Request for Information (RFI) Log 2NE Beautification Construction Project:605-21-406 25 February 2021 Loma Linda VA Medical Center (605)

RFI Number
Question
Answer
001
Scope of Work 1-A-i, notes that this work will take place in the Acute Psychiatric Unit. Will the VA provide any and all needed escorts for the work to take place or will the Contractor need to employ escort observers for all workers / work crews? Will any other special requirements be enforced for working in this unit?

The VA cannot provide a dedicated escort for the entirety of the work to be performed in 2NE, and as such the contractor is directed to employ additional personnel for escorting purposes.

002
Scope of Work 1-A-i, notes that televisions are to be replaced. Will the VA confirm that only the bedside TVs are to be provided and installed by a separate Contractor and that the new common area TVs are to be Contractor Furnished Contractor Installed?
All televisions there are moving to the bedside television platform and as such will be contractor furnished contractor installed. Acceptable televisions are provided by Allen Technologies and will be purchased out of the VA maintenance fund.
003
Scope of Work 1-A-i, notes that televisions are to be replaced and that the old coaxial network is to be removed and the new televisions will be IP-based. Will the VA or the Contractor be responsible for integrating the new TVs into the existing IP network?
These televisions are being integrated into the existing bedside television platform and as such digital integration will be handled by Allen technologies. Their TVs will use data that is installed as part of this contract, and will be terminated into the Allen Technologies patch panel in the TR. The contractor providing the televisions is Allen Technologies.
004
Scope of Work 1-A-i, notes that “Because of COVID-19, providers and patients are not able to meet face-to-face, and a request is made to provide a video-conferencing solution. This solution will consist of a monitor, webcam, and keyboard/video/mouse interface, with the computer itself remoted into the nurses’ station.” Will the VA provide the quantities for each of the listed components and either specifications or basis of design for each component? Will the VA confirm that all of these equipment components are to be Contractor Furnished?

The computer in the nurses station, the keyboard and the mouse will be VA provided as they must go onto the existing VA network and we are unable to place computers that process VA patient data on the network when provided from a source OTHER than VA National.

005
Will the VA clarify if any materials scheduled for demolition or disturbance contain asbestos? Will the VA provide the current Hazardous Materials Survey for all areas within this project’s work area, including the interstitial space?
The VA will provide the Mabbett report, which details our asbestos findings in all areas.
006
Scope of Work 1-A-ii-6, notes that only (2) rooms will be made available at any one time. This phasing will extend the time required to complete this project. Will the VA extend the Period of Performance from 90 days to at least 150 days?
The period of performance for this requirement has been adjust to 240 days after issue of NTP by this amendment. See attached revised SOW and Bid Schedule.
007
Scope of Work 1-A-ii-6, notes that (2) rooms will be made available at any one time. How will the corridors, non-patient rooms, and common areas be scheduled? Will any of the specified work require after hours performance?

None of the specified work should require after-hours performance. Ordering of corridors and non-patient rooms will be left to the installers and coordinated with the local staff so as to ensure that needs are being met without impeding work. Common areas are going to require the new walls with the inset televisions, and should not be done simultaneously.

008
Scope of Work 1-A-ii-19 requires a new drop-in sink and the assumed required fixtures. Will the VA provide the specifications or basis of design for the new drop-in sink and fixtures?

2B-700 is the staff lounge. The VA at Loma Linda does not have an established standard for sinks in non-medical areas.

009
Scope of Work 1-A-ii-20 requires the fabrication & installation of a new 60” lower cabinet. Will the VA provide the specifications, elevation drawing, section drawing and/or the basis of design for this 60” cabinet and the assumed countertop? The Contractor will need more information to price this requirement correctly.

The 60” lower cabinet will be VA Provided Contractor Installed.

010
Scope of Work 1-A-ii-21 requires the provision and installation of an outdoor quality vinyl mural. Will the VA clarify what the exiting exterior wall surface consists of and provide a specification on the mural thickness and quality level? Since the mural requires a submittal of several choices for the VA to approve, will the VA establish an allowance dollar amount for this requirement to facilitate a fair bidding process?

