About this file

This is a Fire Protection Equipment Performance Report documenting the initial fire alarm system inspection conducted at the VA Outpatient Clinic located at 351 E Temple Street, Los Angeles, CA 90012 on August 28, 2025. The facility is a five-story business occupancy building constructed in 1973 with 105,000 square feet, featuring a fully sprinklered outpatient veterans clinic with secondary occupancy as parking (S-2). The building includes two levels below ground, four passenger elevators, two freight elevators, five stairways, a fire pump, and an emergency generator.

The fire alarm system utilizes a Notifier 30-30 Fire Alarm Control Unit located in the FACP room with an annunciator panel in the lobby. The system is monitored by Valley Alarm central station (Account #LZ6303, Phone: 818-787-0999) via a separate cell dialer. The initial test identified seven defects requiring correction within 30 days: (1) no Sequence of Operations matrix posted at the fire control panel; (2) no breaker identified for the fire alarm panel during secondary battery power testing; (3) dedicated fire alarm circuit not verified or identified; (4) dedicated circuit not locked or secured; (5) RPS signal continuing to display supervisory and "other" status despite normal panel operation, requiring troubleshooting; (6) a broken butterfly valve supervisory switch in room 211; and (7) a failed horn/strobe in the second floor elevator lobby. Corrections must be documented on Form F-340RT and submitted to the Los Angeles Fire Department via the Compliance Engine within 30 days of the initial test completion.
