About this file
This is an Indoor Air Quality Investigation Report prepared by AEHS, Inc. for the Audie Murphy VA Medical Center regarding mold contamination in Area 4B, 4th Floor – West. The investigation was conducted on July 22, 2026, by Ronald Bishop, MPH, CIH, with assistance from Erica Madrid. The investigation employed visual assessment with photographic documentation, particle dosimetry at multiple locations (occupied areas, above ceiling, and outside), thermal imaging, temperature and humidity measurements, and mold sampling using both air and bulk/tape lift/swab methods.
The investigation identified significant mold growth attributed to water intrusion in ventilation ducts above the ceiling. Key findings include water staining throughout ceiling tiles, rusted ceiling grids, saturated duct insulation, visible mold on duct insulation and pipe insulation, and mold present on the solid ceiling of Room B431. Particle dosimetry revealed significantly higher particle counts in the Interstitial Building Space (IBS) compared to occupied areas. Temperature and humidity measurements showed occupied areas were not conducive to mold growth, while the IBS was conducive to mold growth. Laboratory analysis detected high amounts of Cladosporium species outdoors and elevated Penicillium/Aspergillus species levels indoors in both occupied and IBS areas. MoldSCORE™ statistical analysis indicated all samples showed high or medium-high likelihood of indoor mold origin. The report concludes that ventilation duct leaks coupled with insulation integrity failures caused mold amplification, and the positive pressure in the IBS causes mold spore permeation into occupied areas. The investigation recommends replacing insulation on all ducts and pipes after repair of duct leaks and sharing results with occupants.

San Antonio, TX 78229
Mr. Martinez:
This letter provides the written results of the limited Indoor Air Quality Investigation of
4B, 4th Floor – West. The investigation occurred on 22 July 2026 with the primary purpose to determine the impact of the water intrusion in the ducts above the ceiling.
The investigation was conducted by Ronald Bishop, MPH, CIH. He was assisted by Erica
Madrid. Ron is a Certified Industrial Hygienist, Certified Safety Executive, Certified
Environmental and Safety Compliance Officer as well as a Texas Department of Licensing and Regulation licensed Mold Assessment Consultant (MAC 106), a Texas Department of
State Health Services licensed Lead Risk Assessor and Project Designer and Asbestos
Consultant, a Green Consultant, and a Radon Measurement Professional.
The specific objectives of this investigation are depicted below:
1. Visual Assessment with Photographic Documentation.
Perform a detailed visual assessment.
Provide photographic documentation.
2. Particle Dosimetry.
Perform Particle Dosimetry throughout the area.
Perform Particle Dosimetry above the ceiling.
Perform Particle Dosimetry outside the building at the beginning of the sampling episode.
3. Temperature and Humidity Measurements.
Measure the temperature and humidity measures at all locations of the particle dosimetry.
4. Thermal Imaging.
Perform thermal imaging throughout the area to depict moisture and mold.
5. Mold Sampling.
Air:
Perform non-culturable air sampling outside the building at the beginning and end of the sampling episode.
Perform non-culturable air sampling at three locations within the occupied area of concern.
Perform non-culturable air sampling at three locations above the suspended ceiling within the hallway of the occupied area of concern.
Bulk/Tape Lift/Swab:
Collect Bulk/Tape Lift/Swab samples as appropriate.
Findings and Discussions.
1. Visual Assessment.
• Photographic documentation is at Enclosure 1.
• Ceiling.
o The halls had a suspended ceiling of tiles.
o The patient and other rooms had solid ceilings.
o The utility space (IBS) was directly above the ceiling accessed by catwalks and containing water pipes, ventilation ducts, etc.
• Water staining on the ceiling tiles was present throughout the area.
• The ceiling grid was rusted at multiple locations.
• The insulation on the ventilation ducts was saturated with water which was most likely due to leaks in the hard supply (cool) ducts.
• Visible mold was present on the duct insulation, pipe insulation, and backside of the ceiling tiles.
• The IBS was under positive pressure.
• Visible mold was present on the solid ceiling of Room B431 near the light fixture due to normal air leakage.
2. Particle dosimetry.
• The result of the particle dosimetry is at Enclosure 2.
• The particle discernment indicated mold growth both within the occupied space as well as within the IBS.
• The total number of particles was significantly higher in the IBS when compared to the occupied space.
3. Temperature and Relative Humidity.
• All measurements within the occupied area were at levels that were not conducive to mold growth.
• All measurements in the IBS were at levels that were conducive to mold growth.
4. Thermal Imaging.
• Thermal images depicting typical locations throughout the area are at Enclosure 3.
• The thermal imaging verified the mold growth and moisture.
5. Laboratory Results:
• The actual laboratory results are Enclosure 4.
• Air:
o High amounts of Cladosporium species were detected as to be expected.
o Elevated Penicillian/Aspergillis species levels were found inside in both the occupied area as well as the IBS.
o Statistical analysis: The MoldSCORE™, which is a unique statistical method for examining air sampling data, was used to evaluate the air sampling results. It compares air sample results with the outdoors and scores each location between 100 and 300, with 100 indicating a greater likelihood that the airborne indoor spores originated from the outside, and
300 indicating a greater likelihood that they originated from an inside source. The Result displayed is based on the numeric score given and will be either Low, Medium, or High, indicating a low, medium, or high likelihood that the spores detected originated from an indoor source. All
MoldSCORE™ results, of all samples, were high medium or high. This means that it is likely that the mold originated indoors.
o The Spearman Rank Correlation and Agreement ratio statistics support the
MOLDSCORE™ results.
o The predominate spores found interior were those of Penicillium/
Aspergillus types while the exterior sample depicted the primary species to be Cladosporium. Cladosporium spores are found over 90 % of all outdoor samples taken in the San Antonio area.
• Bulk:
o A high amount of Cladosporium species was present.
The analyses for mold were performed by EMLab P&K which is licensed in Texas as well as AIHA Certified.
Conclusions.
1. The leaks in the supply ventilation ducts coupled with the lack of total integrity in the insulation were causing the mold amplification.
2. The IBS being under positive pressure, due to the ventilation duct leaks, will cause permeation of mold spores into the occupied area to include patient rooms.
3. Temporary solutions such as wiping the mold with a biocide will be ineffective and not cost effective.
Recommendations.
1. Share the results of this investigation with the occupants.
2. Replace the insulation on all ducts and pipes after repair of the duct leaks.
Note: This may require removal of the hard ceilings as well as the ceiling tiles.
If you have any questions or desire additional information, please contact Ron Bishop at telephone (210) 656-9300.
Sincerely, Ronald M. Bishop, MPH, CIH

