About this file

This is a Statement of Assurances and Certifications document for the City of Bend, Oregon, associated with the Residential Meter Box Upgrade – Phase 16 project (City Project No. WA27BA, Solicitation No. 26-3559). The project involves upgrading 343 existing residential water meter box installations to current City standards and specifications, including soft and hard-scape box locations, installation of standard meter boxes, relocation of meter boxes, and correction of non-compliant connections. Bidders must certify that they have examined all bid and contract documents, understand the scope of work, and agree to furnish all necessary materials, equipment, and labor to complete the work according to specifications. The document indicates that contract award and execution must occur within seven calendar days of bid acceptance, with Performance and Payment Bonds required if applicable.

The certifications require bidders to confirm compliance with numerous federal, state, and local regulations, including Title VI of the Civil Rights Act of 1964, the Americans with Disabilities Act, Oregon Workers' Compensation Law, prevailing wage requirements under ORS 279C.800 to 279C.870 (BOLI Wage Requirements), and the City of Bend's non-discrimination policy. Bidders must declare whether they qualify as "Resident Bidders" under Oregon law, disclose any organizational or individual conflicts of interest, confirm they are not City officials or employees, and certify that their bid is genuine and not collusive. The document requires bidders to provide a CCB (Construction Contractors Board) number and years of registration, identify any outstanding claims against their firm, and provide surety company information for Performance and Payment Bonds. Bidders must also confirm they have inspected work sites, reviewed all plans and specifications, and understand all site conditions and factors affecting project completion.
