Consequently, no assessment or response actions were deemed necessary for any of the identified homogeneous areas. The plan designated Thomas Rattler or Ken Taylor as the facility's designated persons responsible for implementation, with contact through Charles Ballard at the VA Engineering Services (214-857-1069). The inspectors were Texas-licensed asbestos consultants Melissa Kiser (DSHS License #10-5436) and Eric Eitzen (DSHS License #10-5349). Per AHERA requirements, the facility must undergo three-year reinspection and management plan updates, with six-month visual inspections conducted by the designated person to monitor for any material condition changes.

MANAGEMENT PLAN FORMAT
SECTION I: This section contains general information regarding the AHERA survey and management plan, required information, and a facility description.
SECTION II: This section contains general information, sampling information, and Homogeneous Sample Area Summary. The laboratory analytical report for the bulk samples collected during the 2010 inspection has been included. Additionally, information on the survey report and classification of homogeneous areas is located in this section. Drawings depicting the locations of all bulk building material samples collected and asbestos-containing homogeneous area locations can be found in this sub-section.

AHERA ASBESTOS INSPECTION
SECTION I - GENERAL INFORMATION
INTRODUCTION
Micro Air of Texas, Inc. (Micro Air) was contracted by the Veterans Administration North Texas Health Care System (VANTHCS) located in Dallas, Texas to perform an asbestos inspection and generate a management plan in accordance with guidelines established in 40 CFR 763, the Asbestos Hazard Emergency Response Act of 1987 (AHERA), for the aforementioned facility located in Dallas, Texas. This facility consists of numerous buildings constructed between 1940 and 2009. This particular report pertains to Building 2J, which currently houses numerous clinics, surgery rooms, ICU, and radiology. Based on information we obtained from VA personnel, this building is thought to have been constructed in 1998.
Micro Air performed a comprehensive asbestos survey of Building 2J on October 18-20, 2010.
After the comprehensive asbestos survey results were compiled, a Management Planner accredited by an EPA-approved training provider and licensed as an Individual Asbestos Consultant in the state of Texas, reviewed the analytical results of the inspection and current physical condition assessments. The management planner then determined the potential hazard posed by the ACM and evaluated and selected control / response options based on all compiled information. This information was then composed and written into a final report.
The federal National Emission Standard for Asbestos (NESHAP) and the Texas Asbestos Health Protection Act (TAHPA) both require that prior to any demolition or renovation activity in a building, a thorough inspection of the affected facility or part of the facility where the demolition or renovation operation will occur, must be performed for the presence of asbestos, including Category I and Category II non-friable asbestos-containing material (ACM).
Additionally, TAHPA requires that a minimum of three samples be collected from each homogeneous sampling area (material type) identified in the building. The work area and immediately surrounding areas which could foreseeably be disturbed by the actions necessary to perform the project must be inspected and sampled as applicable prior to renovations or demolition. While the VANTHCS is not subject to the TAHPA regulation, due to its exclusion as a federal facility, Micro Air collected bulk samples in accordance with TAHPA and AHERA protocol at the request of the VANTHCS engineering personnel.

REQUIRED INFORMATION
In accordance with AHERA protocol, the VAMC located in Dallas, Texas is required to name a person who shall be responsible for implementation of the management plan (Designated Person - see Section V). Per the Occupational Safety and Health Administration (OSHA) 1926.1101, construction industry standard, maintenance personnel should be made aware of the types and locations of all facility ACM. These employees are required to have adequate asbestos awareness training (minimum 2 hours), and all maintenance personnel who may come in contact with or disturb ACM are required to receive a minimum of 16 hours of asbestos training in asbestos hazards and recognition of ACM. If the owner wishes to perform ACM activities with its own personnel, the owner is responsible for obtaining the required EPA training in accordance with the Model Accreditation Plan (MAP), which is located in Appendix C, Subpart E of 40 CFR, 763 (AHERA). Additionally, all OSHA requirements must be met, as well (i.e., asbestos physical, etc.). Otherwise, it is recommended that all ACM activities be performed by a Texas licensed Asbestos Contractor, and project oversight be conducted by a Texas licensed Asbestos Consultant Agency.
In accordance with the VA directive to comply with AHERA, the entire facility must be re-inspected and an reassessment of ACM conducted by an accredited inspector at least once every three years, and the management plan should be updated by a licensed Asbestos Inspector / Management Planner or Individual Asbestos Consultant. Additionally, visual inspections of the ACM must also be made by the Designated Person for the facility at least once every six months to determine whether any areas of the material have changed in condition, or if there are any other factors which may necessitate abatement actions.
FACILITY DESCRIPTION
Building 2J is a six story building with a seventh floor penthouse, which houses a mechanical room and elevator equipment. This building also has a basement and a sub-basement. It is approximately 1,212,934 square feet in size, and currently serves as the Clinical Addition at the Dallas VAMC. The construction date of this building is believed to be 1998.
The general construction of the building is a concrete slab foundation with red exterior brick, a flat tar and gravel roof, and the interior finishes consist primarily of sheetrock walls (ceramic tile walls are located in restrooms and on floors in certain areas of the building), lay-in ceiling tiles, floor tile, rubber sheet flooring, carpeting over concrete floors, and some areas have unfinished concrete floors. Sprayed-on fireproofing was noted above ceiling tiles on metal I-beams. The second, fourth, and sixth floors of the building are known as interstitial floors, as they contain mechanical equipment throughout, and they are not used for patient care. The first, third, and fifth floors are used for patient care, and the basement is utilized primarily by VAMC personnel, as it contains office space, storage, and the morgue. The sub-basement has a dirt floor, and it contains mechanical rooms in various locations, piping is located throughout the sub-basement, and some HVAC equipment was also found in the sub-basement.

