Water Tank Inspection Report Summary

This is a Water Tank Inspection Report prepared by TankSpek, LLC for the Veterans Healthcare System of the Ozarks at the VA Medical Center in Fayetteville, Arkansas. The inspection was conducted on May 22, 2026, on a 100,000-gallon elevated water tower with an approximate height of 140 feet. The facility is a multi-column, elevated, double ellipsoidal welded steel tank constructed with an unknown date and erection contractor. The inspection evaluated the tank's condition across multiple categories including structural integrity, sanitary conditions, safety compliance, and coating systems.

Overall condition assessments rate the tank as follows: structural condition is GOOD, sanitary condition is FAIR, interior coating is FAIR, exterior coating is POOR, and safety condition is FAIR. Key findings indicate visible sediment accumulation requiring cleaning, moderate exterior corrosion from UV degradation, interior coating staining from unsealed roof joints, and several safety deficiencies including inadequate fall arrest cable tension, undersized balcony passthrough opening (16 inches versus required 24 inches), and damaged electrical components. Critical safety concerns include RF exposure from cellular equipment exceeding OSHA occupational limits, requiring equipment deenergization during worker presence, and security vulnerabilities including damaged perimeter fencing and an accessible onsite ladder enabling unauthorized access. Recommended corrective actions include scheduled tank draining and cleaning, exterior coating application, interior coating improvements at year four, safety climb device adjustments, roof vent screening replacement, and aviation beacon bulb replacement with FAA-compliant incandescent bulbs. The report notes that cost estimates are not provided and that a supplemental report on cathodic protection system maintenance will be submitted separately.

No action

PLACARDS /SIGNAGE

No warning signage for EM or RF exposure as required by OSHA Confined Space placard in place at Riser Manway Entry.

Maintenance needed

TELECOM ITEMS

Components installed onto balcony railing create added lateral wind loads and stresses. Additional supports should be added to prevent damage to balcony railing and floor. Alternatively, components should be installed at roof level to a dedicated corral. Balcony floor is a structural component and is at risk of damage or cracking from repeated or continuous movement generated by telecom items attached to railing.

Maintenance needed All costs should be absorbed by Telecom Installer(s)

RF EXPOSURE

Electromagnetic (EM) and Radio Frequency (RF) readings taken during inspection show the existing cellular components emit frequencies exceeding occupational exposure limits for personnel as defined by OSHA. Future work for any extended period will require these components to be deenergized in accordance with OSHA Lock Out/ Tag Out Procedures during the presence of workers.

Corrective Action for Worker Safety to be implemented and enforced

OBSTRUCTIONS

Impediments and safety obstructions were observed and noted.

Telecom connectors and wiring connected to siderail of roof ladder. Various trip hazards noted -poorly secured wiring/coax.

Maintenance needed All costs should be absorbed by Telecom Installer(s)

VA Medical Center 100MG Storage Tank 2026 INSPECTION

OVERALL SAFETY CONDITION : FAIR with exceptions as noted

Some deciencies indicated for any Corrective Action or Maintenance are documented and ready for scheduled service. Other items can be progressively corrected as needed under a Not to Exceed option of a proposed maintenance plan.

Testing showed that RF levels near the cellular equipment exceed occupational safe distance requirements. Equipment will need to be isolated if workers are near this equipment for longer than a few moments. Safety conditions relating to telecommunication installation and improvement should be corrected by the telecom equipment owner (e.g., provider, contractor, company, etc.). ALL costs should be on the equipment owner. Future work should be restricted or limited access should be enforced, along with a detailed work plan for any additions, changes or modications. Oversight and follow-up inspection by a neutral third party will ensure proper installation and procedures are followed.

5.0 Safety Observations (Cont.)

Maintenance Needed Schedule with any option year {Misc. Repairs Not to

Exceed}

FALLING OBJECTS

Loose items, hardware, spare telecom parts, etc. should be removed to protect passers-by from potentially falling objects.

Primary roof vent cover should have a permanent tether installed to prevent accidental drops during maintenance or complete removal in the event of wind/storm damage.

Maintenance Needed Schedule with any option year {Misc. Repairs Not to

Exceed}

ABANDONDED or

OUT-OF SERVICE

ITEMS

Components no longer in use, abandoned, or damaged should be removed periodically as part of routine maintenance.

8.0 INTERIOR COATING EVALUATION

OVERALL INTERIOR COATING CONDITION : FAIR

Recommendation: Proceed with scheduled interior coating improvements at year four of an ongoing maintenance plan. Specify sealing of the open roof joints with an NSF approved urethane (exible) joint sealant during the coating procedure to prolong coating life and prevent unsightly staining.

Fair Condition = Light-to- moderate coating damage; Localized or isolated areas of debonding; (Acceptable) weathering on aged coatings; (Acceptable) abrasions on aged coatings. Light-to-Moderate mold or debris staining. No excessive corrosion, No large areas of debonding. Acceptable adhesion of existing coatings; Often repairable with maintenance coatings; Typically, acceptable for overcoating.

VA Medical Center 100MG Storage Tank 2026 INSPECTION

9.0 EXTERIOR COATING EVALUATION

OVERALL EXTERIOR COATING CONDITION : POOR

Recommendation Consider coating the exterior. Existing coating is no longer adequately protecting the exterior surfaces. The existing coating is likely still in a condition that maintenance or overcoating will extend the lifespan another 7-12 years. A full exterior renovation is inevitable. Coating samples were not collected and tested for presence of lead or heavy metals during this inspection process. Sampling will be necessary to determine if containment & abatement procedures are necessary in the event of a full renovation.

Poor Condition = Large % of surface area damage, debonding or (Unacceptable) weathering on aged coatings. Heavy mold, mildew, or mineral staining. Poor adhesion of existing coatings; Often prolonged with maintenance coatings; Conditionally acceptable for overcoating.
