Part 3 - Site Analysis
Siting Option 5
Relocate SPS to 15th Floor
9,600 SF Renovation Total
Fourteenth Floor Plan Fifteenth Floor Plan
Location of vertical shaft for new cart conveyor/elevator system.
Boundary of Work Above:
Fourteenth Floor Clinics include On-call services below on 14th Floor.
Existing fl oor area requires structural reinforcement.
Overview
Option 5 looks to use a portion of the 15th fl oor for SPS. The existing fl oor is currently unoccupied and could be re-confi gured without displacing other departments. This option would not require any exterior alterations except for any louvers or grilles for mechanical systems. It does represent a challenge in that it is on the fi fteenth fl oor. All materials would need to move through Building 200 or be lifted to the fl oor where there is no exterior landing point. Electrical systems would need to be extended from the lower fl oors. Being an elevated fl oor, the building envelope could be enhanced to improve its performance. The ease of construction and phasing is off set by the lack of direct adjacency to the OR Suites. A dedicated soiled cart elevator in an abandoned shaft could provide access to all fl oors. Separating the SPS from all other departments and placing it on a separate fl oor will decrease the situation of contamination. The existing concrete fl oor will need to be reinforced with carbon fi ber to support the new fl oor load. The fl oor below will need to be vacated during this installation process.
Pros
This is located on an abandoned fl oor which reduces phasing and constructability issues.
All work is isolated to the fi fteenth fl oor and the fourteenth fl oor ceilings.
Adjacent to the main elevator core.
Does not increase footprint of the building, no site work.
Cons
Not connected directly to the surgery.
Structural slab reinforcing required for storage areas.
Assessment Factors
Architecture: Score 4. This option is primarily an interior renovation and therefore there are no new “forms” on the outside. However, mechanical louvers need to be added and arranged to not be on the front of the building or incorporated to minimize the impact to the building architecture.
Civil Engineering and Existing Site Conditions: Score 3. The project is internal, consisting of interior renovation only. No site access for construction is anticipated.
Constructability: Score 3. The fl oor is vacated allowing work to be completed in a single phase with limited disruptions. Disruptions to the areas beneath the renovation for the addition of structural bracing requires operational shutdowns of areas below.
Cost Estimation for Design and Construction: Score 5. $7.9M which would be a NRM project. This project is the interior renovation of interior, vacated space. There is no exterior construction or temporary utility requirements. The cost does include structural reinforcement of the fl oor slab.
Electrical: Score 5. The electrical needs shall be accommodated within the building’s existing service. Tele/ data service fi ber to new project IT closets will homerun back to the main computer room located in Building
1. Fiber will be routed from Building 1 through the existing connecting corridor into Building 200. Fiber will then be routed above accessible ceilings and up existing shafts to the area of work. Electrical service for emergency critical and equipment power can be derived from new distribution panels located in Building
200’s south basement emergency electrical room adjacent to electrical service entrance room. Power conduits can be routed in the crawlspace below this room and will then be routed up an existing shaft to the area of work. Normal and life safety power can likely be derived from a power source in this area.
Services for low voltage systems, including but not limited to nurse call, fi re alarm, PACS, Biomed, paging, etc. will be tied to local headend equipment near the area of work. Scope of work shall be coordinated with the work being completed in the VA’s ERHM Infrastructure Upgrade project.
Energy: Score 4. The project is an interior renovation with an exterior wall that can be insulated, increasing the r-value in this location which increases building effi ciency.
Environmental Design: Score 5. No environmental impact.
Function and Adjacencies: Score 1. The unoccupied fl oor allows for maximized operational layout of the relocated department for internal workfl ow. The location is not directly located adjacent to the surgery department and all materials will be required to be transported vertically through the building. There is a centrally located shaft that is proposed to be outfi tted with a new cart elevator for soiled material to reduce travel time of scopes and instruments from other fl oors in Building 200.
Infection Control: Score 1. This option requires all material to be transported vertically through the building.
The size of the available square footage will allow for the space to be designed to maximized operational processes and separation of workfl ows.
