About this file
This is an Environmental Design Narrative for a VA Medical Center roof replacement project at Jack C. Montgomery VAMC in Muskogee, Oklahoma. The project (VA Project 623-23-121) involves re-roofing portions of multiple buildings, with key objectives including designing a roofing system meeting energy requirements, replacing perimeter and penetration flashings, installing a permanent guardrail system, removing unnecessary penetrations, replacing roof drain hardware, and evaluating/upgrading lightning protection. The project covers approximately 9,240 square feet across Buildings 24, 24 Corridor, 52 Outdoor Pavilion, 53 Above ER, and 54.
Environmental testing revealed several critical findings: 123 material samples were collected, with 24 identified as asbestos-containing materials, primarily on Building 24's roof (including flashing, sealants, and curb components). Lead testing showed one sample on Building 24's steel I-beam containing 39% lead by weight. Additionally, moisture intrusion was observed in Building 53B Roof Cores D2 and D3, with saturated insulation noted. The project will require careful asbestos and lead abatement, with all roof areas to be evaluated for code compliance and necessary updates. The design must also address the moisture issues and meet ASHRAE 90.1 insulation requirements.

SUBJECT Environmental Design Narrative
Environmental - 1
Project Overview and Background
Riverfront Safety & Health, LC (Riverfront), a Service-Disabled Veteran Owned Small Business, has performed a limited asbestos and/or lead sampling inspection review in support of the Re-Roof Portions of Various Buildings Pt.4 Design Project (VA Project 623-23-121) proposed at the Jack C.
Montgomery VA Medical Center in Muskogee, OK (The Project).
Scope of Work
Project objectives are as follows:
1. Design a roofing system that meets energy requirements
2. Replace all perimeter and penetration flashings
3. Install a permanent guard rail system that meets OSHA 1910.29(b) and OSHA 1926 Subpart M
4. Remove all unnecessary penetrations and permanently close and seal
5. Replace all roof drain hardware, rings, gutters, and downspouts down to cast iron pipe
6. Lightning protection system on all buildings shall be evaluated and restored or upgraded as needed
7. Roofing insulation value of R-25 Continuous insulation is required by ASHRAE 90.1
8. All items shall be evaluated for replacement or refurbishment to include exhaust fans and vents
9. A/E to evaluate alternative solutions to current roofing system and present to the VA prior to design
10. Approximate Roof Areas:
BLDG 24 - 3340 ft2 BLDG 24 Corridor - 1350 ft2 BLDG 52 Outdoor Pavilion - 1750 ft2 BLDG 53 Above ER - 1400 ft2 BLDG 54 - 1400 ft2
11. All roof in design area shall be evaluated for code compliance and design is to reflect any necessary code updates that would be required
Historic asbestos and lead records were not provided for areas likely to be impacted by The Project. Based on age of facility, and potential methodology of contract work, asbestos and lead containing materials are anticipated as present and likely to be impacted by The Project
Asbestos Containing Assessment & Inspection Results
RSH completed a field investigation on October 24 and 25, 2024. This limited project specific asbestos and lead survey was performed by Charles Davis and Jamison Bear. Mr. Davis and Mr. Bear are accredited under the Asbestos Hazard Emergency Response Act (AHERA) and the U.S. Environmental Protection Agency Model Accreditation Program for Asbestos Building Inspectors. All work was performed under the direct supervision of a Certified Industrial Hygienist (CIH), Mr. Justin R. Rhyneer, to ensure standard EPA- NESHAP/AHERA/OSHA field inspection and sampling methods were employed for representative project inspection. Access to known work areas was provided during regular hospital hours.
The limited asbestos and lead field survey was conducted with the following limitations:
• At the time of inspection, RSH performed inspections based on drawings showing proposed areas of renovations and demolition to existing roofs.
• The limited mold assessment was based on visual observations only.
• General areas that were inaccessible for inspection and limited the inspector’s ability to directly observe and/or collect sufficient bulk samples included: Building 24 Penthouse Roof due to access issues and Building 24 Exhaust Fans, Condensing Units, and Mechanical Unit due to equipment being operable at the time of inspection.
Environmental - 2
Riverfront personnel collected a total of 123 representative samples of suspected asbestos-containing materials related to The Project. All samples were transferred via written chain-of-custody, to EMSL, an AIHA & NVLAP accredited laboratory, for analysis. Sampled building materials included roofing system cores, various wall and equipment flashing, various penetration caulks, overflow scupper flashing, grout, lightening protection caulk, coping sealant, pitch pocket, and termination bar caulk. The suspect materials, which were similar in general appearance and age of construction, were grouped into homogeneous material sets and sampled. A minimum of three samples were collected of all suspect asbestos containing building materials found during the site visit based on conditions noted October 24 and 25, 2024. Practical precautions were used in collecting samples to ensure safety of all personnel. All samples were safely collected, placed in sealed containers, and transferred with a written chain-of-custody, to EMSL, an AIHA & NVLAP accredited laboratory, for analysis. All samples were analyzed using polarized light microscopy (PLM) analysis with dispersion staining (DS) according to U.S. Environmental Protection Agency (EPA) Method 600 IR-93/116 which is presently recommended by the EPA. Asbestos Containing Materials are defined by EPA as having greater than 1% asbestos as measured by PLM methods.
The EPA defines asbestos-containing materials (ACM) as having greater than 1% asbestos as measured by polarized light microscopy (PLM) methods. Out of the one hundred twenty-three (123) suspect asbestos samples collected, twenty-four (24) samples were identified as asbestos containing materials by PLM analysis. These materials are:
Building 24 Roof
• Flashing on Condensing Unit Curb
• Flashing on Capped Curb/Vent
• Perimeter Flashing (Lower)
• Perimeter Flashing on I-beam (Higher)
• Roof Scupper Flashing System
• White/Silver/Black Sealant on 3”x12” Brick Parapet Wall
• Curb Flashing on Exhaust Fan Unit
Building 53A Roof
• Overflow Scupper Flashing
A summary of The Project material sampling efforts, including sample numbers, descriptions, locations, quantities, category (friable or non-friable) are presented in Tables 1. The full laboratory analytical reports for RSH collected samples are presented as Appendix A. Photo documentation of identified asbestos containing materials are presented in Appendix C.
Lead Based Paint Assessment & Inspection Results
A targeted lead inspection was performed to identify lead-based paint and/or lead-based glazing on primary substrates potentially impacted by project work. Material samples taken were performed in accessible project work locations.
Riverfront collected lead paint samples from potentially impacted materials and targeted surfaces to identify the presence of lead-based paint (LBP), which is defined by the EPA as greater than or equal to 0.5% of weight by paint chip. EPA/HUD sampling methods, which are non-destructive, were employed for this project assessment.
It is stressed that this sampling was intended to identify the presence of lead-based paint and/or lead glazed tiles and blocks within readily accessible areas of the building likely to become disturbed during
Environmental - 3
The Project and associated with this particular project design, and not necessarily an EPA-HUD type “surface-by- surface” inspection. Readings were collected by an EPA-HUD trained Lead Inspector/Risk Assessor.
A total of five (5) lead paint chips percent by weight samples were collected from painted or suspect lead containing surfaces. Of these samples, all five (5) samples are lead containing but one (1) sample exceeded the EPA/HUD stand of equal to or greater than 0.5% by weight paint chip standard on substrates or lead containing materials. These materials are:
Building 24 Roof
• Deteriorated Grey/Red/Orange on Steel I-Beam
A summary of lead paint chip sampling efforts for The Project including testing locations and lead percent by weight results are provided in Table 2. The full laboratory analytical reports for RSH collected samples are presented as Appendix B. Photo documentation of identified lead containing material is presented in Appendix C.
Mold Impacted Buildings Materials – Sampling Plan
Historical records have not been provided regarding known mold impacted building materials at the VA campus. During the October 24 and 25, 2024 field investigation, no evidence of mold was observed in the identified project areas, however, evidence of excessive moisture intrusion was observed in the form of water and saturated insulation being observed in Building 53B Roof Core’s D2 and D3. Excessive moisture in roofs can lead to mold growth and compromise structural integrity and affect building occupants’ health.
Photo documentation of observed moisture intrusion is presented in Appendix C.
Sincerely, February 5, 2025 Justin R. Rhyneer, CIH Date

