ASBESTOS INSPECTION REPORT

FACILITY: ........................ VA LOMA LINDA

11201 Benton Street Loma Linda, California 92357

INSPECTION DATE: ....... October 25, 2019

INTRODUCTION

DAVY Architecture, (referred to hereafter as the Client), retained ENCORP to conduct an asbestos inspection of the Veterans Administration Loma Linda, VAMC Renovations of the Surgical Clinic 2E South, at 11201 Benton Street, Loma Linda, California 92357. The inspection was conducted in order to determine the presence of asbestos and lead in building materials that may be impacted in upcoming renovation/demolition activities at the site. Please note this inspection is limited to those areas which were determined to be impacted by planned renovations and representative site visits.

Asbestos is a general term applied to a group of naturally occurring minerals which separate into fibers. This fibrous material (e.g., Amosite, Chrysotile, Crocidolite, Tremolite, Anthophyllite, and Actinolite) are composed of silicates of aluminum, magnesium and other metals which are incombustible and very difficult to destroy or degrade. Asbestos has a tendency to break into a dust of tiny fibers which can float in the air and be inhaled or swallowed. Asbestos inhalation exposure has been shown to increase the risk of developing lung cancer, mesothelioma (cancer of the lining of the lung and/or abdomen.) and asbestosis (chronic lung disease), as well as other damage to the lungs. Exposure occurs by breathing asbestos fibers produced as a fine dust when asbestos is handled during fabrication, installation or removal. By definition Asbestos Containing Materials (ACM) are any material or product which contains 1 percent (1%) or more asbestos. CAL/OSHA further regulates the content of asbestos in materials or products that contain 1 tenth of a percent (0.1%) or more asbestos for the purpose of worker and occupant protection.

ENCORP’s certified representatives conducted a visual investigation of the property to identify and quantify all suspect asbestos containing materials. Upon completion of the visual investigation, building materials were grouped into homogeneous categories and homogenous with previous asbestos containing materials identified at the facility. Where sampling could not be collected, suspected materials are identified as presumed asbestos containing materials (PACM). ENCORP’S Certified Asbestos Consultant (CAC), Mr.

Alexander Blankevoort performed preliminary site inspections on August 9, 2019, and site inspections on October 25, 2019 along with Site Surveillance Technicians Mr., Jesus Roman and Nestor Canas.

SAMPLING METHODOLOGY – ASBESTOS

No asbestos sampling was performed during this assessment. Previous samples materials provided by the VA to assess conditions and locations of materials. Where previous sample materials were not observed, sample materials were listed a presumed asbestos containing materials by the certified asbestos consultant.

Materials where assessed using the Environmental Protection Agency (EPA) Categories of Asbestos Containing Materials as follow.

Surfacing Asbestos Containing Materials (Sprayed on trowled on materials):

Thermal System Insulation & Other Friable Materials (Mechanical Insulation on pipes, boilers, tanks)

Miscellaneous Materials (Variety of other products such as ceiling tile, floor tiles, sealants, and cement products) shall be collected from each homogeneous area as follows:

The first two types of asbestos pose the greatest risk of exposure if the asbestos becomes friable. Friable materials can be crumbled, pulverized, or reduced to powder by hand pressure. All of the materials mentioned above have the potential to become friable during renovation, demolition, installation, and maintenance activities.

Friable ACM and Non-friable ACM materials will also be categorized as follow:

The U.S. Environmental Protection Agency (EPA) distinguishes between friable and non-friable forms of ACM. By definition ACM is any material or product which contains more than 1 percent (1%) asbestos (AHERA, OSHA definition), a Friable Asbestos Containing Material is any materials that contains more than 1% asbestos and can be “crumbled, pulverized, or reduced to powder by hand pressure when dry.” It is generally understood, that Friable ACM is thought to release fibers into the air more readily, however, many types of non-friable ACM can also release fibers if disturbed.

Materials Conditions

The survey list each condition of asbestos containing material found in location of scope of work. The condition of each material is listed as good, damaged or significantly damaged using the criteria listed below.

Damaged ACM: Material which has deteriorated, delaminated, water damage, lacks cohesion, is crumbling, gouged, marred heavily, abraded, or in any way has lost its structural integrity over more than 1% but less than 10% if the surface area If the damage is evenly distribute, or less than 25%, if the damage is localized in one area of the homogeneous area.

Significantly Damage: Material which has deteriorated, delaminated, water damage, lacks cohesion, is crumbling, gouged, marred heavily, abraded, or in any way has lost its structural integrity over 10% of the surface if evenly distributed on the surface or over 25% damage localized in one area of the homogeneous area.