The wall is assumed to be painted concrete at 112’ long by 12-12.5’ tall, and expected to be mostly non-porous. Basis of design for the wall mural is Coastal Creative high tack outdoor vinyl with matte lamination, and we will require the edges to be sealed. Previous estimates came in below $11K

011
Scope of Work 1-A-ii-22 requires the courtyard surface cracks and perforations to be filled and sealed. Since there are no drawings for this element, will the VA provide a maximum linear footage for bidding purposes in order to accomplish this requirement?

Approximately 240 LF of courtyard surface cracks to be filled and sealed.

012
Scope of Work 1-A-ii-26, 27 requires the Contractor to provide wall build-outs to accommodate TV’s and recessed speakers. Will the VA clarify where the power is to be taken from for these installations and verify those panels have sufficient room for the new dedicated circuits?

*Question has been received and will be answered in a later amendment to this solicitation

013
Scope of Work 1-A-ii-26, 27 requires the Contractor to provide wall build-outs to accommodate TV’s and recessed speakers. The TV’s are to be covered by plexiglass. Will the VA provide the specifications or basis of design for the required plexiglass?
Plexiglass basis of design: Duraplex® Impact Modified Flat Sheet - DURAPLEX - Order Online (professionalplastics.com)
014
Scope of Work 1-A-ii-26, 27 requires the Contractor to provide wall build-outs to accommodate TV’s and recessed speakers. The TV’s are to be covered by plexiglass. Will the VA describe how the recessed TV spaces, covered by plexiglass, are to be vented to avoid component heat build-up?

The VA will accept louvered vents above the televisions to vent heat up into the plenum space and drilled perforations in the plexiglass near the base to facilitate fresh air and air flow.

015
Scope of Work 1-A-ii-26, 27 requires the Contractor to provide wall build-outs to accommodate TV’s and recessed speakers. Will the VA provide a specification or basis of design, including dimensions, for the speakers? Are the speakers Contractor furnished or VA furnished? Will the speakers be covered by plexiglass as are the TVs.

Speakers are contractor furnished and the VA would like a powered water resistant solution. Amplified marine speakers secured with tamperproof screws would be ideal (Pyle 6.5in 4-Channel Waterproof Amplified Marine Speaker System (PLMRKT4A) (solidsignal.com)) and would not require installation behind plexiglass.

016
Scope of Work 1-A-ii-27, 28 requires the Contractor to provide recessed monitor, webcam, and microphone for video conferencing. Will the VA provide specifications or a basis of design for these components? Will the VA clarify if these components are Contractor furnished or VA furnished? Will these components be covered by plexiglass

0. Clarification: The monitor will be VA-provided and come from OIT. Dell 23” monitors are our current standard and for the purposes of buildout may be found here: Dell 23 Inch Monitor - FHD for Office: P2319H | Dell USA

0. A standard Logitech webcam sitting on top of the monitor will suffice, as the monitor will be behind security plexiglass and the camera will be unable to be tampered with.

0. Microphone basis of design (since it won’t work behind plexiglass) http://www.etsnm.com/pdfs/sm1-fe.pdf

017
Scope of Work 1-A-ii-27, 28 requires the Contractor to provide recessed monitor, webcam, and microphone for video conferencing. Will the VA describe how these components are to be vented to avoid heat build-up?

The VA will accept louvered vents above the televisions to vent heat up into the plenum space and drilled perforations in the plexiglass near the base to facilitate fresh air and air flow.

018
Will the Superintendent of this project, if fully qualified, be able to serve as the Supt., Site Safety Officer, and Contractor Quality Control, or will separate individuals be required for any of those roles?

If fully qualified, the superintendent may serve all of those roles.

019
Scope of Work 1-A-xii states that the Contractor will not be required to provide a new patch panel if there is sufficient space. For fair bidding purposes, should the Contractor assume they will need to provide a new patch panel, or will the VA confirm there is sufficient space?

For bidding purposes, please include the patch panel.

020
Will the VA provide as-built plumbing, electrical, and communications reference drawings for this project?

The VA will provide as-built plumbing and electrical, but cautions that they may be out of date and should be verified visually before work begins. There are no communications drawings to provide as we do not maintain communications as-builts.

