A site visit will be held on 1/04/2024 at 11 A.M. MT at the Sheridan Veterans Affairs Medical Center at 1898 Fort Rd.
Sheridan, Wyoming. Participants will meet at Building 5, Room 219. Participants will then be escorted to the renovation site. All participants shall wear hard hats and reflective vests.

Statement of Work The Sheridan Veteran Affairs Medical Center (VAMC) located at 1898 Fort Road, Sheridan, WY, requires repairs to a portion of the wood floor on the first floor of Building 7. Building 7 is a wood and brick building constructed in 1907. The 1st floor of the building is being used as a commercial kitchen to feed Veterans receiving VA Care. A section of wood floor in corridor C102 has failed and sagged. The cause of the failure has been linked to overloading of the wood joists by heavy carts transporting supplies from the kitchen to the storage area. The VAMC requires construction service to repair the wood structure and remodel portions of the building that were removed to make the repairs.
1. General
2. The construction work will include the removal of existing walls, flooring, subflooring, and other necessary building elements to access the structural joist system underneath the floor. The floor will then be repaired according to the project design drawings and specifications. The work will also include replacing walls, doors, subfloor, flooring, finishes, and other building elements that had to be removed to access the floor support structure. Finishes will be restored from wall to wall of the areas disturbed and does include the basement underneath which is an employee break room with breakroom cabinets and sink.
a. Construction project is to be completed within 90 calendar days from date of notice to proceed.
b. Contractor shall provide all equipment, material and labor required for demolition work, providing negative pressure environment for work, providing new walls, and providing new room finishes as indicated in the plans and specifications per VA construction standards.
c. Safety submittals shall be complete prior to the commencement of onsite construction work. Material selection submittals shall be complete and approved prior to ordering of materials for the project.
d. The building tenants not in the construction areas will remain at work in the buildings during this construction. Patients will remain on the floor in areas not under construction so the contractor shall make patient safety and infection control measures a priority. The contractor shall coordinate work locations and schedules with VA so as to minimize impact to building tenants.
e. All electrical, carpentry, architectural, mechanical work, product design, project management and other work needed on this project shall be provided by the contractor. All electrical work low, medium or high voltage will be installed and terminated by the contractor.
i. No wiring or cabling shall be abandoned in place.
ii. All piping, cabling and wiring shall be labeled.
f. The installation must, as a minimum, be in accordance with all applicable codes indicated in the Department of Veterans Affairs (VA) Design Manuals and VA PUBLICATIONS (Master Construction Specifications, Construction Standards, National Fire Protection Association (NFPA) Underwriters Laboratory, International Building Code IBC 2003, NFPA and National Electric Code (NEC) building codes and standards and VA Information Security Handbook 6500.6, 6550.0 etc.).
g. Any penetrations through any floors, ceilings or walls, shall be fire caulked, or otherwise fire sealed, and shall be done in this contract to meet VA standards and National Fire Protection Association (NFPA) standards.
h. The contractor shall inquire of the VA if any doors, frames, associated hardware, and any other property removed is desired to be retained by the VA for reuse elsewhere within the medical center, at the time of removal.
i. If the VA desires to retain an item, the contractor shall deliver it to a location designated by the VA.
ii. If the VA does not desire to retain an item, the contractor shall dispose of it.
i. The new components installed shall be tested and commissioned, recertified and/or tested before VA acceptance. All testing parameters will be provided by the contractor with source references to the COR and 3rd party reviewer before testing begins.
i. HVAC systems that have changes made to them as part of this project, beyond diffuser replacement, shall be tested and balanced by the contractor.
ii. Any IT data cables installed or modified as part of this effort shall be tested after installation. Test methods and plans shall be coordinated with the COR.
iii. Any MEDGAS systems modified as part of this effort shall be recertified back to the closest shutoff in the system. The contractor shall arrange of the recertification as part of this contract and shall provide plans and results to the COR for approval.
iv. Fire alarm devices that are tied into the building fire alarm system, that are replaced or relocated as a result of this work, shall be tested before that phase of the project is complete. The contractor shall arrange for all required fire alarm testing.
v. Any fire sprinklers that are relocated as part of this project shall be reviewed by VA Fire Safety prior to installation. All fire sprinkler work shall be coordinated with VA Fire Safety to avoid accidentally setting off the system.
j. At the conclusion of the project, the contractor shall provide the VA and AE with a redlined set of construction drawings, showing any as-built variations from the plans as called for in the specifications.
3. Safety and Security
a. The contractor shall follow all safety specifications and shall always have an OSHA 30 hour certified person onsite, at each work location, overseeing the construction.
b. Work will not be in medical areas of the hospital. However, patients may be present in Building 7, therefore the contractor shall plan for infection control measures to include dust containment, negative pressure and cleanup to hospital standards.
i. The contractor shall, with the participation of the COR, complete an infection control risk assessment, using the VA forms and process, and shall implement all protection measures called for as a result of the risk assessment.
c. Due to work being on an active mental health unit, the contractor shall follow strict tool control and access control in construction areas. Any patients found in construction areas shall be immediately reported to ward staff.

