About this file
This document is a Proposal Materials and Instructions attachment that outlines submission requirements and evaluation criteria for the Roseburg Health Care System Single Award Task Order Contract (SATOC) construction solicitation.
Offerors must submit proposals in two volumes, each limited to 50 pages maximum. Volume I requires technical capability demonstration through three to five project examples, with mandatory inclusion of projects reflecting the seed project scope, experience in active medical spaces, and infection control measures. All projects must be at least 50% complete, performed as prime contractor, and demonstrate concurrent project management capability. Volume II requires pricing submission including completed SF1442 form, any amendments (SF30s), representations and certifications, an offer guarantee/bid bond, completed Task Order Pricing attachment with proposed percentages, and a fully priced seed project using proposed rates and calculated as 100% self-performed by the prime contractor with no subcontractor pricing included. Past performance references, between three and five submissions, must correspond to technical projects and demonstrate experience managing multiple subcontractors and stakeholder coordination. The Government anticipates making two awards (one for seed project, one for SATOC contract) and intends to award without discussions unless deemed necessary. Evaluation utilizes three factors: Technical (rated Exceptional to Unsatisfactory), Past Performance (assessed by relevancy and confidence level), and Price (evaluated for reasonableness and unbalanced pricing). Non-price factors are weighted as significantly more important than price and approximately equal to each other. Recent performance is defined as within the past five calendar years, and relevant experience requires construction or alteration in completed clinical/hospital/medical spaces with minimum contract value of $200,000.

Proposals received that are not compliant may be removed from further substantive evaluation at the discretion of the Government.
All proposals shall be submitted in two Volumes. Each volume shall be no more than 50 pages.
Volume I: Technical and Past Performance.
1. Factor 1: Technical.
a. The Offeror shall demonstrate recent and relevant experience in repair, maintenance, and construction at existing facilities and/or infrastructure, in accordance with the applicable scope of work, construction drawings, and specifications.

ii. At least one project must demonstrate experience in an active medical space.
iii. At least one project must demonstrate experience requiring the application of infection control measures.
b. The Offeror shall provide no less than three (3) and no more than five (5) examples of their experience. Comment by Thornton, Andrew S. (Portland): Should we name the form and tell them the name here? Comment by Thornton, Andrew S. (Portland): Stupid Question: Do we allow them to modify the form? i.e. making it longer to add more words, which would make us read more. Comment by Valentine, Meredith F.: I would say no to letting them alter the form. If we are going to do that, why provide the form?
i. All projects submitted must be at least 50% complete if not fully completed within the recent time span.
ii. The project must be completed by the offeror as the prime contractor for the project.
iii. Submitted projects should demonstrate contractor’s ability to work multiple projects concurrently.
c. In describing the projects, major or critical aspects of the requirement(s) to demonstrate the construction experience must be provided.
2. Factor 2: Past Performance References
a. Offerors may submit no less than three (3) and no more than five (5) past performance references, (corresponding to the projects submitted for technical), of work as a prime contractor responsible for managing multiple subcontractors and coordinating with multiple stakeholders. In describing the effort, provide major or critical aspects of the requirement to demonstrate relevancy.
b. Offerors may submit PPQ’s or CPARS to demonstrate relevant efforts.
c. If the Offeror has no past performance, it shall be indicated.
Volume II: Price and Reps/Certs.
1. SF1442, completed and signed. Note: Contractor’s signature shall be in accordance with FAR Part 4.102 (e.g., proper signature and format of person who is able to bind an individual, partnership, corporation, etc.).
2. Amendment(s). Signed SF30(s), if any issued, and/or acknowledgement in Box 19 of the SF1442.
3. Representations and Certifications. Provisions or clauses incorporated by full text in this solicitation requiring completion by the offeror must be completed and submitted with the offer.
4. Offer Guarantee. Failure to furnish an offer guarantee (also referred to as a bid bond or bid guarantee) in the proper form and amount, by the time set for receipt of proposals, may be cause for rejection of the proposal.
5. Complete and return the red blocks on Attachment 5 - “Task Order Pricing.” The submitted percentages will be used in each task order resulting from the Single Award Task Order Contract for the Roseburg Health Care System.
6. Fully completed price schedule for provided seed project (must use proposed rates in the submitted Attachment “Task Order Pricing”). Do not include any subcontractor pricing. Must be priced as if 100% self-performed by Prime Contractor.
EVALUATION
The Government may reject any or all proposals. Any informalities or minor irregularities in received proposals may be waived at the Government’s discretion.
The Government will evaluate offers in response to this solicitation in accordance with the evaluation factors below. The Government anticipates making two awards, one for the seed project and one for the SATOC contract but reserves the right to make no award if no proposal is found to be of best value to the Government. The Government intends to award without discussions but reserves the right to determine if discussions are necessary. The Government intends, if an award is made from this solicitation, to make award to the proposal that offers the overall best value to the Government and may award to other than lowest price offeror. Comment by Nicholson, Colleen M (Portland): Change to The Government anticipates making one award Comment by Nicholson, Colleen M (Portland): Change to discussions if necessary Evaluation factors are as follows:
Factor 1: Technical Factor 2: Past Performance Factor 3: Price All evaluation factors other than price, are approximately equal to each other. All evaluation factors other than price, when combined, are significantly more important than price.

EVALUATION FACTORS
1- Technical
a. Technical evaluation will rate the assessed capability of the offeror to successfully manage and complete the construction project for the Roseburg VAMC by reflecting the degree to which the offeror’s experience meets or does not meet the minimum performance or capability requirements as required in the project solicitation. Strength will be assessed for at least two submitted projects performed during the same time period.
2- Past Performance
a. Past performance will consider each offeror’s demonstrated recent and relevant record of performance. One performance confidence assessment rating will be assigned using Table 3 for each offeror after evaluating the offeror’s recent past performance, focusing on performance that is relevant to the requirement.
i. The Government will assess recency of projects as either:
· Recent
· Not Recent
ii. The Government will assess relevancy of projects in accordance with Table 2.
b. The Government will utilize any combination of the following methods to evaluate an Offeror’s past performance in addition to reviewing questionnaires submitted including:
i. Query the Contractor Performance Assessment Reporting System (CPARS) for project ratings within the past five years.
ii. Query other Government performance reporting systems or data sources within the past five years.
iii. Review past performance data provided by the contractor.
iv. Contracting references provided or identified elsewhere in offer.
c. Offerors with no past performance will be rated as Neutral.
d. NOTE: Technical submittals will not be used to evaluate past performance or develop a past performance rating.
PRICE EVALUATION
The seed project will be utilized as the total evaluated price to determine best value. Price will be evaluated on a basis of reasonableness using any combination of methods identified in FAR subpart 36.2 and FAR part 15. Offerors are required to review to ensure their price(s) are complete and accurate prior to submitting to the government.
In evaluating the Line-Item price portion of the proposals for the seed project, the PEB will evaluate the offeror’s proposed price for reasonableness IAW FAR 15.404.
In evaluating the labor hour rates portion of the proposals, the PEB will evaluate the proposed rates for unbalanced pricing IAW FAR 15.404.