Good Condition ACM: ACM with no visible damage or deterioration in less than 1% percent of the material and/or covering.

INSPECTION RESULTS – ASBESTOS

The following contains the summary of the suspect asbestos containing materials sampled during this inspection, including the location and laboratory analysis.

Positive ACM’s are distinguished in “bold”. Samples collected and found not to contain asbestos are classified as being None Detected “ND”. The complete sampling results can be found in the attachment section of this report. “Presumed” are those materials not sampled and identified as presumed by the Certified Asbestos Consultant. All floor tiles and associated flooring mastic adhesives are presumed asbestos containing based on previous sample data and contract requirements to prevent destruction to intact materials.

Note: The listed quantities are for budgetary information and are not to be used for bidding purposes. The contractor has the sole responsibility for confirming the location, quantity and degree of difficulty in impacting the identified materials.

Material Categories identified as, Surfacing (Surf), Thermal System Insulation (TSI) and Miscellaneous Materials (Misc). The quantities listed are for budgetary purposes only. Contractors completing proposals for the removal of asbestos containing materials are responsible for verifying the location, quantity, degree of difficulty and necessity for removing the identified materials. *Estimate provided for positive asbestos content only.

Joint compound results were found to contain a “Trace” <1% Chrysotile asbestos. In compliance with NESHAP Title 40, CFR Subpart 61, point counting procedures and SCAQMD Rules 1403, this material should be presumed to be above 1% asbestos content unless PLM point count analysis is performed to confirm content of asbestos below 1%. No previous sample results for point count analysis have been provided in the previous assessment reports.

DISCUSSIONS AND RECOMMENDATIONS (ASBESTOS)

• Asbestos building materials have been confirmed present in various materials within the scope of work at the facility. The materials found within the Surgical Clinic 2E South work area will require complete removal to accommodate demolition and renovation activities within the facility. Spot abatement may be necessary in the Interstitual spaces should renovation activities need to access utility, duct, and service lines.

• The facility has had various renovation activities and previous samplings reports may not reflect the current conditions or materials present. Where material could not be confirmed as previously sampled and inspected, these materials have been listed as presumed asbestos containing by the Certified Asbestos Consultant. These materials will require abatement as an asbestos containing material unless renovation and demolition activities allow for the specific material bulk sampling to determine asbestos content through laboratory analysis.

• Joint compound results were found to contain a “Trace” <1% Chrysotile asbestos.

• Leveling compound materials sampled from the Interstitual floors have indicated the materials to be non-asbestos containing, however core sampling from the bottom decks impacting plaster, drywall and joint compounds have not been confirmed. The ceiling deck materials should be handled as a presumed asbestos containing materials until specific material bulk sampling is performed to determine asbestos content through laboratory analysis.

LEAD-BASED PAINT INSPECTION REPORT

FACILITY: ........................ VA LOMA LINDA

VAMC SURGICAL CLINIC 2E SOUTH

11201 Benton Street Loma Linda, California 92357

INSPECTION DATE: ....... October 25, 2019

INTRODUCTION

DAVY Architecture, (referred to hereafter as the Client), retained ENCORP to conduct a lead-based paint inspection of the Veterans Administration Loma Linda, VAMC Renovations of the Surgical Clinic 2E South, at 11201 Benton Street, Loma Linda, California 92357. The inspection was conducted in order to determine the presence of lead in building materials that may be impacted in upcoming renovation/demolition activities at the site. Please note this inspection is limited to those areas which were determined to be impacted by planned renovations and representative site visits.

ENCORP’s certified representatives conducted a visual investigation of the property to identify and quantify all suspect lead containing materials. Upon completion of the visual investigation, building materials were grouped into sampling testing combinations or representative surfaces. ENCORP’S Certified Lead Inspector and Risk Assessor, Mr. Alexander Blankevoort, and Lead Certified Sampling Technician Mr. Jesus Roman, performed preliminary site inspection on October 25, 2019.

INSPECTION RESULTS -- LEAD (XRF Analysis)

Lead-Based Painted Components (LBPC’s) were widely used in the construction industry prior to 1978. In addition to finding the materials on accessible facility components and/or mechanical items, there are frequently shafts and/or other void spaces created by the structure itself, which will contain lead-based paint.

All facilities built prior to 1978 are suspect for lead containing materials. Paint bulk sample collection, sampling via X-ray Fluorescence (XRF) Spectrum Analyzer or a combination of both, are the only approved methods of identification recognized by federal and state regulations. This inspection was conducted via XRF Instrumentation. The parameters used to interpret the XRF results are outlined in the HUD Guidelines.