1) Positive ACM samples are noted in red type.
2) An asbestos-containing material is defined as a material that contains greater than 1% asbestos.
3) A friable asbestos-containing material is a material than can be pulverized, crumbled, or reduced to powder by hand pressure. Likewise, a non-friable asbestos-containing material is a material than cannot be pulverized, crumbled, or reduced to powder by hand pressure.
4) Sample result reported as not submitted or insufficient material. Applicable Federal regulations require a minimum of 2 bulk samples for determination of asbestos content within a given Miscellaneous Homogenous Material Area (HMA) and at least 3 samples for determination within a given TSI HMA. Please see samples results provided for identical HMAs.

EMSL maintains liability limited to cost of analysis. Interpretation and use of test results are the responsibility of the client. This report relates only to the samples reported above, and may not be reproduced, except in full, without written approval by EMSL. EMSL bears no responsibility for sample collection activities or analytical method limitations. The report reflects the samples as received.
Results are generated from the field sampling data (sampling volumes and areas, locations, etc.) provided by the client on the Chain of Custody. Samples are within quality control criteria and met method specifications unless otherwise noted. The above analyses were performed in general compliance with Appendix E to Subpart E of 40 CFR (previously EPA 600/M4-82-020 “Interim Method”) but augmented with procedures outlined in the 1993 (”final”) version of the method. This report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST or any agency of the federal government. Non-friable organically bound materials present a problem matrix and therefore EMSL recommends gravimetric reduction prior to analysis. Unless requested by the client, building materials manufactured with multiple layers (i.e. linoleum, wallboard, etc.) are reported as a single sample. Estimation of uncertainty is available on request.
Samples analyzed by EMSL Analytical, Inc. Saint Louis, MO NVLAP Lab Code 200742-0, CA 2668, OR 4194-001