Edward Hines, Jr. VA Medical Center | 64
Mechanical: Score 0. The option off ers a potential Penthouse Roof AHU. Passage through the Penthouse may off er a location for air valve and reheat coil service without aff ecting the User space. Connection to utilities located above the 15th Floor is more challenging than if it were located on lower fl oors. The existing
8” chilled water lines are slightly beyond the allowable limit. Cooling coil booster pumps (N+1) would most likely be required to serve the new AHU. The existing 5” high pressure steam line will be suitable for this option. The existing rooftop AHUs are to remain. Airside services to the 15th Floor are to be removed from the two (2) existing rooftop AHUs, where services cross into the new area of work. Hoisting of rooftop equipment to the Penthouse is a disadvantage. The new rooftop equipment locations will ensure more square footage for the Users. Refer to the Appendices for relocation of mechanical equipment and piping services that are required for this option. Reference appendix for drawing VA Project # 578-07-115, Owner
Review dated September 8, 2009.
Phasing of Construction/Schedule Development: Score 2. This option shall be completed in a single phase, aff ecting the fl oor below for overhead sanitary runs. Over head structural and sanitary work is proposed to be phased for nighttime work within the over project.
Plumbing: Score 1. Plumbing service shall be routed to the new project location from within Building 200.
Extensive new sanitary and above ceiling work will be required to be completed in outpatient areas below.
Work would be typical of any addition/interior renovation work. Water treatment will be required in equal measure in all proposed locations. Fixture and fl oor drain piping will require disturbing Level 14 spaces below. The existing plumbing services within and nearby this site should be suffi cient to support the new
SPS.
Security: Score 3. Interior Only, no physical security issues on the exterior.
Structural: Score 1. The existing concrete fl oor will need to be reinforced with carbon fi ber to support the new fl oor load at storage areas and the fl oor below will need to be vacated during this installation process.

First Floor Plan Second Floor Plan
Location of vertical shaft for new cart conveyor/elevator system.
PACU
SPS DEPARTMENT
Existing fl oor area under
SPS storage areas to require structural reinforcement.
OR unrestricted, semi-restricted and restricted circulation to be revised to address reconfi guration of PACU and addition of SPS Department.
Boundary of Work Above:
First Floor Clinics include
Entry to BLDG 200, Pathology, Radiology, and
Eye Clinic.
Option 6 moves the PACU to where central sterile storage is currently and locates the SPS where PACU is currently. This places SPS contiguously to the OR Suite while centrally locating PACU. This option will also plan on a soiled cart elevator and new soiled utility room in the core running the full height of Building 200.
The larger footprint for SPS will allow for mechanical systems to be internal, except a new air handler. The existing concrete fl oor will need to be reinforced with carbon fi ber to support the new fl oor load. The fl oor below will need to be vacated during this installation process. The surgery and SPS departments will share locker and lounge areas. Anesthesiology space shall be consolidated and downsized to accommodate the addition of SPS on the fl oor.
Pros
Directly adjacent to the OR Suite.
Relocates PACU between the two main banks of OR’s.
Interior Renovation, does not require exterior work.
Cons
Total project cost is greater due to increased size because PACU needs to move.
Structural slab reinforcing required for storage areas.
Adjacent to the main elevator core.
Does not increase footprint of the building, no site work.
Assessment Factors
Architecture: Score 4. No changes to the exterior of the building other than a new air handler unit will impact the architecture of Building 200.
Civil Engineering and Existing Site Conditions Score 3. The project is internal, consisting of interior renovation only. No site access for construction is anticipated.
Constructability: Score 1. Disruptions to the areas beneath the renovation for the addition of structural bracing will require operational shut down of areas below. The project also requires the temporary relocation of surgery central support space, the construction and relocation of PACU, and the construction of the SPS department and surgery central core support. These items are above and beyond typical construction expectations.
Cost Estimation for Design and Construction: Score 5. $11.8M which would be an NRM project. This project involves all interior renovation. The project creates a new PACU in the area of the existing surgery support services and then creates an SPS area in the place of the existing PACU.
Electrical: Score 5. The electrical needs shall be accommodated within the building’s existing service. Tele/ data service fi ber to new project IT closets will homerun back to the main computer room located in Building
1. Fiber will be routed from Building 1 through the existing connecting corridor into Building 200. Fiber will then be routed above accessible ceilings to the area of work. Electrical service for all branches of power
(normal, life safety, critical, equipment) can be derived from new distribution panels located in Building
200’s south basement emergency electrical room adjacent to the electrical service entrance room. Power conduits can be routed in the crawlspace below this room and will stub up into the area of work.
