About this file
The document is a Bidder Declaration form issued by the State of California's Department of General Services, Procurement Division (GSPD-05-106 Rev 08/09). The form is designed for prime bidders to provide detailed information about their business certifications, subcontractor usage, and compliance with state procurement requirements. The form requires bidders to disclose California certifications such as Microbusiness (MB), Small Business (SB), Nonprofit Veteran Service Agency (NVSA), and Disabled Veteran Business Enterprise (DVBE) status.
The form instructs bidders to specify whether subcontractors will be used, and if so, to provide comprehensive details about each subcontractor, including their contact information, California certification status, specific work to be performed, corresponding percentage of the bid price, and confirmation of good standing. Certified bidders must demonstrate a "commercially useful function" as defined by specific California legal codes, and the form requires verification of subcontractors' valid licenses, business qualifications, and certifications. The document emphasizes transparency and accountability in the state's procurement process, with bidders required to certify under penalty of perjury that all information provided is true and correct.
