AMENDMENT-0002, 36C78620B0002

Fort Gibson National Cemetery Gravesite Expansion Contractor Questions & GOV Responses (1 thru 74)

1. Sections 28 & 29 is designated for IGC. There is no detail as to what is required for sections 28 & 29 IGC. Please specify Sheet CS106 calls for 3,035 units of Type 4' x 4' separate from the 3,000 units of precast lawn crypts.

Are we to provide precast 4' x 4' IGC/Urn Crypt Vaults?

1.GOV Response: No Preplaced Urn crypts vaults are required for this project. Review the grading, control markers, section markers, and landscaping sheets.

2. Is specification 01 00 02 missing page 4?

2.GOV Response: See missing page 4 attached with amendment-0002 Named:

“S06_Amendment-0002_Attachment-1_Page 4_SPECIFICATIONS_1-29-19”.

3. Is the specification for 03 48 21 page footer mislabeled 09 48 21?

3. GOV Response: Section 03 48 21 is called out in the Section heading and the Table of Contents. The footer call out is incorrect.

4. Are the specifications supposed to have section 31 25 10 included twice pages 137-138 and 274-275? There are small differences.

4. GOV Response: Ignore the second section 31 25 10

5. Are sections 32 92 19 and 32 92 23 for seed and sod missing per Paragraph 2.2 in section(s) 31 25 10?

5. GOV Response: See Section 32 90 00 for specifications pertaining to sod, 2.3, A and B; and seeding 3.16.

6. Ref 31 25 10: Paragraph 1.1 B. which is correct “ODOT” or “Tribe”?

6. GOV Response: Delete Tribe from Specification.

7. Ref 31 25 10: Paragraph 2.1 A. which is correct “ODOT Prevention, Control, and Abatement of Erosion and Water Pollution” or “ODOT Standard Specification for Erosion and Pollution Control “?

7. GOV Response: The current ODOT applicable criteria is: ODOT Section 220, Management of Erosion, Sedimentation, and Storm Water Pollution Prevention and Control.

8. Please specify the expected LF and the expected location of the 7' privacy construction fencing required on this project.

8. GOV Response: The Project Work is defined by the Contract Documents. The extent of the fencing can be determined by review of the Contract Documents and on-site verification. The location is west of the adjacent existing burial section.

9. If construction fencing is placed on east side of expansion area, is the temporary / privacy fencing required as shown on drawing CS101?

9. GOV Response: Yes.

10. Is the intent to remove all trees and stumps within the stream bed and within the entire expansion area and is fleximate required throughout this area?

10. GOV Response: No, the bed and bank of the stream are not to be disturbed during construction. Such disturbance would trigger environmental and USACE permitting. The trees marked for removal in the stream bed and bank should first be verified as dead or damaged/dangerous, flagged for removal by an NCA official or representative, then cut down leaving as little a stump as possible without disturbing the bed and bank of the stream, see Demolition Sheet CD 101. Flexamat is not required throughout the expansion only at the ends of SWS structures, see CU Sheets for locations.

11. G003 indicates measured quantity bid item - Base Bid as well as Add Alternate #1. Is there Items of Work (Quantities) for the new proposed roadwork shown on CP101 as well as all work within the Proposed Expansion Area. This would be very helpful to insure no expected work is left out of lump sum required for performance of this project. Please advise.

11. GOV Response: All work shown on contract document are part of the based bid, items note in CLIN 2. The quantity schedules are for reference only final quantities for bidding are the responsibility of the contractor to verify. The quantities are to be determined by the bidder.

12. Since there are no Electrical drawings included in the bid package, can you confirm that the only electrical requirements for this project are temporary power for construction activities and power to the Irrigation Controller? Can you also tell us where power is to be pulled from and if the temporary construction activity power will need to be sub metered?

12. GOV Response: The electrical power requirements for the Project will serve the in-ground irrigation system only. Temporary construction power access point is to be coordinated with the COR.

13. How long are typical committal, burial, and cemetery activity shutdowns and what is the minimum notice you can provide the contractors prior to shutdowns?

13. GOV Response: The cemetery director will provide the weekly schedule; the contract will need to check in with the Director daily to see if any the is a change in the daily schedule. Most work can continue in the new burial areas can continue, work around the shelter will need to shut down during the service. The cemetery averages 3 services a day.

0. Also, the specifications call out for this fence to be placed around the construction limits. Can you clarify if this is required for all of the construction locations scattered throughout the cemetery grounds? It doesn’t seem feasible or necessary for construction fence to be utilized on any other areas other than the cemetery expansion.

b. See note above.

