Sandra Fusco Amendments to the Solicitation
1. Section B.2, Schedule of Services and Price is deleted in its entirety and replaced with the attached document titled Amendment 0001 to RFP 36C26221R0082 Amended Section B.2, Schedule of Services and Price.
2. Section D.3 Price Worksheet is deleted in its entirety and replaced with the attached document titled, Amendment 0001 to RFP 36C26221R0082 Amended D.3 Price Worksheet
3. Para 24.4 is amendment to read, “for each of the Arizona health care systems covered by this contract.”
4. Paragraph 10.8 of the PWS, first sentence, is amended to read, “The Contractor shall distribute VA provided Oxygen Conserving Devices that are battery-powered or pneumatic powered devices that limits oxygen flow to inspiration only and thereby increase the duration of the supply.
Questions and Answers The following are answers to questions received in response to RFP 36C26221R0082 for In-home Oxygen and In-home Ventilator Services for Phoenix VA Health Care System (PVAHCS) and Northern Arizona VA Health Care System (NAVAHCS) (Prescott)
Question 1 (Q1). Line Item Number 0027 for the base year and all option years in section B.2 Schedule of Services and Price state the following: “Fee for deliveries/pick-ups made for over 100/150 miles”. However, attachment D.3 Price Worksheet, for the same line item, states the following: “Fee for deliveries/pickups made for over 100 miles from the closest Contractor facility or VAMC/CBOC to the delivery location, whichever is closer. HCPCS: A9901” Please clarify if it’s 100 or 150 miles.
Answer 1 (A1). This has been amended to read 100 miles on the amended section B.2 Schedule of Services and Price.
Q2. Ref para 11.3 Reconditioning/Repair of Government Owned Equipment - Is it the government’s intention to have the contractor repair and recondition at their own expense or will they be reimbursed by the VA?
A2. Per paragraph 11.3 Reconditioning is at no charge to the Government.
Q3. Ref para 10.8 Oxygen Conserving devices - Will the VA be providing the contractor with the conserving devices to distribute the patients or will the contractor be responsible for purchasing the conserving devices?
A3. This paragraph has been amended to read that the VA shall provide the device and the Contractor shall distribute the device.
Q4. Ref Section 2.7 Patient Visitations - Will the contractor be able to bill for a concentrator servicing and a patient visit by a technician for the same visit?
A4. CLIN 019 should be billed for the patient visitation described in Section 2.7.
Q5. Ref Section D.3 Pricing Worksheet for Prescott Item A001PRE - Month Quantity is indicated to be 1300. Is this correct? How many patients are currently on service for this VA?
A5. Per paragraph one (1) of the PWS, there are an estimated 1600 patients for in-home oxygen.
Q6. Ref Section D.3 Pricing Worksheet for Prescott Item A022PRE - How many Ventilator Patients are currently on service with the VA?
A6. Per para one (1) of the PWS, there are an estimated 5 patients
Q7. Ref Section D.3 Pricing Worksheet for Phoenix-item A002PHO & Prescott-item A002PRE - Is it the VA’s intent for these CLINs to be for rental of portable oxygen systems vs what is stated currently as portable oxygen concentrator system?
A7. CLIN 002 for both PHO and PRE are for government owned equipment. Rental is under CLIN 005
Q8. It appears, based on the pricing schedule, the Prescott VAMC already own their concentrators. Does the Prescott VAMC already have in place VA-owned concentrators in the Veterans homes? If not, when do you anticipate VA-owned concentrators in the Veterans homes for the Prescott VAMC?
A8. No, the Prescott VAMC does not already have VA owned concentrators in the Veteran’s homes. The expectation is to have VA owned concentrators drop shipped to the contractor at the beginning of the contract and have them placed in the homes during the transition period at the beginning of the contract.
Q9. It appears, based on the pricing schedule, the Phoenix VAMC won’t own their concentrators until the first option year of the contract. If the VA wants to switch to VA-owned concentrators, can the switch be done during the transition period of the new contract? Or will the VA consider purchasing the contractor owned concentrator already in the patient’s home?
A9. Yes, the switch can be done during the transition period of the new contract. VA will not purchase the contractor owned concentrator already in the patient’s home.
Q10. Ref Page 16 2.5.2 = Would the VA further elaborate on “ to a designated discharge location”; home, hospital, clinic, patient room, outside facility? Will there be any in hospital tank deliveries or are discharge tanks only being setup at the home?