PART I - INSTRUCTIONS, CONDITIONS AND NOTICES TO OFFERORS
PART I/SECTION A – REQUEST FOR QUOTE
This is a Request for Quote in accordance with FAR Part 13. Potential offerors must follow all instructions to be compliant with the solicitation.
It is the responsibility of the offeror to verify that their offer is complete. The offeror is responsible for submitting all required forms and documents with the offer.
The Government will award a single contract resulting from this solicitation to a responsible SDVOSB offeror whose offer conforming to the solicitation will be most advantageous to the Government based on Past Performance and Price.
I.1A JOINT VENTURES
If submitting an offer as a joint venture as defined by P.L. 109-461 AND Department of Veterans Affairs I.L. 049-06-4. Certified SDVOSB/VOSB listed in the SBA certification database at https://veterans.certify.sba.gov/ (see 13 CFR 128). A joint venture may be considered eligible if it meets the requirements in 13 CFR part 128, and the managing joint venture partner makes the representations under VAAR 819.7003 paragraph (b). An offeror must provide a copy of the joint venture agreement specific to the project that is consistent with the percentage of required work and net proceed. Additionally, the offeror must provide a breakdown or labor of which is to be performed by the SDVOSB firm and that which is performed by the small business joint venture firm and that which is sub-contracted. All Joint Ventures will be required to be registered and verified as an entity in VETCERT.

I.4A SELECTION CRITERIA/QUOTE PREPARATION INSTRUCTIONS
Quotes will be evaluated in accordance with FAR 13.106 Simplified Acquisition Procedures Award will be made based on the following factors:
Factor 1 - Past Performance Factor 2 - Price Offerors are encouraged to provide their best offers with the past performance information requested for evaluation. The Government, based on the offered material presented, will award to the offeror that best demonstrates to be most advantageous for the Government.
Factor 1 - Past Performance: Offerors shall provide past performance information on two (2) federal, state, local government or private contract of similar type, scope, size, and complexity. For purposes of this evaluation, a relevant project is further defined as completing construction modifications in a medical facility environment. Project shall have a minimum amount of $25,000.00 or more and completed within the past five years from the date of issuance of this RFQ. If a completed Contractor Performance Assessment Reporting System (CPARS) evaluation is available, it shall be submitted with the quote. The Government may also evaluate recent and relevant past performance, based upon sources available to the contracting officer that may include Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS) and Electronic Subcontract Reporting System (eSRS). IF THE OFFEROR CANNOT PROVIDE A CPARS FOR PROJECTS LISTED IN FACTOR 1, then the offeror shall submit Past Performance Questionnaires (PPQ).
Completed PPQs should be submitted directly from your client via email to Christopher Wentworth at christopher.wentworth@va.gov. If you have any questions, please contact Christopher Wentworth at christopher.wentworth@va.gov. If the offeror is unable to contact a client for a project(s) before closing date, the offeror should complete and submit with the quote the first page of the PPQ, which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires.
Basis of Evaluation: Past Performance will be evaluated in accordance with FAR 13.106-2 and award will be made to the offeror presenting the best value to the Government.
The Government also reserves the right to award to the lower priced offeror with lower past performance rating if the Contracting Officer (CO) determines the price difference does not merit awarding to the higher priced offeror. In that event, the CO shall make an integrated assessment best value award decision.
Factor 2 - Price: Offerors shall provide pricing as requested in the Schedule of Service identified within the SF 1442 Basis of Evaluation: The Government will evaluate price based on the total price. Total price consists of the basic requirements. Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:

| • | Comparison of quoted prices with the IGCE. |
| • | Comparison of quoted prices with available historical information. |
The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
Special Standard of Responsibility Solicitation Submittal Requirements:
The Offeror shall submit the following information: (For a partnership or joint venture, the following submittal requirements are required for each Contractor who is part of the partnership or joint venture and for the entity itself. EMR shall not be submitted for subcontractors.)
1. OSHA or EPA Violation(s):
Offerors shall provide Summary of Work-Related Injuries and Illnesses, affirming that the Offeror has no more than three serious, or one repeat, or one willful OSHA or EPA violation(s) in the past 3 years.
2. Experience Modification Rate (EMR):
Offerors shall provide documentation to show that the offeror has an Experience Modification Rate (EMR) of equal to or less than 1.0. If an offeror’s EMR is above 1.0, the offeror must submit a written explanation of the EMR from its insurance carrier on the insurance carrier’s letterhead, describing the reasons for the EMR and the anticipated date the EMR may be reduced to 1.0 or below.
Offerors who fail to meet any special standard of responsibility may not be eligible to receive contract award.

