation is mandatory as part of the bid submittal for this City of Fort Worth public works project.