A10. The majority will be setup in home, but there is a potential to deliver in hospital for discharge.
Q11. Ref Page 29 16.1 = The transition shall include the transfer of all inventory records. Is this transfer going to be electronic or paper record transfer?
A11. Electronic.
Q12. Ref Page 33 23.7 = If vendor owned equipment is out, but patient is discontinued the VA will not pay for the recovery of equipment?
A12. No, all costs associated with recovery will be borne by the contractor.
Q13. Will the servicing VAHCS' give consideration to transitioning all patients on tanks to portable oxygen concentrators?
A13. No. This is a clinical determination.
Q14. 1. In the pricing table on the Excel worksheet, the CLIN item numbers for the Base year start with A001PHO and A001PRE for the two VAMCs. Each option year afterwards starts the same way, without changing to reflect the year. Should the number change each year, as it does in the price tables in section B.2 (example= 0001, 1001, 2001, 30001, 40001, 5001, 6001, 7001)?
A14. Price Worksheet has been amended.
Q15. CLIN # A002PHO and A002PRE state delivery, initial set-up, and education of portable oxygen concentrator system. Portable system for new set-ups consists of “E”, “C”, “D”, or “B” size cylinders with regulator and supplies. It indicates that this line item is for the set-up of portable concentrators (POCs) and portable oxygen systems with tanks. Typically, these would not be on the same line item as they are completely different systems. Can you verify that this CLIN number is intended for both portable concentrators and portable tank systems?
A15. Yes, these CLINs are intended for both portable concentrators and portable tank systems.
Q16. CLIN # A004PHO and A004PRE are for the rental of stationary oxygen concentrators with a flow rate of 0-5 and 0-10 liters per minute. These are 2 separate types of concentrators. The cost of the 10 Lpm unit is twice the cost of the 5 Lpm unit. Typically, the rental of these two separate types on concentrators have two separate CLINs. Would you consider splitting the 5 and 10 lpm units into separate item numbers?
A16. Yes, see amended Schedule of Services and Price and amended Price Worksheet.
Q17. Based on the estimated quantities provided on the prices sheet, it appears that the Phoenix VA owns 3% (50) of the home oxygen concentrators and the vendor own 97% (1,450) of the concentrators. However, the quantities show the Northern Arizona owns 97% (1,300) of the concentrators and the vendor only own 3% (40) of the concentrators. Is this correct? If so, why is it the opposite at the two VAMCs (one owns most concentrators, and the other owns very little).
A17. See amended Price Worksheet.
Q18. Does the VA intend on keeping the quantities/ percentages of VA owned equipment roughly the same throughout the contract, as it related to the percentage of equipment owned by the Vendor and the VAMC? We would not want to price the bid based on us purchasing 1,450 oxygen concentrators up front just to have the VA buy them directly and return our units a year or two later.
A18. No, the intention is to have the majority of the concentrators VA owned.
Q19. In section 2.4.1.2 it states items included in supplies to be provided at no additional cost consist of oxygen cannulas and pendant cannulas. Are you referring to an Oxymizer when you say pendant cannula? Oxygen cannulas are considered a standard supply in all VA contracts. However, Oxymizers (pendant and mustache style) are not considered standard supply. These cannulas cost roughly 10 times the amount as a standard cannula. If you intend for Oxymizer pendants to be included, would you consider adding a price CLIN for this item?
A19. No, this is not referring to an Oxymizer. VA can purchase Oxymizers for distribution to patients.
Q20. In the second paragraph of section B.2, it states the maximum value of this contract is $44,000,000.00. Is that amount intended for the standard 5-year contract or do you feel the amount is appropriate for this 8-year contract?
A20. Per the updated Section B. Schedule of Services and Price, this amount has been updated to read $50,000,000.00.
Q21. Would it be possible for you to provide a list of patients broken down by county and city for each of the VA Medical Centers?
A21. That information is not available.
Q22. How many 10-liter concentrators are currently being provided by the vendor for each VA?
A22. That information is not available.
Q23. Line Item A019PHO and A1019PRE states it is for home oxygen visits and assessments. Does this include non-scheduled visits for tank deliveries, or is this just intended for 6-month visit?
A23. This CLIN is for the intended 6 month visit and replacement of equipment as needed.
Q24. If there is a current or previous contractor who has performed similar services, can you please provide the incumbent contract or task order number?
A24. Rotech is the incumbent provider under contract VA258-16-D-0142.
