Site Preparation for MRI #2

Bidder Request for Information

1. 5/AE 901 – What does the window framing consist of (i.e. pre-manufactured unit or custom built)? o If pre-manufactured, no requirements listed in the specs?

o If custom built, please list the components required to assemble?

Spur Design – The window unit is to be ETS-Lindgren RF Shielded window system or equal. (AE-901)

2. 6/AE 901 – Provide elevation of RF window assembly that sits in front of the storefront. o If pre-manufactured, no requirements listed in the specs?

o If custom built, please list the components required to assemble?

o RF Shielded Window Frame System detail resembles a “corner guard”, please provide a detail defining components.

Spur Design – The window unit is to be ETS-Lindgren RF Shielded window system or equal. (AE-901)

3. 2-4 & 7-11/AE 901 – The location of these details are not documented on the plans, please provide where these conditions occur?

Spur Design – Details 10/60-AE-901 and 11/60-AE-901 have been removed from the documents. All other door details are referenced in the Door Schedule on sheet 60-AE-901.

4. 7 & 8/AE 901 – References a “RF Shielded” HM Frame, the only HM Frame per the schedule is opening 124, which does not require shielding, please clarify?

Spur Design – Details 7/60-AE-901 and 8/60-AE-901 have been updated with wood door frames.

5. 9&12/AE 901 – Reference a floor assembly made of multiple components; these components appear to require a recessed slab. Neither structural or architectural indicate a recessed slab for the MRI room? Please clarify.

KFC Engineering - Adjust finish slab elevation to 99’-10 ¼” from 100’-0” to account for floor thickness.

6. 12/AE 901 – Wall construction indicated does not match wall Type 4A, which is correct?

Spur Design – Wall Type 4A is correct. 12/60-AE-901 has been updated to match.

7. 1 & 2/AE 701 – Reference an RF Shielded Ceiling System, please provide the components of the systems in a detail.

Spur Design – Detail 13/60-AE-901 has been added.

8. 1/AE 401 – What are the requirements of the Cryogenic Quench Exclusion Zone (currently not existing on roof of other MRI)?

Spur Design – The MRI Design Guide states that “the exclusion zone should be clearly marked, and staff and contractor should be restricted from working in the area until they have been educated to the risks of cryogenic gasses escaping the vent pipe.” The Cryogenic Quench Exclusion Zone is to be coordinated with the MRI Manufacturer.

In addition to Spur Design comments: The area within the RF zone of the MRI #1 roof and MRI equipment room as shown on DWG. FP101 Fire Protection Plan for MRI #1 shall be included in the phasing plan. Zone within that existing RF boundary and above the operating room will not be accessed unless coordinated and MRI #1 is not in operation.

9. AE 201 – Note states to add a letter of 5/8 GYP to existing framing, according to sheet G011, the majority of the existing walls are to be 2hr rated. 1 layer of 5/8 gypsum, does not meet the requirements indicated by Wall Type 1 (G010). Please clarify.

Spur Design – The note “5/8” GYP APPLIED TO ALL EXISTING 6” METAL FRAMING TO DECK” has been deleted. Follow wall types for all interior partitions.

10. 1/S500 – Are the existing piers at the exact elevation needed to build per the detail, or will modification (bushing) be required to meet the elevation requirements per plans?

KFC Engineering - Elevations shown in documents is the assumed elevation based on the as-build drawings. Actual elevations will need to be verified in the field during construction.

11. 1/S500 Please reference detail below….is the indicated area already notched, or will this area need to be part of the demo?

KFC Engineering - As-built drawings indicate the notch was originally cast into the grade beam. Demolition for the notch will not be required if it is not present. Answered by 1/S500 addendum.

12. Please provide the sign-in sheet so all offerors have subcontractor contact information.

The sign-in sheet and Site Visit document was posted as part of Amendment 002.

13. Please provide information, including points of contact, for the manufacturer/installer of the current roof. Also, please clarify if there is an existing warranty on the roof.

VA to address. The original roof installed in 2013 is Genflex by Sarratoga. It was installed by HGL Construction, Inc. in Midwest City, OK. Per submittal information.

