About this file
This is a Cost Proposal Form (Attachment E) for solicitation NC26-53, issued by the State of Utah Division of Purchasing through the Forestry, Fire, & State Lands department, for the HB 48 Lot Assessment Database. The RFP seeks a software tool package to assist in implementing lot assessments for high wildfire risk properties and facilitate data sharing. The resulting contract will be awarded to a single responsive and responsible vendor with the highest evaluation score as justified by procurement code. The contract term is five years, with vendor pricing guaranteed for the first year.
Offerors must submit all costs necessary to implement and maintain the proposed system over the initial five-year contract lifecycle using the standardized Cost Proposal Form. Cost categories include initial implementation, licensing, annual maintenance, initial user training, and other costs for each year of the contract. Offerors may also submit configuration, training, and future enhancement costs using specified hourly rates, and must provide complete product descriptions with cost drivers and pricing for all additional value-added services or solutions proposed. All costs must be entered in the designated yellow-highlighted cells; any costs not provided on the form will not be allowed into the resulting contract. Technical requirements and cost proposals must be reflective of one another, and any deviation from the prescribed format may result in proposal disqualification.