38. Drawing Sheet G-003

b. The second work item schedule is titled as “ADD ALT #1 Existing Drainage Channel Improvements”. There is no mention of ADD Alternates in the specifications or the Bid Schedule. Can you please clarify what this work item schedule is intended for?

41. GOV Response: No sand layer required per plan sheet CG504. Disregard reference in spec.

42. Chain-link Fence, drawing sheet CS101 only shows construction fence on the east side of the cemetery expansion and it is called out at 6’ tall. Can you clarify that this is the only location for construction fence and the height is what’s required?

42. GOV Response: 6’ tall construction screen fenced is required only on the east edge of expansion area as shown on sheet CS101.

47. GOV Response: Provide minimum of eight (4) images of the burial areas, No framed photographs will be required.

48. Spec 01 00 02 para 1.30.A Project Health and Safety Plan- This requirement appears to be redundant considering the Spec Section 01 35 26 requires a complete Accident Prevention Plan (APP) that is also site specific. Would the government be willing to drop the APP in lieu of a completed Site Specific Safety Plan that contains JHA's vs AHA's?

48. GOV Response: Section 01 35 26 is deleted

50. GOV Response: Spec 01 35 26 deleted

51. Spec 01 35 26 para 1.8.C Competent Person (CP). The government is requesting that the CP have "SPECIALIZED FORMAL COURSE" in their activities as a CP. Does the government consider "in house" formal course? An OSHA "competent person" is defined as "one who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them" [29 CFR 1926.32(f)] There is no OSHA requirement for them to be Specialized Formally Trained. Would the government consider following the OSHA standard or removing the SPECIALIZED FORMAL COURSE?

51. GOV Response: Spec 01 35 26 deleted

52. Spec 01 35 26 para 1.7.C SSHO- Would the government consider allowing other collateral duties to the SSHO as to maximize the efforts of all onsite employees?

52. GOV Response: Spec 01 35 26 deleted

53. Spec 01 35 26 para 1.8.D OSHA 10- Typical OSHA standards allow in house training of Safety regulations to meet the OSHA-10 requirement. Will the government consider letters from contractors that the employee has received education training on OSHA applicable standards related to the hazards of their jobs?

53. GOV Response: Spec 01 35 26 deleted

54. Spec 01 35 26 para 1.9.B Inspections- Would the government consider removing the requirement for a CSP or CSHT if a full time SSHO is required. This project is redundant in task and will allow a better cost option to the government?

54. GOV Response: Spec 01 35 26 deleted

55. Spec 01 35 26 para 1.9.B Inspections- Would the government consider removing the requirement for a CSP or CSHT if a full time SSHO is required. This project is redundant in task and will allow a better cost option to the government?

56. GOV Response: Spec 01 35 26 deleted

57. Spec 01 81 13 Sustainable Construction Requirements- This section is typically reserved for vertical construction of buildings over 5000SF. Would the government consider removing this entire section and it's reporting requirements considering the scope of work is outdoors.?

57. GOV Response: Spec 01 35 26 deleted

58. Will the VA please provide a copy of the sign-in sheet from the Pre-Bid Site Visit.

58. GOV Response: Sign-in sheet will be provided

59. The RFI deadline is Tuesday, 21 January 2020. However, the site visit was just held on Thursday, January 16, 2020, one day before a three-day federal holiday weekend. Would the VA please consider extending the RFI deadline to Thursday, January 23, 2020, to provide seven (7) days between the site visit and when RFIs are due so Contractors have time to thoroughly review the solicitation, specifications, drawings and site visit?

59. GOV Response: RFI deadline is completed.

Completed 1 page SECTION B, B.1 PRICE/COST SCHEDULE;

Completed, as many pages needed of, S02_Attachment-8 _IFB Contractor Cost Estimate Worksheet Executed 20% Bid Bond

61. Section 01 00 02, page 2, paragraph 1.1(G)(2) states that all employees of general contractor and subcontractor shall have a minimum of OSHA 10 certification. Would the VA please consider and allow the requirements to be that all general contractor and subcontractor site supervisors and/or foremen have OSHA 30 certification in lieu of all personnel having OSHA 10?

61. GOV Response: No

62. Drawing G-003 states that there is an Add Alternate for Existing Drainage Channel Improvements. However, the CLIN schedule does not include any options or add alternates. There are only two CLINs, the first being the turn-key project, and the second asking for just to furnish and deliver (i.e, not to include installation), the 3,000 pre-placed crypts. Please confirm that the note on Drawing G-003 is incorrect and should be ignored?

62. GOV Response: All work shown on contract document are part of the General Construction CLIN 01, except items note in CLIN 02. The quantity schedules are for reference only, final quantities for bidding are the responsibility of the contractor to determine. The quantities are to be determined by the bidder.

63.