All pipe insulation noted in this building was fiberglass with a white paper / foil wrap, PVC fittings, sealed with white mastic. HVAC ducts in the building were observed to be covered with a silver foil and fiberglass insulation sealed with white mastic. HVAC ducts and equipment in mechanical rooms were not exteriorly insulated, and the metal ducts were noted to have gray mastic seam sealant (interior fiberglass insulation was noted in these particular ducts). Vibration dampeners were noted to be rubber.
Micro Air did not have access to several areas located on the first, third, and fifth floors of the building. Many of the areas were operating rooms, CAT scan rooms, surgery recovery areas, the emergency room, etc. However, based on visual observations, Micro Air was able to determine that the homogeneous areas identified and sampled in other areas of the building appeared to be consistent throughout the building.
Micro Air identified two stainless steel sinks with gray soundproofing during the course of this comprehensive inspection. There may be additional sinks with soundproofing material located in other sections of the building that were not accessible at the time of this inspection. If additional sinks are identified that contain soundproofing, they should be assumed ACM until they can be tested and proven otherwise.

SECTION II- ASBESTOS LABORATORY ANALYTICAL REPORT
METHODOLOGY
Micro Air of Texas, Inc. performed a comprehensive asbestos survey on Building 2J on October 18-20, 2010 in accordance with AHERA and TAHPA sampling protocol and assessment factors. The inspection was performed by Melissa Kiser, an accredited asbestos inspector who is licensed as an Individual Asbestos Consultant by the state of Texas (license #10-5436) and Eric Eitzen, an accredited asbestos inspector who is licensed as an Individual Asbestos Consultant by the State of Texas (license # 10-5349). Micro Air of Texas, Inc. is licensed as an Asbestos Consulting Agency (#10-0110). All samples collected during this inspection were analyzed by J Resources, Inc., whose laboratory is accredited by the National3
Institute of Standards and Technology (NIST) under the National Voluntary Laboratory Accreditation Program (NVLAP) program (accreditation #200525-0) and licensed by the state of Texas (license #30-0273). Laboratory analysis of collected samples was performed in accordance with EPA and NVLAP prescribed methodologies for Polarized Light Microscopy of bulk material samples for the presence of asbestos. The results of laboratory analysis of collected bulk material samples are included in this section for reference.
A total of 118 bulk material samples were collected and analyzed by PLM during this comprehensive survey. Federal regulations require that each identifiable layer of an individual bulk building material sample be analyzed separately for asbestos content. In addition, federal regulations state that if any one sample from a homogeneous area is found to be asbestos-containing, then the entire homogeneous area is considered asbestos-containing, regardless of the results from the remaining samples from that homogeneous area.
CLASSIFICATION
All materials which were sampled as being suspect for containing asbestos or were assumed to be asbestos-containing were grouped by sample type into Homogeneous Sampling Areas for purposes of assessment and quantification. Homogeneous materials are those which have similar appearance, texture, and function. If the inspector believes these materials to be the same, even if they are located in separate areas of a building, they may be grouped into the same sampling set. For purposes of this inspection report, the Homogeneous Area is designated as : HA. Each Homogeneous Area is given a number to distinguish it from other Homogeneous Areas (Example: HA-01A).
A CAD drawing(s) demonstrating the location of each collected bulk building material sample has been included in this section.
LIMITATIONS
Micro Air’s scope of services was limited primarily to accessible interior building materials.
While Micro Air made every effort to perform a thorough survey, the possibility exists that materials containing asbestos were not identified, inaccessible, or different from those identified at other locations. The findings and opinions presented are indicative of conditions on the date of our site inspection and should not be relied upon to represent conditions at substantially later dates, due to possible changes in the condition of building materials, renovations, and repairs. Additionally, most of the buildings located at this VA facility have steam pipes running underground to supply heat to each building. This material was inaccessible at the time of this inspection. All pipes located in wall chases, underground, in the crawlspace of a building, etc., is assumed to be ACM until tested and proven otherwise.

SECTION III - ACM Material Assessment and Quantification
Each area where ACM was identified in the asbestos management plan was visually inspected and assessed as to physical condition and potential for airborne asbestos fiber release.
Included in the assessment factors are such items as friability, accessibility, amount of current damage, amount of airflow, vibration, or abrasion to which the material is subject, potential for water damage, type and number of personnel normally located within each homogeneous sampling area, proximity to normal maintenance activities and potential for disturbance of such materials under normal operating conditions, potential for deterioration, and overall health risk of the material.
Final hazard assessment ratings are based upon the system designed in the EPA AHERA protocol for asbestos building inspectors. The material assessment categories in order of decreasing critical importance are as follows:
1. Damaged or Significantly Damaged TSI ACM
2. Damaged Friable Surfacing ACM
3. Significantly Damaged Friable Surfacing ACM
4. Damaged or Significantly Damaged Friable Miscellaneous ACM
5. ACBM with the Potential for Damage
6. ACBM with the Potential for Significant Damage
7. Any Remaining Friable ACBM or Friable Suspected ACBM
These assessment categories were used to determine appropriate response actions for all ACM, which are included in Section II of this report.