I.1B COST RANGE: Between $100,000 and $250,000 I.2B GENDER: Wherever the masculine gender is used in this solicitation and contract documents, it shall be considered to include both masculine and feminine.
I.3B METRIC PRODUCTS:
Products manufactured to metric dimensions will be considered on an equal basis with those manufactured using inch-pound units, providing they fall within the tolerance specified using conversion tables contained in the latest revision of Federal Standard No. 376, and all other requirements of this document are met.
If a product is manufactured to metric dimensions and those dimensions exceed the tolerance specified in the inch-pound units, a request should be made to the Contracting Officer to determine if the product is acceptable. The Contracting Officer, in concert with the Contracting Officer’s Representative (COR), will accept or reject the product.
I.4B REQUESTS FOR INFORMATION:
All requests for information must be submitted in writing to the Contracting Officer for action no later than the date shown in Block 10 of the SF 1442, Solicitation, Offer and Award.
I.5B QUOTE REQUIREMENTS
General – Both past performance information and price proposal shall be submitted in response to this solicitation. Quotes shall be submitted via email to christopher.wentworth@va.gov. Emails are limited to 15 MB. The proposal, in its entirety, shall be one email (one attachment for price proposal, one attachment for LOS certification, and one attachment for past performance information) not to exceed 15MB. Quote can be submitted in one email if 15MB or less. Failure to submit in the format required and clearly address those factors may result in rejection of the offer. Electronic copies of the quotes will be time/date stamped by Microsoft Outlook upon receipt of email. Further details on electronic submission are outlined below.

Important Notice: Apparent successful offerors must apply for and receive verification from the Small Business Administration (SBA) through the SBA certification database via 13 CFR Part 128, VAAR 819.7011, and VAAR 819.7003 by submission of documentation of Veteran status, ownership and control enough to establish appropriate status. Offerors must be both VISIBLE and VERIFIED by the SBA certification database at the time of offer submission. Failure to be both VERIFIED by SBA and VISIBLE on the SBA certification database at the time of offer submission and contract award will result in the offeror’s proposal being deemed non-compliant. All offerors are urged to contact the SBA and submit the required documents to obtain verification of their SDVOSB status if they have not already done so.
I.6B REQUIREMENTS FOR ELECTRONIC SUBMISSION OF OFFER
Unless paper offers are specifically authorized in an individual solicitation, all responses to solicitations must be submitted electronically as described below. Responses submitted in a paper form are unacceptable. Failure to comply with this requirement may jeopardize the possibility of receiving an award for the contract due to noncompliance with the terms of the solicitation. You must submit your electronic offer, and any supplemental information (such as spreadsheets, backup data, technical information), using any of the electronic formats and media described below.
In addition, contractors are notified of the award via an electronic Notice of Award e-mail. The award document will be attached to the Notice of Award e-mail.

Security Issues, Late Bids, Unreadable Offers
1. Late submission of offers is outlined at FAR 52.215-1(c)(3). Particular attention is warranted to the portion of the provision that relates to the timing of submission.
1. Please see FAR 15.207(c) for a description of the steps the Government shall take with regard to unreadable offers.
1. To avoid rejection of an offer, vendors must make every effort to ensure their electronic submission is virus-free. Submissions or portions thereof submitted and which the automatic system detects the presence of a virus, or which are otherwise unreadable will be treated as “unreadable” pursuant to FAR 15.207(c).
1. The virus scanning software used by our e-mail systems cannot always distinguish a macro from a virus. Therefore, sending a macro embedded in an e-mail message or an e-mail attachment may cause the e-mail offer to be quarantined. You may send both the spreadsheet and the spreadsheet saved in PDF format to ensure that your quote is readable.
1. Password protecting your offer is not permitted. The Contracting Officer will file the offer electronically which will allow access only by designated individuals.
2.1 52.222-5 CONSTRUCTION WAGE RATE REQUIREMENTS—SECONDARY SITE OF THE WORK (MAY 2014) (a)(1) The offeror shall notify the Government if the offeror intends to perform work at any secondary site of the work, as defined in paragraph (a)(1)(ii) of the FAR clause at 52.222-6, Construction Wage Rate Requirements, of this solicitation.
(2) If the offeror is unsure if a planned work site satisfies the criteria for a secondary site of the work, the offeror shall request a determination from the Contracting Officer.
(b)(1) If the wage determination provided by the Government for work at the primary site of the work is not applicable to the secondary site of the work, the offeror shall request a wage determination from the Contracting Officer.
(2) The due date for receipt of offers will not be extended as a result of an offeror's request for a wage determination for a secondary site of the work.

