1. 3-26-2018 4-3-2018 Does this project contain any Commissioning requirements?

Commissioning is not required on this project.

2. 3-26-2018 3-30-2018 Is a full-time superintendent required for this project? At all times during the performance of this contract and until work is completed and accepted, the Contractor shall provide a full time Superintendent who shall exclusively be dedicated to the supervision of this project. The Superintendent must meet all construction competency requirements, including OSHA credentialing, and have satisfactory performance as determined by the Certifying Officer. (OSHA 10 is required for any workman on a construction site and OSHA 30 is required for any Superintendent.)

3. 3-26-2018 3-30-2018 Are there any unique requirements for the As-Built Drawings/Safety Data Sheets and Security Plan?

The Superintendent shall maintain two full size sets of as-built drawings which will be kept current during construction of the project, to include all contract changes, modifications and clarifications.

The Superintendent is required to have SDS’s of all hazardous materials on site.

The Superintendent is required to maintain a Safety and Security plan. Plans need to be unique to the current project – No Generic Plans!

4. 3-26-2018 3-30-2018 What are the quality assurance requirements? It is your superintendent’s responsibility to assure your subs are installing per the construction documents. Throughout the specifications it talks about Quality Assurance – provide a QA plan!

Quality Assurance also includes coordinating the site with your subs and scheduling them to do work.

i.e. If your sprinkler sub puts his pipe where the VAV is supposed to go and makes it inaccessible for maintenance. To the VA, this is a coordination and quality assurance issue on the prime.

Also, provide coordination drawings, to include elevations, sizes of duct work, pipes, conduits, VAV’s, FCU’s etc. Don’t do the work then come back and tell us it doesn’t work! Lack of Coordination and Quality Assurance only creates delays and needless change orders.

5. 3-26-2018 3-30-2018 Are there are any unique Infection Control requirements at Iowa City?

This should be part of your superintendents daily Quality Assurance Plan.

- Neg Air Hepa Filters need to be cleaned once a week, if not more.

- Paths of egress need to be clear at all times.

- Sweep inside your construction site daily with sweeping compound.

- Insure sticky matts are available and changed regularly throughout the day.

- Contain construction waste before transport in a tightly covered container.

Draping a cloth over the container is not acceptable - it must be secured to the cart.

- Insure that the outside of your construction site is free of dust and debris. This includes, to and from your construction site; i.e. the Freight Elevator, Corridors, Loading Dock and Dumpsters.

6. 3-26-2018 3-30-2018 What are the requirements for daily logs and payrolls? The full-time superintendent should be able to provide Daily Logs that are detailed! Need to know the Name of the Sub, Name of the Employee, Where and What they are Working on and the Duration of the Work. The daily log will include your full-time superintendent’s time as well. Note: Daily logs are daily, not weekly!

Payrolls must reflect what your daily logs say.

You must provide payrolls and daily logs for everyday of the work week, even if there is no work.

17. 4-03-2018 4-13-2018 For the Amcase wardrobes & sinks, do you have a floor plan of the patient rooms these will be going in on 5West & 7West so that we can have Amcase quote the correct quantities? Or do you know room #’s and quantities that we can include with the elevations?

Floor plans, room numbers and elevations for Amcase wardrobes and sinks are shown on sheet IN12. This was further clarified in Addendum 00002.

18. 4-03-2018 4-05-2018 We have a list of items we would like the VA to review as substitution requests. Will you please review now and provide a response?

Please refer to the instructions in FAR 52.211-6 Brand Name or Equal and VAAR 852.211-73 Brand Name or Equal that are listed in the solicitation.

19. 4-06-2018 4-13-2018 There is a note on electrical drawings that states that no exposed conduit will be allowed in stairwell. Since this is a poured in place stairwell. It will be very expensive to embedded conduit in walls. All existing stairwells have exposed conduit in them.

See response to duplicate question #21.

20. 4-10-2018 4-10-2018 There is no equipment schedule on electrical drawings It is on the Mechanical Schedules on sheets M3 and M4.

21. 4-10-2018 4-10-2018 Per the Electrical Drawing Note: No surface mounted conduit & Junction Boxes allowed. Including stair tower.

Is it possible to surface mount electrical boxes in the stair tower?

Yes, it would be preferred that conduit and electrical boxes be surface mounted in the stairwell. All other conduit and boxes shall be roughed in as specified.

22. 4-12-2018 4-13-2018 Note 7 is missing off sheet EP10-09W. Electrical Keynotes incorrectly go 1-6, (skip 7), 8.