Spore types listed without a count or data entry were not detected during the course of the analysis for the respective sample, indicating a raw count of <1 spore.
† The spores of Aspergillus and Penicillium (and others such as Acremonium, Paecilomyces) are small and round with very few distinguishing characteristics. They cannot be differentiated by non-viable sampling methods. Also, some species with very small spores are easily missed, and may be undercounted.
††Background debris indicates the amount of non-biological particulate matter present on the trace (dust in the air) and the resulting visibility for the analyst. It is rated from 1+ (low) to 4+ (high). Counts from areas with 4+ background debris should be regarded as minimal counts and may be higher than reported. It is important to account for samples volumes when evaluating dust levels.
The analytical sensitivity is the spores/m^3 divided by the raw count, expressed in spores/m^3, per spore and per sample.
For more information regarding analytical sensitivity, please contact QA by calling the laboratory.
‡ A "Version" indicated by -"x" after the Lab ID# with a value greater than 1 indicates a sample with amended data. The revision number is reflected by the value of "x".
§ Total Spores/m3 has been rounded to two significant figures to reflect analytical precision.

§ Total Fungal Spores has been rounded to two significant figures to reflect analytical precision.
Spore types listed without a count or data entry were not detected during the course of the analysis for the respective sample, indicating a raw count of <1 spore.
The limit of detection is 1 spore per area analyzed; Analytical Sensitivity is 1 spore per unit times the dilution factor.
Where tape lifts are performed for bulk sample analysis, the unit reported is specific to the area of tape analyzed. Where swabs are performed, the unit reported is specific to the swab analyzed.
Due to the inherent inhomogeneity and variable recovery rates of surface fungal samples, all reported spore counts are extrapolated estimates. Counts of fewer than 400 spores or greater than 40,000 spores are considered imprecise and not reproducible due to limitations in the counting method and equipment sensitivity.
All samples were received in acceptable condition unless noted in the Report Comments portion in the body of the report. Due to the nature of the analyses performed, field blank correction of results is not applied. The results relate only to the samples as received and tested.
