About this file

This is a Cost Proposal Form (Attachment C) for Solicitation NC26-56, issued by the State of Utah Division of Purchasing on behalf of State Parks. The RFP seeks to procure golf carts designed for rough terrain courses through a competitive bidding process. Offerors must submit pricing for one new electric cart and one new gas-powered cart, each configured according to the specifications outlined in Attachment B Scope of Work. The contract will be awarded to the responsive and responsible vendor with the highest overall score as determined by the procurement evaluation criteria. The resulting contract will have a term of five years, with vendors required to guarantee their pricing for an initial 12-month period.

The cost proposal must be completed using the provided form with all pricing entered in designated yellow-highlighted cells, and any deviation from the required format may result in disqualification. Evaluation is based on two primary cost categories: Product Cost (200 points available) and Discount Cost (100 points available), with pricing assessed as outlined in the RFP. Vendors must also provide additional cost information for training, maintenance and repair services (hourly rates), parts discounts, and accessory discounts for planning purposes, though these supplemental items will not be scored in the price evaluation. Future price increases beyond the initial 12-month guarantee period must be submitted with supporting market research and require agency approval, with each subsequent increase guaranteed for one year.