Electrical Keynote #8 should read as #7, which corresponds to keynotes on floor plans.

23. 4-12-2018 4-13-2018 In regards to the price schedule, item 3. Of the “NOTICE” at the bottom of page 6 of the solicitation states the following:

3. Prices for each Deduct Alternate shall be expressed as the dollar amount to be deducted from CLIN 0001, Item I.

But then the CLINS State (BASE BID MINUS)

CLIN 0002, DEDUCT ALTERNATE NO. 1 (BASE

CLIN 0003: $__________________________________

The instructions under the “NOTICE” seems to contradict the instructions on the bid schedule.

Can you please clarify if you want the new totals for each CLIN (Base Bid minus the work) or if you want us to list the deduct amount only?

If you want the deduct amount only, will there be a revised price schedule issued?

Notice Item #3 in the Price Schedule stating “Prices for each Deduct Alternate shall be expressed as the dollar amount to be deducted from CLIN 0001, Item 1,” shall be deleted in its entirety. The instructions under each CLIN shall be how the pricing is provided. A Revised Pricing Schedule is provided in Amendment 00004. See attached - Price Schedule - Revised 4-13-2018.

The elevation of the electrical and fiber duct banks are unknown. They will have to be field verified during excavation. The depth is graphically shown to the best of our knowledge on Detail 4/AS-1.

26. 4-14-2018 4-17-2018 Where is the contractor’s laydown area and location for construction trailer, etc?

The laydown space will have to be shared with the construction of the MRI Building. Per the contractor’s construction schedule for the MRI it will be completely enclosed in September. This will significantly free up space in Lot M for the stairwell construction. Also, once we have the pre-construction meeting for Correct Life Safety we will coordinate a meeting to discuss Laydown Space with the MRI Contractor.

27. 4-16-2017 4-16-2018 On sheet AD8-07W Demolition note 06 says:

“Contractor shall sole source contract with Prism Medical INC. to remove patient lift.”

It does not give any contact information or anything.

Who would be the point of contact for Prism INC. Do you have a number?

Installer POC: Charlie Swanson (cjswanson1992@gmail.com) #320-282-5301

28. 4-17-2018 4-19-2018 Ives Continuous Hinges are specified. On some of the hardware sets the continuous hinge needs an adjusta screw. I do not believe that is an option for Ives Continuous Hinges.

Will be addressed in Addendum 00005.

See Addendum 00005 documents posted on 4/20/2018, as part of Amendment 00005.

29. 4-17-2018 4-19-2018 What is an “integral Hinge Guard Channel“ specified in hardware set SH-3F? Would that be a Finger Guard?

This is continuous hinge with a door edge guard incorporated into one side of the hinge.

30. 4-17-2018 4-19-2018 Set 4z calls for two passage sets? Is that a mistake? It is possible on Dutch doors but usually we see one lockset on the lower leaf with a surface bolt on the top leaf.

Two passage sets for the dutch doors are correct.

These are for the corridor side of the doors to the patient room nurse servers. This matches other existing nurse server doors. Latching of both leaves are required.

mailto:cjswanson1992@gmail.com

31. 4-17-2018 4-19-2018 Set SH4A calls for an “alarm contact”. Would you have any idea what they are wanting for this?

“Alarm Contact” is defined in the “Security Hardware Abbreviations Legend” in Specification Section 08 71 00, Page 25 as a “Door Position Switch”.

32. 4-17-2018 4-19-2018 Set 12h Calls for 2 utility locks for a pair of doors. This would work only if a removable mullion would be include in the hardware set or non-removable mullion is part of the frame.

Will be addressed in Addendum 00005.

See Addendum 00005 documents posted on 4/20/2018, as part of Amendment 00005.

33. 4-17-2018 4-19-2018 Sets 1K and 4P call for two sets of Self-Adhesive Seals?

Is this Correct?

Will be addressed in Addendum 00005.

See Addendum 00005 documents posted on 4/20/2018, as part of Amendment 00005.

34. 4-17-2018 4-19-2018 What Floor Plan Page is opening TD on? TD is an abbreviation for “Temporary Door”.

This is intended for doors in temporary construction walls. The temporary construction wall locations are shown on the demolition plans.

TD is referenced on the temporary construction wall types 5A/5B on sheet AT1.

35. 4-17-2018 4-19-2018 Should we use hardware set X1 or X2 on opening TD? It says both on door schedule.

Set X1 is intended for the typical temporary construction door. The doors on floor 9 (mental health) have other requirements and are noted as set X2 in Specification Section 08 71 00.

