About this file
This document is a 3-Year AHERA (Asbestos Hazard Emergency Response Act) Asbestos Reinspection Report for Mann Grandstaff Veteran Affairs Medical Center in Spokane, Washington, dated May 31, 2024.
The inspection covers Buildings 1, 2, 3, 5, and 7 to assess the condition of known regulated asbestos-containing materials (ACMs). The facility contains multiple categories of ACM throughout these buildings, including thermal systems insulation (TSI) with piping insulation at 4-12% chrysotile and mudded fittings at 65-70% chrysotile, vinyl floor tiles (VFT) at 6-8% chrysotile, black mastic at 4-8% chrysotile, and exterior tar and felt roofing systems (assumed asbestos-containing). Building 6 contains potentially asbestos-containing loose-fill vermiculite insulation in wall cavities. Materials are located in basement, first, second, eighth, and ninth floors across various rooms including mechanical spaces, offices, laundry operations, and maintenance areas. The inspection identified most materials in intact to good condition with minimal visible damage. However, two areas of asbestos-contaminated ground soil in Building 1 basement rooms (A005 and C005 East Side) are exposed and present accidental exposure risks, with recommendations for either abatement or enclosure in the near future. Two floor areas in the basement with substrate expansion cracks and minor visible damage have been sealed with wax-sealer requiring continued maintenance. The next AHERA 3-Year Reinspection is scheduled for Summer 2027, with 6-month ACM surveillance required for Winter 2024/25. The inspection was conducted by certified AHERA Building Inspector Samuel W. Bailey Jr. under modified protocols of 40 CFR 763.85.

During this AHERA 3-Year Reinspection, prior identified Regulated ACMs were assessed for existing condition & newly uncovered suspect ACMs were inspected and added to ongoing documentation. An assessment of each ACM was attempted or carried out and recorded in this report.
Seven (7) categories are required for assessing RACM as mandated by 40 CFR Section 763.88, Assessment.
Defined as follows:
(1) Damaged or significantly damaged TSI-ACM.
(2) Damaged friable Surfacing-ACM.
(3) Significantly damaged friable Surfacing-ACM.
(4) Damaged or significantly damaged Friable Miscellaneous-ACM.
(5) ACM with potential for damage.
(6) ACM with potential for significant damage.
(7) Any remaining Friable ACM or Friable suspected ACM.
Next 6-Month ACM Surveillance: Winter 2024/25
After field assessments, a Priority Level Determination (PLD) is made and assigned for each asbestos containing material (ACM).
The PLD is a judgment, based on our experience and expertise in asbestos evaluations, which provides a framework for assessing material condition and exposure potential.
The PLD ranking prioritizes the nature and immediacy of any corrective actions for repair/removal activities that may be required.
Mountain Consulting utilizes several techniques to approach the PLD. All materials were rated on the PLD scale, which describes the relative comparable risk of material location, condition and potential for occupant’s exposure to asbestos.
The PLD scale is numerical and is prioritized as follows:
1 = Damage extensive or severe; exposure potential high.
2 = Localized damage (Contact, age, repair, vibration); exposure potential intermediate.
3 = Minimal or slight damage (age deterioration); exposure potential low.
4 = No visible damage, or non-friable in good condition; minimal exposure potential for undisturbed materials.
Mountain Consulting Services, LLC hereby certifies that the AHERA Based Asbestos 3-Year Reinspection conducted for the known regulated asbestos-containing building materials present throughout the interior of Buildings 1, 2, 3, 5 & 7 located on the Mann Grandstaff Veteran Affairs Medical Center at 4815 North Assembly Street in Spokane, Washington was conducted under modified protocols of 40 CFR 763.85.
All statements contained herein are certified true and correct to the best of Mountain Consulting Services ability, using "State-of-the- Art" practices.
