About this file

This is a Debarment and Suspension Certification form issued by the Cherokee County School District (CCSD) Procurement Services Department in Georgia for an Air Quality Testing Services RFP. The certification requires all bidders and their subcontractors to confirm under penalty of perjury that they are not debarred, suspended, ineligible, or excluded from contract awards by the U.S. Department of Agriculture or any other Federal, State, or Local Agency. Bidders must certify their eligibility at the time of proposal submission, and the District reserves the right to verify all responses. The form mandates that any bidder who becomes debarred or placed on the Consolidated List of Debarred, Suspended, and Ineligible Contractors during the bid process or after contract award must immediately notify CCSD of such status change.

Bidders unable to execute this certification will be deemed ineligible for contract award consideration. The certification is based on procedures outlined in the Federal Acquisition Regulation (48 C.F.R. Ch. 1 Subpart 9.4) and serves to protect the Government's interest by ensuring that only responsible contractors are engaged. All bidders must provide their legal company name, with signature and certification by a corporate officer authorized to bind the company. The form was last updated in August 2023.
