About this file

This is a Debarment and Suspension Certification document issued by the Cherokee County School District (CCSD) Procurement Services Department in Georgia for the Adaptive Learning Pathways Request for Proposal. The certification establishes the requirement that bidders must be responsible contractors in good standing and not debarred, suspended, ineligible, or excluded from contract awards by the U.S. Department of Agriculture or any other Federal, State, or Local Agency. The RFP solicits proposals from qualified proposers to provide a secure comprehensive instructional digital platform for both math and English Language Arts that allows for customization, personalized, and adaptive support for students. The platform must support use in centers and with small group populations, including English Immersion Program (EIP), Reading Enrichment Program (REP), English Language Learners (ELs), and Students with Disabilities (SWDs) through individualized and adaptive learning pathways. All questions regarding this RFP must be submitted through the Euna Procurement Message Center.

By signing the certification, bidders and any subcontractors they may utilize certify under penalty of perjury their non-debarred status. Bidders are required to immediately notify CCSD if they become debarred or placed on the Consolidated List of Debarred, Suspended, and Ineligible Contractors by a Federal entity at any point during the bid process or after contract award. Bidders unable to execute this certification will not be considered for award. The certification document was last updated in August 2023 and requires signature by a corporate officer with their printed name and title.