021
Will the ICRA negative pressure containment requirements for this project be able to be accomplished with hepa-carts and fire resistant plastic sheeting or will the much more expensive drywall or modular wall containment be required for each of the many small phases of the construction? Will hepa-filtered negative pressure exhaust be allowed to be vented into existing facility exhaust duct systems?

HEPA carts and plastic sheeting are perfectly acceptable.

022
Specification Section 01 35 26, Part 1.9-B requires monthly site safety inspection by and independent or corporate safety professional (CSP or CSHT). Will the VA confirm this requirement will be enforced for this project?

The VA at Loma Linda will perform its own weekly construction safety rounds and the PM will perform daily site walkthroughs.

023
Specification Section 01 35 26, Part 1.13 requires pre-placement tuberculosis screening for all contract employees. Will this be a requirement for this project?

This will not be a requirement.

024
Specification Section 07 84 00 Firestopping, Part 1.5-C – Inspector Qualifications requires a 3rd party independent ASTM E699 qualified inspection of all firestopping installations. Will the VA confirm this is a requirement for this project?

This VA will perform this inspection in conjunction with the contractor and the installer.

025
Drawing Sheet A1 – Floor Finish Schedule only indicates they type of flooring material for each of the depicted areas. Neither the Floor Finish Schedule nor the Specifications indicate the color, style, or basis of design flooring manufacturer. Pricing will vary based upon color, style and BOD manufacturer. Will the VA provide a complete flooring finish schedule that provides the color, style, & BOD manufacturer?

*See Table A Below

026
Scope of Work, Part 1-A-ii-15 requires corner guards at all outside corners. Will the VA provide a specification or a basis of design for the required corner guards?

*See Table B Below

027
Are there plans for the above-referenced project? Also, was there a mandatory walk through of the job?
Yes, there are plans and drawings attached to this solicitation. They are available at BETA.SAM.Gov in the same place you access the solicitation at. The site walk is not mandatory but attendance is highly encouraged.
028
Is it just the 1 arranged site visit or is there scope to arrange a separate one?

Only one site visit is planned for this requirement.

029
Is there an engineer’s estimate?
The magnitude of this project is disclosed on the first page of the solicitation.
030
Is the remodeling project of this specific job (2NE) including the renovations of the architectural equipment that has been up in the air for the last year or so? AKA division 11 / 10 equipment; patient lifts, capital equipment such as med gas booms or headwalls in the patient/ICU rooms?
Please review the solicitation and attached scope of work to see the requirements of this project.
031
Scope of Work, Part 1-A-ii-8 states “Patch and paint walls as identified per attached paint finish schedule drawing.” Drawing A1 – 2NE Paint Finish Schedule shows no paint legend number whatsoever for many rooms and other rooms have a legend box stating “No Change”. Additionally, no paint legend number is depicted for any of the corridors. Will the VA confirm that rooms or areas without a paint legend number or a legend box indicating “No Change” are not to receive new patching or painting?

The notes have been revised to address # 27 (See Attachment)

032
Scope of Work, Part 1-A-ii-23 states “Courtyard surface to have two lane stripes, 42 inches between stripes, painted along perimeter of courtyard. Each stripe will be 2 inches in width. The first stripe will be located 48” from wall. The second stripe will be located 42” from edge of first stripe.” There was no courtyard drawing provided and we are unable to determine from this statement the lineal footage for the striping scope. For bidding purposes, will the VA please provide an as-built drawing for this courtyard? Also, there are many types and qualities for striping markings. Will the VA please specify the striping material specification for what is being requested?

A courtyard drawing with dimensions has been provided (See Attachment)

033
The Drawings provided to not have dimensions and are not to scale. Will the VA provide dimensioned as-built floor plans to facilitate the estimating process for this solicitation?

The Scale has been Revised (See Attachment)

034
The VA did not provide elevations for the areas to be painted. Will the VA provide as-built elevation drawings or provide the ceiling heights to facilitate the bidding of the requested painting?
The notes have been revised to address this (See Attachment)
035
The VA response to RFI #005 is “The VA will provide the Mabbett report, which details our asbestos findings in all areas.” This report was not provided in Amendment 0001. Will the VA provide this report in a following Amendment?

