About this file
This document is an annual medical gas inspection report for American Lake VA Medical Center, dated April 24, 2024, certifying compliance with NFPA-99 (2021 edition), ASSE 6000 (2018 edition), and Joint Commission standards.
The inspection covers medical gas systems including oxygen, medical air, surgical vacuum, and nitrous oxide across three buildings (81, 85, and 200). The facility operates leased bulk oxygen systems supplied by Air Liquide (Building 81) and Praxair (Building 200) at 50 psi main line pressure. Medical air compressors in Building 85 operate at 74 psi with good overall condition; the Building 200 unit is out of service. Surgical vacuum systems are located in Buildings 81 (23"Hg), 200 (21"Hg), and 85 (20"Hg), with the Building 85 dental system experiencing pump performance issues. The inspection tested 145 oxygen outlets, 55 medical air outlets, and 152 vacuum outlets, with flow rates and concentration levels meeting NFPA-99 standards. Nine master alarm panels across the facility were tested and passed, though observations note several deficiencies requiring correction: missing dew point sample ports on medical air compressors, undersized vacuum piping, zone valve installation gaps, missing pressure switches, valve leaks, and improper vent discharge locations over egress areas. The facility maintains calibrated testing equipment and employs credentialed personnel (Jeff Champagne, Lead ASSE 6040; Ashton Steely, Apprentice). Notable exceptions and observations document equipment requiring repair or replacement to achieve full NFPA-99 compliance.

Re: Cat 1 annual medical gas inspection
1.0 Letter of verification
An annual medical gas inspection of the source equipment, master alarm panels, valving hierarchy, labeling and identification, and patient care inlets and outlets has been completed.
Criteria for this testing is based on the NFPA-99, 2021 edition, chapter 5, Standard for Health Care Facilities, the ASSE 6000 2018 edition, JC EC.02.05.09, AIA, and CGA guidelines.
This annual medical gas inspection was found to be consistent with the requirements of the above-mentioned publications with noted exceptions.
