About this file

This is a Conflict of Interest Disclosure form issued by the City of Lubbock, Texas, in compliance with Local Government Code Chapter 176. The document serves as a standardized questionnaire (Form CIQ) that vendors must complete and file when they have a business relationship with the city and meet specific conflict of interest criteria. The form requires vendors to disclose any employment or business relationships with local government officers or their family members, any gifts given to officers exceeding $100 in aggregate value over a 12-month period, or any family relationships with city officials. The questionnaire must be filed with the city's records administrator no later than the seventh business day after the vendor becomes aware of facts requiring disclosure or after beginning contract discussions or negotiations. The form lists the city's current municipal officials, including Mayor Mark McBrayer, six council members representing districts one through six, and City Manager W. Jarrett Atkinson, for whom vendors must disclose potential conflicts.

Vendors are required to disclose business relationships that result in taxable income exceeding $2,500 during the 12-month period preceding the vendor's awareness of a potential or executed contract with the city. The form specifically addresses employment relationships, business connections with entities where city officers hold board positions or ownership interests of one percent or more, and gift disclosures. Failure to comply with the conflict of interest filing requirements constitutes a misdemeanor offense under Texas law. The questionnaire references detailed provisions of Local Government Code Section 176 and excludes certain transactions from the definition of "business relationship," including those subject to rate or fee regulation by governmental entities and transactions conducted at prices available to the general public.