According to the United States Department of Housing and Urban Development's (HUD) Guideline Document, Guidelines For The Evaluation and Control of Lead Based Paint Hazards in Housing; and Section 1017 of the Residential Lead Based Paint Hazard Reduction Act (Title X), paint which is found to have a concentration of at least 1.0 mg/cm2 (milligrams per centimeters squared) by X-ray Fluorescence (XRF) or

0.5 percent (5000 parts per million) by weight is considered to be regulated as lead based paint. Based on the location of the subject property in San Bernardino County, the "abatement" level” (threshold) setting of 1.0 mg/cm2 was chosen for the inspection. It should be noted the CAL/OSHA considers all lead surface levels to be of concerns.

The scope of work performed by ENCORP was to conduct a U.S. Department of Housing and Urban Development (HUD) LBP Inspection of the exterior and interior substrates of the Site building (s). The following listed regulations were applied:

• 24 CFR Part 35.80-35.98 and 35.3120(b) – U.S. Department of Housing and Urban Development

• Toxic Substances Control Act (TSCA) Section 406

• 40 CFR 745.103 – Environmental Protection Agency (EPA)

• Title 17 Section 35000 – Code of California Regulations

• Cal/OSHA Title 8 Section 1532.1 – California Occupational Safety and Health Administration

• Cal/OSHA Title 8 Section 5194 – California Occupational Safety and Health Administration

• Title 17, CCR, Division 1, Chapter 8 and 24 CFR Subtitle A, Part 35.125, Form 8552 Notification

SAMPLING METHODOLOGY

The Niton XRF Spectrum Analyzer was utilized for the analysis of suspect lead-based painted materials. In this method of analysis, the material is exposed to X-Rays or other high-energy radiation (such as gamma rays), which causes lead to emit X-Rays with a characteristic frequency. The intensity of this radiation is measured by the instrument’s detector and is then converted into a number that represents the amount of lead in the material per unit area, usually milligrams per square centimeter (mg/cm2).

Analytical sensitivity of the of the XRF measurement methodology reports positive lead-based content results at or above 1.0 mg/cm2, presence of lead content result between 0.1 and 0.9 mg/cm2, and negative results at less than 0.1 mg/cm2.

Testing of the painted surfaces was performed in accordance to inspection protocol in Chapter 7 of the HUD Guidelines. In every “room equivalent” within the tested property, one representative surface of each “testing combination” was tested. Commonly encountered interior components tested (if painted or varnished) included walls, baseboard, doors, door trim, jambs, windows assemblies, trims and windowsills. Typical exterior components that were tested included the walls, fascias, doors and assemblies, and window assemblies. An appropriate number of readings were collected from interior and exterior building components and common areas. Multiple readings were collected to resolve inconsistencies in the test results.

The XRF used was the Niton LLC, Model XLp300, manufactured by Thermo Scientific. No time setting is required with this option since the device automatically adjusts its reading time to the different paint substrates for precision. The duration of each test result was determined by the substrate density in combination with the age of the radioactive source of the device and the actual reading relative to the "abatement" level (threshold) chosen.

An XRF Performance Characteristic Sheet (PCS) developed jointly by HUD and the Environmental Protection Agency (EPA) for the Niton LLC, Model XLp300 was used. The PCS provides information necessary to conduct an inspection of LBP using specific XRF. Based on the PCS, no inconclusive readings on brick, concrete, drywall, plaster, or wood substrates are reported.

Field calibration checks were performed prior, during, and after each XRF lead inspection (at least within 4 hours) to determine that the device is functioning within acceptable limits (tolerance) determined by the manufacturer. Three readings of the Standard Reference Material (SRM) paint film, developed by the National Institute of Standard and Technology (NIST) were taken in the confirmation mode option during each calibration check. Each set of readings were averaged and compared to the PCS calibration check limit for the device. Maps are provided with locations of positive lead-based sampling results. A copy of the Site Map(s) is contained in the field documentation section of the report.

INSPECTION RESULTS -- LEAD (XRF Analysis) The HUD and EPA have set a lead level of 1.0 mg/cm2 as being a regulated lead-containing material.

Analytical sensitivity of the of the XRF measurement methodology reports positive lead-based content results at or above 1.0 mg/cm2, presence of lead content result between 0.1 and 0.9 mg/cm2, and negative results at less than 0.1 mg/cm2. CAL/OSHA considers all lead surface levels to be of concerns. Items in “Bold” indicate lead-based paint content above HUD/EPA lead-based content.