14. Please clarify the scope of work as it pertains to concrete slab. The only note on any of the drawings that indicates any direction is the architectural demolition drawings, 60-AD-101, Note #1, which states to remove existing light weight concrete floor. Is the existing floor only 2”-thick?

KFC Engineering - The exact thickness is unknown. Allow for up to a 4” thick slab for demolition.

15. Is the existing MRI #1 going to remain operational? If so, how or when will work in Room 120 be completed?

VA to address. MRI #1 removal date is dependent VA planning for scans. Work in Control Rm. 120 requires a phasing plan for both isolation of the area and may have to occur during non-patient hours or weekends.

16. Who is responsible for providing, installing and starting up the MRI #2 ACCU?

Spur Design - The mechanical contractor is responsible for providing, installing and starting up the new ACCU-2 on the roof per schedule on sheet 60-M-601.

17. Who is responsible for providing, installing and starting up the MRI #2 chiller?

Spur Design - VA is responsible for providing and GE is responsible for installing and starting up the MRI#2 chiller. Mechanical contractor is responsible for piping and connecting the chiller to the associated MRI equipment on the floor below.

18. There is an existing desk or casework with computers, etc., just below the window adjacent to MRI #1. Is the VA to remove this desk prior to construction or are Contractors responsible to relocate? If Contractor, where are VA-items to be relocated?

Casework would be shown on the drawings per install. If there is a desk the contractor will move it temporarily while work is performed as worked out with the COR.

19. Please confirm that all VA equipment within Room 120 will be removed prior to construction.

All VA equipment within Room 120 will either be removed in coordination with contractor or contractor covered for protection.

20. Please confirm that the existing cubicle curtain and track in Room 120 is not part of this contract.

The cubicle curtain may have to be removed as a room fixture to be dealt with removal/replacement even if not shown on drawings. This pertains to access to work in and above Room 120.

21. Please confirm that no ceiling mounted equipment nor the ceiling itself in Room 120 is part of this contract.

Spur Design – Room 120 is not included in the scope of work with the exception of what is required for Mechanical / Electrical work above the ceiling. Existing ceiling tile and grid are to be removed then put back as required for the previously described work.

22. During the site visit, we noticed existing piping through the fire-rated walls of Room 124 did not have fire caulking. Are Contractors expected to fire caulk existing penetrations in Room 124, even if those pipes are not covered under this contract?

Please include in proposal to bring fire rated walls up to code by adding approved fire seals in all areas of the wall affected by the job.

23. Drawing 60-AE-202 includes the table, GE MRI Equipment Schedule. All items listed on this table are VA provided, GE installed. Please confirm that contractors are not responsible for installing any of this equipment.

GE will install their equipment as provided as included on GE location drawing MRI-M145015-PRE-00-A.DWG.

24. Will contractors be allowed access through the backside of the Five Civilized Tribes Museum? Especially during any roofing work, to set new mechanical units on the roof?

The VA cannot provide access for other than Federal VA property.

25. Please confirm this project is tax exempt.

Yes, as this is for a federal government agency it is exempt from Federal taxes. A tax exempt form certificate will be given to the awardee at time of Notice to Proceed.

26. Within the solicitation, under Factor 1- Past Performance, it states, “Design Build projects will be rated more favorability.” Since the project within the RFP is a design-bid-build, this seems like a typo. Shouldn’t design-bid-build projects be rated more favorably?

This was an error in the form and has been corrected through Amendment 002.

27. Please confirm that the special standard of responsibility information (OSHA/EPA violations, EMR, etc.) are to be included in the price proposal.

It can be included in the price or past performance factors of the proposal.

28. Where in the building is the Fire Riser located that supplies the area of work?

Please see attached Fire Protection Drawing No. FP-01. Facility guidelines are required to work on sprinkler systems and will be coordinated with the COR. The requirement will include a fire watch as approved by safety for certain shutdowns.

29. Will the original Fire Sprinkler drawings for the fire sprinkler system in the area of work be available to the awarded fire sprinkler contractor?

Please see attached Fire Protection Drawing No. FP-01.