36. 4-17-2018 4-19-2018 Note 7 on page A2-01W is indicating a display case. Is that to be hollow metal and is there an elevation for it?

It is OF/CI (Owner furnished, Contractor installed). The intent is a wall mounted display.

It is also shown on Detail 1/AS4.

37. 4-20-2018 4-24-2018 Specification section 31-6326 1.5 c-2 requires a machine with 50,000 lbs of down pressure to achieve Rock auger refusal. Can this be changed to 10,000 lbs. Very few machines will have 50,000 lbs of crowd? We have drilled the majority of the drilled piers at this site with our attachments with Kelly bars weighing approximately 10,000 lbs. Can this be changed t 10,000 lbs. ?

See attached Addendum 00006, provided in Amendment 00006.

38. 4-20-2018 4-24-2018 Specification section 31-6326 1.8-D AND 1.8G as well as foundation note f-13 refer to a load test performed on the drilled pier. Unlike piles, drilled piers are not typically load tested and doing so is very expensive. This has not been required on previous drilled pier projects at the Iowa City VA.

See attached Addendum 00006, provided in Amendment 00006.

39. 4-20-2018 4-24-2018 Bottom elevations of drilled piers are not provided.

Please provide a bid tip elevation for the drilled piers.

See attached Addendum 00006, provided in Amendment 00006.

40. 4-20-2018 4-24-2018 Foundation note F15 requires drilled pier concrete to be placed by tremie, whereas specification section 31-6326 allows free fall of concrete up to 30 feet. On past projects drilled concrete has be placed by free call without a limit on free fall distance. Can this requirement be changed to allow free fall of concrete, without a limit on the distance of fall, as long as the hole is dry or dewatered and the concrete is placed down the center of the shaft without striking the rebar or the sides of the hole? Unlimited free fall of concrete is the industry standard method of placing concrete in drilled pier foundations.

See attached Addendum 00006, provided in Amendment 00006.

41. 4-20-2018 4-24-2018 Specification section 31-6326 2.1 G requires the casing to be equipped with a cutting edge capable of coring through 10 feet or broken or solid rock. There is no need for a casing to be able to be cored through 10 feet of solid rock, nor is there any way to make such a casing. Can this requirement be deleted from the specification?

See attached Addendum 00006, provided in Amendment 00006.

42. 4-20-2018 4-24-2018 Specification section 31-6326 2.1 g lists other requirements for design of steel casing. Is there a need for designing of steel casing, especially when the casing is temporary will not be left in place? We do not design temporary casing for each project and we have not designed temporary casing for the other projects at the Iowa City V.A.. Can the language in this section be changed to simply read “supply water tight steel casing of sufficient length and thickness to withstand drilling forces and earth and water pressures”?

See attached Addendum 00006, provided in Amendment 00006.

43. 4-23-2018 4-24-2018 The 084413 Glazed Aluminum Curtain wall specification has references to “spandrel infill” but does not clarify if insulation and a backpan is required at spandrel glass conditions. Please clarify the spandrel insulation and backpan design intent for this project.

"Spandrel infill" refers to the insulating spandrel glass (IL#2) as specified in section 08 80

00. Spandrel insulation and steel backpan is only required as denoted on the drawings (detail 12/AH4 as an example).

44. 4-23-2018 4-24-2018 Will the VA be extending the bid date for this project?

Knowing if this will get extended or not will help us plan out how to use our time in the estimating department.

Our intent is to stay on course for the proposals for this project to be submitted on 4/27.

45. 4-24-2018 4-25-2018 Would you have a contact for the Hawkeye Video company mentioned on page EY3-02W Note 3? I'm having trouble locating any contact information.

It appears that Hawkeye Vision has changed ownership or partnered with Tred-Mark Communications.

Contact Info:

Robert (Gus) Gustafson Tred-Mark Communications 14775 Grover Street Omaha, NE 68144

46. 4-26-2018 04-27-2018 We are in the process of putting a number together as a sub to a general contractor and I am having a hard time finding what the asbestos abatement Davis Bacon wage rates are. Can you please provide the Davis Bacon rate and fringe for this project for asbestos removal or which laborer rate we would fall under?

Reference the attached – Construction Wage Rates – Asbestos – Johnson County – 01-05- 2018. This is from General Decision Number IA 180001, Iowa, Heavy and Highway, 01/05/2018.

The applicable rate and fringe are highlighted, along with the reference to the Zone and Group.

This Wage Determination only applies for the Asbestos laborer.