Q25. Reference para 2.3 Backup System, Veterans on continuous mechanical ventilation require two (2) ventilators on hand. May the Government please clarify if it’s intent is for continuous ventilator patients to have a total of three (3) ventilators in the home (i.e. 1 primary vent and 2 back-up vents) or a primary and back-up only?
A25. Primary and back up only.
Q26. Ref para 24.4. states, “there will be one COR for each of the California healthcare system covered by this contract.” May the Government please clarify if it was intended to reach “each of the Arizona healthcare system”?
A26. Para 24.4 is amendment to read, “for each of the Arizona health care systems covered by this contract.”
Q27. Ref price worksheet, A001 Delivery, initial set-up, and education of stationary oxygen concentrator system. May the Government please clarify if all disposable supplies provided at the initial time of setup will be borne of the contractor or provided by the Government?
A27. The cost for all disposable supplies will be borne by the contractor.
Q28. Ref price worksheet A003 Preventive Maintenance (PM) for Government-Owned concentrators. May the Government please provide the make/model of the Government-Owned concentrators to be used for the provision of the contract?
A.28. Invacare Platinum 10 Concentrators – Model 9153642105, SimplyGo Portable Concentrators – Model 16088987.
Q29. Reference price worksheet A005 Monthly Rental of portable oxygen concentrator system. May the Government consider updating the HCPCS from E0431 (Portable Gaseous O2) to E1392 (Portable Oxygen Concentrator)?
A29. Yes, the correct HCPC is E1392, see amended price worksheet.
Q30. Reference price worksheet A006 Delivery, set-up, and education of self-fill oxygen system to include 4 self-fill compatible oxygen tanks. May the Government please clarify if the 4 self-fill oxygen tanks provided with the Government Owned self-fill system will be provided from Government inventory or Contractor borne?
A30. Contractor borne for tanks.
Q31. Ref price worksheet A007 Rental of self-fill oxygen system to include 4 self-fill compatible oxygen tanks. May the Government please clarify if patients will receive more than 4 self-fill cylinders and if so, consider adding a CLIN for additional self-fill oxygen tanks when requested for the patient?
A31. The Government will not add an additional CLIN.
Q32. Reference price worksheet A008 Stationary Cylinder set-up, size H or M60. May the Government please confirm A008 may be billed in conjunction with A001 at all initial setups for the use of back-up?
A32. No, it may not be billed in conjunction with 001. 008 is the charge for the initial set-up of the stationary cylinder and includes regulator, flow meter, safety stand, humidifier and disposable supplies. This is a one-time charge for set-up. (This can be used as a back up system or as a primary system). Includes delivery, setup, and education.
Q33. Reference price worksheet A020 Delivery, initial set-up, and education/training of ventilator for continuous and/or intermittent ventilation. May the Government please confirm the primary and back-up ventilators will be Government-Owned equipment?
A33. The primary and back-up ventilators will be Government-Owned equipment unless none are available, then a rental would be requested.
Q34. Reference price worksheet A020 Delivery, initial set-up, and education/training of ventilator for continuous and/or intermittent ventilation. May the Government please clarify if supplies provided at the time of setup are borne of the Government or Contractor?
A34. Supplies are borne by the contractor.
Q35. Reference price worksheet A022 Monthly Licensed Respiratory Therapist (LRT). May the Government please confirm CLIN A022 will be utilized for all monthly LRT visits, regardless of equipment being Government-owned or contractor owned?
A35. Yes, CLIN 022 will be utilized for all monthly LRT visits for patients on ventilators only, regardless of equipment being govt. owned or contractor owned.
Q36. Reference price worksheet A022 Monthly Licensed Respiratory Therapist (LRT). May the Government please clarify if supplies issued by LRT during monthly visit to patients on Government-owned equipment will be borne of the Government or contractor?
A36. Supplies issued will be borne by the contractor.
Q37. Reference price worksheet A027 Fee for deliveries/pickups made for over 100 miles from the closest Contractor facility or VAMC/CBOC. May the Government please clarify CLIN A027 will be used for any delivery and/or pickup beyond 100 miles, in conjunction with another CLIN (i.e. A001, A004, etc.)?
A37. Yes, this CLIN shall be used in conjunction with other CLINs.
Q38. Reference section E.1 52.212 Instructions to Offerors. May the Government please confirm there is no “Technical Factor” that needs a response from the Offeror? Usually there are specific questions that must be answered on how the Offeror will meet the performance of the contract.
