About this file

This is an Insurance Coverage Declaration form for a state and local contract opportunity issued by Letcher County Fiscal Court in Kentucky for asbestos abatement services. The declaration specifies mandatory insurance requirements that contractors must maintain and submit with their proposals for pre-demolition asbestos abatement and removal services at residential and commercial properties acquired through a voluntary property buyout grant program. Failure to provide complete insurance documentation, including all endorsements attached to the proposal, may result in proposal rejection. The contract requires contractors to be licensed by the Division of Air Quality and accredited in the State of Kentucky.

The insurance requirements include Commercial General Liability coverage with a minimum aggregate limit of two million dollars and one million dollars per occurrence; Comprehensive Automobile and Water Vehicle Liability coverage with a minimum of one million dollars per occurrence combined single limit for any auto, along with physical damage insurance for owned or rented machinery, tools, equipment, office trailers, and vehicles; and Worker's Compensation insurance with employer's liability coverage of not less than one million dollars per occurrence for each accident or illness. The Worker's Compensation policy must include a provision requiring thirty days' prior written notice to the County before cancellation or material changes. Contractors with owner/operators who have filed exemptions under applicable Worker's Compensation law must supply signed copies of the exemption notice. Proposers must complete the declaration form and provide carrier information, coverage type, limits, and expiration dates for all required policies.