2.2 52.225-10 NOTICE OF BUY AMERICAN REQUIREMENT—CONSTRUCTION MATERIALS (MAY 2014)
(a) Definitions. "Commercially available off-the-shelf (COTS) item," "construction material," "domestic construction material," and "foreign construction material," as used in this provision, are defined in the clause of this solicitation entitled "Buy American—Construction Materials" (Federal Acquisition Regulation (FAR) clause 52.225-9).
(b) Requests for determinations of inapplicability. An offeror requesting a determination regarding the inapplicability of the Buy American statute should submit the request to the Contracting Officer in time to allow a determination before submission of offers. The offeror shall include the information and applicable supporting data required by paragraphs (c) and (d) of the clause at FAR 52.225-9 in the request. If an offeror has not requested a determination regarding the inapplicability of the Buy American statute before submitting its offer, or has not received a response to a previous request, the offeror shall include the information and supporting data in the offer.
(c) Evaluation of offers.
(1) The Government will evaluate an offer requesting exception to the requirements of the Buy American statute, based on claimed unreasonable cost of domestic construction material, by adding to the offered price the appropriate percentage of the cost of such foreign construction material, as specified in paragraph (b)(3)(i) of the clause at FAR 52.225-9.
(2) If evaluation results in a tie between an offeror that requested the substitution of foreign construction material based on unreasonable cost and an offeror that did not request an exception, the Contracting Officer will award to the offeror that did not request an exception based on unreasonable cost.
(d) Alternate offers.
(1) When an offer includes foreign solicitation in paragraph (b)(2) of the clause at FAR 52.225-9, the offeror also may submit an alternate offer based on use of equivalent domestic construction material.
(2) If an alternate offer is submitted, the offeror shall submit a separate Standard Form 1442 for the alternate offer, and a separate price comparison table prepared in accordance with paragraphs (c) and (d) of the clause at FAR 52.225-9 for the offer that is based on the use of any foreign construction material for which the Government has not yet determined an exception applies.
(3) If the Government determines that a particular exception requested in accordance with paragraph (c) of the clause at FAR 52.225-9 does not apply, the Government will evaluate only those offers based on use of the equivalent domestic construction material, and the offeror shall be required to furnish such domestic construction material. An offer based on use of the foreign construction material for which an exception was requested—
(i) Will be rejected as nonresponsive if this acquisition is conducted by sealed bidding; or
(ii) May be accepted if revised during negotiations.

3.2 52.204-24 REPRESENTATION REGARDING CERTAIN TELECOMMUNICATIONS AND VIDEO SURVEILLANCE SERVICES OR EQUIPMENT (NOV 2021) The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it ‘‘does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument’’ in paragraph (c)(1) in the provision at 52.204–26, Covered Telecommunications Equipment or Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212–3, Offeror Representations and Certifications–Commercial Products and Commercial Services. The Offeror shall not complete the representation in paragraph (d)(2) of this provision if the Offeror has represented that it ‘‘does not use covered telecommunications equipment or services, or any equipment, system, or service that uses covered telecommunications equipment or services’’ in paragraph (c)(2) of the provision at 52.204–26, or in paragraph (v)(2)(ii) of the provision at 52.212–3.
(a) Definitions. As used in this provision— Backhaul, covered telecommunications equipment or services, critical technology, interconnection arrangements, reasonable inquiry, roaming, and substantial or essential component have the meanings provided in the clause 52.204–25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.
(b) Prohibition. (1) Section 889(a)(1)(A) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L. 115–232) prohibits the head of an executive agency on or after August 13, 2019, from procuring or obtaining, or extending or renewing a contract to procure or obtain, any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. Nothing in the prohibition shall be construed to—
(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.
(2) Section 889(a)(1)(B) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L. 115–232) prohibits the head of an executive agency on or after August 13, 2020, from entering into a contract or extending or renewing a contract with an entity that uses any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. This prohibition applies to the use of covered telecommunications equipment or services, regardless of whether that use is in performance of work under a Federal contract. Nothing in the prohibition shall be construed to—
(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