The Mabbett report will be provided in the next amendment.

036
The issued amendment 0001 RFI Request Log states the name of the project as “Upgrade IT Closet Areas”. Will the VA confirm that these are the correct RFIs and responses for this project?

The title of this project is “2NE Beautification”.

037
Is the VA furnishing the cabinets, tv’s or are we furnishing these items?
Cabinets are to be furnished by the contractor, tv’s by VA.
038
Do we have to use the named mural contact given in the RFP for the mural? – even if we can do this cheaper than the rate he is advertising of ~$18k?

The VA is vendor agnostic where the mural is concerned. The primary concerns are outdoor durability, sunlight fading and the ease with which patients would pick or pull pieces of the mural off the wall and or otherwise damage it.

039
Can we get drawings of where the additional walls in Items. 26, 27 & 28 are required to be built– size, spec finish etc.
Current as-builts will be provided as a response to this RFI, and a pdf detailing the additional wall locations will also be provided.
040
What are the sink specs?
Sink specs to follow.
041
Are we expecting the lockers to be emptied prior to construction?
The lockers will be empty.
042
If we are expected to provide the TV equipment what spec?

Television equipment will be provided by the VA.

043
Can the VA provide a photo and know the construction of the wall for the Mural?

Photos were taken by the vendors at the site walk, and measurements were provided both in the previous RFI answers (112 long x 12.5 tall).

044
Can the VA provide a photo and measurements of the wall where new TVs and new Casework is being installed?

Photos were taken at the time of the bid walk, CAD files are being sent with the RFI answers as well as a drawing displaying the proposed locations. Is this still needed?

045
There was mention during the site walk about lockers and the cabinetry being changed to an option? Will that be change be reflected on the Bid form?

This is incorrect. The lockers will be moved, and the cabinets are provided by the vendors. Specs to the cabinet and sink will be provided by VA interior design and sent to the contracting specialist for inclusion as a document in beta.sam

046
Amendment 0002 was to provide scaled drawings to the bidding Contractors. The scale on the provided drawings is reported to only be one half of the dimensions back checked in the field. Will the VA re-issue the scaled drawings full size, with a scale that represents the actual dimensions?
The drawings will be issued as .dwg files as part of this amendment.
047
Video conference solution spec?

VA provided PC, Keyboard, Mouse. Vendor provided webcam, monitor, speakers and microphone.

048
Request data / low voltage / telecommunication drawings, specs & quantities.

VA Loma Linda Structured Cabling standards will be included as an attachment to this. Accurate low-voltage as-builts do not exist at this facility. All copper low voltage cabling will be CAT-6a U/FTP cabling per new facility standard, and all runs will conform to the low-voltage specifications. There will be four runs.

049
Can the mural have any borders or does the image have to cover the entirety of the wall. i.e. can say a 1’ border of the wall be blank?

A border of up to 6” is acceptable provided that the edges are secure to the wall and that there are no bubbles or areas to remove/pick at the mural

050
With regards to RFI #001 response on “contractor is directed to employ additional personnel for escorting purposes”, what are the qualifying “escort” training/background/professional expertise/specialty equipment requirements? What is minimum number of additional escorts the VA will require?

1 additional escort is necessary per entrance to the work areas as the contractors should be working behind a hard “McCain-style” wall. The intent is simply to ensure that patients and contractors do not meet in such a manner that would result in tools or other items winding up in the possession of patients. There are no minimal requirements, other than a valid TB test and COVID-19 precautions that are in effect throughout the rest of the facility. It is further expected that in certain areas the negative air machine ductwork will be exposed to patients who may damage it and the escort will need to watch that as well. Escorts will not be armed.

051
With regards to RFI #002 response on “bedside television platform…will be contractor furnished contractor installed… and will be purchased out of the VA maintenance fund”, please confirm/clarify if this bedside television platform will be a separate procurement contract via the VA maintenance fund and therefore furnished and installed via a separate contractor outside of the scope of this project 36C25821B0003 Y1DA--605-21-406 2NE Beautification?

These displays and their associated network equipment have already been purchased. The VA has a separate contractor who can perform the installation, though that work will need to be coordinated with the COR, the GC, and the television installer.

