About this file

This document is a Statement of Work Attachment for a federal contract opportunity. It provides additional technical requirements for the construction of a new Radiation Oncology Building at the VA Medical Center in Lexington, Kentucky.

The key details include requirements for emergency power, a CT simulator room, procedure/exam rooms, linear accelerator vaults, supplemental spaces, and coordination with equipment manufacturers. Specific design parameters are provided for the linear accelerator vaults, such as vault dimensions, shielding requirements, and supporting systems. The design team is required to engage a qualified medical physicist for shielding design and post-installation radiation safety surveys. The contract will be set aside for Service-Disabled Veteran-Owned Small Businesses, with an estimated construction cost between $10-20 million and a 280-day design period.

SOW Attachment 08 – Additional Technical Requirements

Update of: 9/12/2024

Emergency Power

- Per TIL, it is up to each VA hospital if the dept./service/building has emergency power. However, spaces where interruption of power would be unacceptable/detrimental to patients, computers or equipment shall be fully backed up by emergency power. This new construction will require new standalone diesel generator with 96 hours of fuel.

- Facility will also require UPS to prevent power lag during switchover from normal to emergency.

CT Simulator

- The space shall include a sink, a wand type eye wash and an IV injector (Missing in current TIL)

- The floor trench from the control room to the CT machine shall be situated as close to the equipment as allowed by the manufacturer. This is to provide sufficient space for the installation of modular storage cabinets that are not on top of the trench floor box.

- This space will include a ceiling mounted bariatric patient lift. Lift will be provided by the VA under activation funding. Design team will be required to coordinate with lift manufacturer for track layout locations and supports.

Procedure/Exam Rooms (CRT08) and Exam Rooms (CRT06)

- The SEPS has the space program to have 2 exam rooms per Linac Vault. Design shall instead provided 3 exam rooms per Linac Vault based on discussions with program office.

- Rooms shall have oxygen, medical air, and vacuum.

- Power and data shall be located to accommodate a scope chair and equipment towers in one Exam Room CRT06. This room is to have power on both sides of the scope chair. One side of the equipment tower will have two (2) double stacked quad outlets.

- Two of the rooms will include a ceiling mounted bariatric patient lift; one (1) in Exam Room CRT06, and one (1) in Exam Room CRT08. Lifts will be provided by the VA under activation funding. Design team will be required to coordinate with lift manufacturer for track layout locations and supports.

Linear Accelerator (Linac) Room

- Linac vaults shall be designed to accommodate multiple manufacturer’s systems. The VA currently holds contracts with three (3) companies who can supply Linacs. Manufacturers at this time are Accuray, Elekta and Varian.

- Basis of Design - Vault Design Parameters: Radiotherapy treatment vaults must be designed to accommodate any state-of-the-art radiotherapy delivery equipment. The wall penetrations for HVAC and conduits for QA /IT cables are typically determined after the selection of equipment.

However, the difference in the requirements from vendor to vendor and type and brand of equipment are minimal. In the event equipment has not been selected by the VA, the basic requirements for the vault are as follows:

o The vault will be constructed using regular density concrete (typically, 143 lbs./cubic foot). The use of high density concrete due to space limitation is also acceptable o Maze less vault with electrically operated shielded door o Highest photon beam Energy; 15 MV o Highest electron beam energy; 18 MeV o Photon beam dose rate range; 600-2400 MU/min o Largest field size at isocenter; 50 cm diameter o Finished wall/ceiling dimensions at isocenter; 28 feet wide, 26 feet deep, and 12 feet height.

o Vault shielding to comply with permissible radiation levels in controlled and uncontrolled spaces around the wall.

SOW Attachment 08 – Additional Technical Requirements

Update of: 9/12/2024

- Design team shall work closely with Linac manufacturer to ensure construction contractor provides all required utilities for a successful equipment installation.

- The A/E shall engage a Qualified Medical Physicist who has demonstrated expertise in structural shielding design and evaluation for megavoltage radiotherapy facilities. The room's shielding should be designed by this expert in partnership with the DB firm. This design should be reviewed whenever changes are made to the room or the surrounding space. A post-installation radiation safety survey must also be performed by the same to ensure the space and shielding have been constructed properly in accordance with state and federal regulations. All reports and certifications must be provided to the VHA National Radiation Health Physics Program (NHPP).

- Linac vaults are to have ceiling mounted bariatric patient lifts. Lift will be provided by the VA under activation funding. Design team will be required to coordinate with lift manufacturer for track layout locations and supports.

- There shall be two control rooms, one for each Linac vault. The counters in the control room shall be at standing height. DB to verify with COR.

- Rooms will include cabinetry along walls to house all equipment. Vault walls shall have finished drywall including behind the cabinetry.

- Shield wall penetration is to be sized 15% larger than required to accommodate future needs.

- AE shall verify location of isocenter with their physicist and selected manufacturer.

- The vault must be equipped with a TV system for remote viewing of the patient.

- An intercom system shall be provided to maintain voice contact with the patient.

- A “Beam On” indicator light must be installed over the entrance door and one inside the room.

- The entrance door shall be interlocked so that if the door is opened the accelerator cannot be turned on.

- A physics port must be installed and shall be a 4” lead lined conduit running between the control room and treatment room for placement of physics equipment cables. The conduit is to be located through a secondary barrier wall and must be angled into the room approximately 45 degrees and placed in the corner of the treatment room. Before concrete pour, verify position with the DB’s physicist. Cabinetry design in the treatment room shall be designed such that the conduit terminates inside a closed cabinet where the physicist equipment will be housed. The elevation of the conduit shall be just below counter height so that equipment cables can be run up through a counter or cabinetry and placed on the surface.

- A rigging path study shall be conducted after 35% design to assure door sizes are adequate and all turns can be accomplished. Chillers servicing the Linacs will be purchased under the equipment contract and will not be required to be sized by the design team. Design team will be required to coordinate with equipment manufacturer to ensure all accommodations are made for the Linac system depending on the location of the chiller whether located on the roof of the structure or at grade.

- A network of cameras and positioning will be part of the manufacturer’s network equipment.

Construction contractor should plan to install conduit and wiring back to control panel. These locations will need to be coordinated by the design team with the equipment manufacturer.

Supplemental Spaces

- Supplemental associated spaces shall accommodate crash carts, computer on wheels (COW), blanket warmers, uninterrupted power, medicine cabinets/refrigerators (controlled and uncontrolled), and power conditioning among other typical operational components. These spaces will require power, data, and temperature monitoring as applicable for that space and component.
