About this file

The document is a Bid Schedule spreadsheet for the Dividend Phase 3 AMR/049/908 project, issued by the Utah Division of Oil, Gas & Mining's Abandoned Mine Reclamation Program (AMRP). The procurement involves 48 mine closures and revegetation of disturbed areas in Utah County, with construction activities including backfilling (both hand and machine methods), rebar grate installation, and rebar barricades across various site locations. The project has an Engineer's Estimate of $300,000, requires a mandatory site visit, and is scheduled for completion within 40 calendar days after the notice to proceed, with a final completion deadline of December 31, 2025. The bid document provides detailed site-specific information, including site IDs, opening types (horizontal, vertical, inclined), and estimated quantities for each closure activity.

The bid schedule includes a comprehensive breakdown of construction tasks with specific site tags, estimated quantities, and unit pricing mechanisms. The procurement allows for potential quantity variations, with the owner reserving the right to negotiate unit prices if they appear excessive. Bidders are required to submit both a Summary Bid Schedule and a Detailed Bid Schedule, with instructions to complete the spreadsheet and PDF documentation. The project emphasizes that revegetation costs should be incorporated into the mine closure task pricing, with no separate bidding or payment for revegetation. The contract will be awarded to the lowest cost responsive and responsible vendor, with liquidated damages provisions included in the terms.

TOTAL CONTRACT BID PRICE WRITTEN:

By submitting a bid under this solicitation, I/we acknowledge that I/we have examined the site conditions and have made the measurements and evaluations necessary to plan and bid the WORK.
Summary Bid Schedule Notes:
The total contract bid price entered here MUST match the total bid price entered in the item section on SciQuest. If the amounts are different, the bid price entered in the item section of SciQuest will be considered the correct bid and will be considered the official bid to be evaluated with other bids received.
Award of CONTRACT will be based on the base "Bid Price." Contractors are cautioned to submit accurate unit prices for any variations in quantities. Verified quantities can go either up or down from the original estimated quantities. If a unit price appears high at the time of any change order, the OWNER reserves the right to negotiate that unit price down to a reasonable amount regardless of the unit prices quoted in this section.
The "Variation in Contract Bond Rate" will be used to adjust the Lump Sum Amount for bonds when the CONTRACT amount changes (see Section 0200, Part 5.01.D).

Detailed Bid Schedule Notes:
If a unit price appears to be excessive based on the market, OWNER reserves the right to negotiate that unit price for any change order. Otherwise the unit prices will be as bid.
The "Variation in Quantity Unit Price" on the Bid Schedule will be used for adjustments to the CONTRACT amount where the actual WORK quantity varies by more than 15% from the estimated quantity listed in the Bid Schedule (see Supplemental General Condition No. 26: Variation in Estimated Quantities). The "Variation in Quantity Unit Price" will also be used as the basis for determining costs for tasks not currently specified in the WORK that may be added in the future by change order.
Sites are listed here in the same sequence as they are listed in the Mine Closure Schedule in Appendix A, that is, sorted by tag number.
Revegetation is required at mine closure locations, but is considered incidental to the mine closure task. Costs for revegetation of mine closure locations should be incorporated into the mine closure bid. No separate bidding or payment will be made for mine closure revegetation.

Tag Number: The 12-character site ID number is derived from the township, range, and section, the opening type code, and a sequential number. The initial "3" indicates that the township and range are south and west of the SLBM. Thus site 3271314HO002 is the second adit inventoried in T27S R13W Sec 14.

Opening Types:
HO= horizontal opening (adit)
IO= inclined opening
VO= vertical opening (shaft)
HC= horizontal closed (caved or sealed adit)
VC= vertical closed (capped or filled shaft)
HP= horizontal prospect (short adit)
VP= vertical prospect (shallow shaft or pit)
ES= equipment and structures

Closed features (HC, VC, HP, VP) are not considered for closure and do not have inventory data.