The project is an interior renovation with an exterior wall that can be insulated from the interior and provided with new gypsum board fi nish, increasing the r-value in this location which increases building effi ciency.
Environmental Design: Score 5. No environmental impact.
Function and Adjacencies: Score 3. The second fl oor Surgery department is the largest service line SPS serves. Having SPS located on the same fl oor as surgery reduces regulated pick-up and processing time by not requiring transportation of material vertically through the building. Reducing this traffi c also reduces overall vertical traffi c through the building. The adjacency also reduces risk of contamination of sterile supplies or transmission of infection through transport of dirty materials. This project requires the relocation of the PACU suite which would be an independent project to be completed prior to the SPS construction. This relocation would require temporary sterile storage to be accommodated during the construction of both projects, the PACU relocation and SPS construction.
Edward Hines, Jr. VA Medical Center | 70
Infection Control: Score 1. Construction of a new PACU and SPS on the second fl oor will require extensive reconfi guration and temporary phasing to maintain operations for the surgery and PACU department.
Construction in and around a surgery department is always full of risk due to increase in persons in the area, construction dust and temporary shutdown of utilities. This option improves infection control by limiting the time between dirty pick-up and travel distance of dirty and sterile instruments. The size of the available square footage will allow for the space to be designed to maximized operational processes and separation of workfl ows.
Mechanical: Score 3. An air handling unit is anticipated to be required to be added to support modern air quality and modern HVAC standards for sterile processing. Exhaust capacity will also need to be increased.
This need is universal of all ten options. New units shall be placed on the 1st fl oor roof with other existing units. This option has available roof locations for air handling units (AHU) to be placed on the 2nd Floor
Roof, located adjacent to the new cooling towers on the West side. Three separate AHUs to be provided in this option. One (1) AHU will be required to be an indoor unit. Chilled water and steam services will require more connections from the central systems than other options, using a combination of rooftop and indoor piping. This portion of area that is a new location, will not require modifi cations to existing HVAC systems.
The 11’-9” slab-to-slab height is a highly notable restriction.
Phasing of Construction/Schedule Development: Score 1. This option will require multiple phases, aff ecting multiple fl oors such as the fl oor below.
1. The central OR support area will be required to be temporarily relocated for the construction of the new PACU.
2. A new PACU will be constructed and operations relocated.
3. The existing PACU space will be renovated to support new SPS suite. OR support services should also be relocated to their new locations at the end of this phase. This will complete the project.
Plumbing: Score 1. Plumbing service shall be routed to the new project location from within Building
200. New sanitary and above ceiling work will be required to be completed in outpatient areas below.
Work would be typical of any addition/interior renovation work. Water treatment will be required in equal measure in all proposed locations. Fixture and fl oor drain piping will require disturbing Level 01 Pathology and Radiology spaces below as well as requiring a piping connection to the existing site sewer systems. The existing plumbing services within and nearby this site should be suffi cient to support the new SPS.
Security: Score 3. Interior Only, no physical security issues on the exterior.
Structural: Score 1. The existing concrete fl oor at the location of the new SPS area will need to be reinforced with carbon fi ber reinforcing to support the new fl oor load and the fl oor below will need to be operationally vacated in a phased manner to allow for the completion of this work.

Basement Floor Plan
Location of main utilities; water, electrical, and steam, coming into the building. Space is an open area well.
Location of area well.
Chapel exit will need to be coordinated with addition.
Option 7 creates a 2-story addition on the south side of Building
200 in an area that is currently a staging area for other projects and ground mounted utilities but will be partially returned to a parking lot in the future. Unlike Option 4, both the fi rst and second fl oors will be accessible from within Building 200. The addition will require a soiled, clean elevator, and staff elevator for transport within the addition. A separate soiled utility room and soiled cart elevator in the building core will be required for all the options. It is not optimal to split the department vertically but the adjacency to the OR Suite is desirable.
The addition will need a connector from the addition to the second fl oor core corridors. Some spaces may be lost on the second fl oor to make the connection. A secondary connection will be located on the fi rst fl oor.
Pros
New construction.
Ability to adjust building footprint.
Potential simplifi cation of phasing.
Cons
Awkward relationship with 2nd fl oor core access.
2 story design requires additional cart/staff elevators.
Site utilities confl ict.