052
With regards to RFI #004 response on “computer in the nurses station, the keyboard and the mouse will be VA provided”, please confirm that the quantity of data cabling installation required from this project as the video-conferencing solution is to support the 1x computer in the nurses station mentioned in the RFI response? If the aforementioned is not the case and there are additional data requirements for the video-conferencing solution, please provide quantifying and qualifying details?

1x va-provided computer PER VIDEO CONFERENCING LOCATION for a total of two.

The PC, mouse, keyboard and ethernet location will be in the nurses station and the monitor, speakers, camera, and microphone will be in the patient area.

053
With regards to RFI #005 response on “The VA will provide the Mabbett report”, can this report be provided ASAP prior to the site visit in order to accurately quantify and qualify scope bid requirements for the proposal and avoid any unforeseen conditions?

Mabbett report is already sent to contracting and is available as part of this reply.

054
With regards to RFI #020 response on “VA will provide as-built plumbing and electrical”, can these as-built be provided ASAP prior to the site visit in order to accurately quantify and qualify scope bid requirements for the proposal and avoid any unforeseen conditions?

These documents were provided to the contracting and are included as part of this reply. The plumbing documents provided constitute the entirety of available documentation on plumbing at this facility.

055
Does the project need to be completed in phases?

YES

056
For dust control, please specify if we are required to utilize any specific equipment or measures.

The VA requires negative air be maintained in the work environment. The VA does NOT permit venting to the interstitial space

057
Does the VA wish to salvage the concrete seats in the courtyard or dispose off site?

Dispose of concrete benches offsite please.

058
What is the flooring of the courtyard coated with?

Paint on top of an unknown concrete topper/moisture barrier

059
If the courtyard is coated with something – removing the maze may compromise the material used around it – please confirm the VA acknowledges this for any warranty issues on the current surface with the current manufacturer that they may have in place.

No warranties on that material are current. The maze may additionally be removed by painting over the courtyard.

60
Removing the maze with a floor coating already in place will mean an exact color match may not be possible. Is the VA happy with this or do we plan to resurface the entire floor / section (please advise how much you wish to resurface)

The maze is not textured, and as such may be painted over if desired. If the maze is painted over then the entire courtyard deck must be painted to match.

61
What ‘theme’ of landscape options does the VA wish to choose from? i.e. winter, summer, beach, mountains etc.”

Spring, mountains, something near those lines. No winter scenes please, and nothing with people.

062
Amendment 0001, Question #012 asked about routing and source power for the wall build-outs equipment. The VA response was that this will be answered in a later amendment. Amendment 0002 did not address this question. Will the VA provide a response and sufficient time to estimate this requirement in a subsequent amendment?

LL-02-PW.dwg (issued as part of this amendment) has this info

063
Amendment 0001, Question #020 requested as-built plumbing & electrical drawings. The VA response was that the drawings will be provided. To date, the VA has not provided these as-built drawings. Will these as-built drawings be provided in a subsequent amendment?

LL-02-PW.dwg has this info Additional Plumbing and Electrical drawings being issued in this amendment

064
Amendment 0002, Question #035 requested the Hazardous Material Survey for the areas of this project and the VA response was that it will be provided in a subsequent amendment. Will the VA provide the Hazardous Material Survey and sufficient time to estimate this requirement in a subsequent amendment? If Asbestos Abatement will be required for this project, will the VA provide the relative Abatement & Testing Specifications as well?

See Mabbett Report issued with this amendment.

065
We are interested in providing a proposal for the job in Loma Linda VA hospital. Is there going to be a job walk or can we talk about pricing
The site walk was conducted 2/16/2021, pricing for this must be submitted in the form of a sealed bid. Please see the solicitation for more details.
066
Is California DIR Reg a requirement for bidding this project?

The Department of Veterans Affairs does not require registration with the CA Dept. of Industrial Relations for this project.

067
Regarding the flooring requirements and according to the 2NE Finish Schedule, it states that the coving will be rubber base type. I noticed that in the Patient rooms that the flooring is a self-cove style. Will we be using the self-cove style or Rubber Base in the Patient rooms?

Self-cove style should be used, not rubber base.
