LIMITED ASBESTOS AND LEAD SURVEY

1.0 Introduction

Riverfront Safety & Health, LC (Riverfront), a Service Disabled Veteran Owned Small Business, has performed a limited asbestos and lead identification survey for the 35% Schematic Design (SD) phase of the Replace Windows, Building 1 Project (VA Project 657-19-110JC) at the John Cochran VA Medical Center in St. Louis, MO (The Project). The principal contacts for this project were Ben Ramsey of Spur Design and Michael Gomez of the VA Medical Center. The following report is provided for your review and for future design considerations related to asbestos and lead issues associated with demolition and reinstallation of exterior windows.

The limited asbestos and lead field survey was conducted by Justin Rhyneer, Zach Haselhorst and Gerald Reimer, all Missouri-licensed Asbestos Inspectors. The limited lead survey was conducted Zach Haselhorst, a Missouri-licensed Lead Risk Assessor.

The Building 1 exterior (perimeter) window inspections were conducted on April 29, 30; May 1, 2, 3, 6; and June 26, 2019 (A & B Wing Window Demolition Pilot Study), with the following limitations:

• The fully occupied and functioning hospital itself limited access into rooms due to daily hospital functions. At the time of the inspection, exam and inpatient rooms were not inspected if the room was occupied by a patient, current exam was taking place, or an employee meeting was in progress. Also, keys issued to personnel did not always function on every lock and therefore the area was not accessed.

• Select windows throughout the hospital have been covered up during renovations by drywall walls, etc. Windows in these areas were not accessible and therefore not thoroughly inspected.

• Exterior sampling throughout the hospital was very limited. Most windows are screwed shut limiting access to the exterior on floors higher than the basement and first floor levels.

Only a few windows throughout the hospital opened for inspectors to gain access to exterior window components above the basement and first floor levels. This effort was further enhanced by the Window Demolition Pilot Study (Pilot Study), which provided unlimited access to representative window systems.

• In many situations, the inspector noted paint over interior window caulk. This ultimately changes the appearance of the material and makes it difficult for the inspector to classify the specific caulking material and color. This effort was further enhanced by the Window Demolition Pilot Study (Pilot Study), which provided unlimited access to representative window systems.

Prior to the survey, it was reported that 1,050 windows located in Wings A through E were to be considered for replacement. Wing A represents the original structure constructed in the 1950s with later additions B through E. Numerous rehabilitations have been constructed within the A, B, and C wings but a full history of window replacements was not available. Based on the age of the facility, prior survey results, and potential methodology of contract work, anticipated suspect materials included interior and exterior window caulks and seals, framing caulks, glazing, waterproofing, mastics, plaster, and Lead Based Paint (LBP) on window components or adjacent materials. Ceiling Grid systems and drywall material were deemed not to be ACM according to prior surveys. It was not well understood to what extent elevations above ceiling grid would be impacted by window removal, which shall be further defined during the ensuing design phases of The Project.

During assessment of existing field conditions (see 15% submittal) it was determined that a proper and adequate assessment of older windows systems must include an invasive/destructive inspection of concealed window systems, particularly in older building wings such as A & B Wings, whereby newer window system treatments (i.e. aluminum cladding) are commonly installed over remnants of original construction, including components those commonly exhibiting lead based paint and/or asbestos containing caulking/glazing components, leaving them concealed and otherwise undefined. While such a window system may already exhibit exposed environmental risk factors, thus requiring remediation prior to new construction to some extent, the existence of concealed lead/asbestos building materials is likely to present significant additional impacts to construction budget estimates and schedules. As such, the Riverfront inspection team, with substantial assistance from the VA, identified a representative set of window systems from both A Wing and B Wing for proper inspection, including destructive disassembly as necessary, to provide full access for visual assessment and additional bulk sampling. Such an inspection, also referred to herein as the Pilot Study, was carried out on June 26, 2019 under the direction of Riverfront representative Mr. Justin R. Rhyneer, CIH, with assistance from two Missouri Licensed asbestos abatement workers (see Attached for details).

This report summarizes the asbestos and lead-related issues identified with existing conditions for this project. The sampling results have been summarized in Tables 1 and 2, with the laboratory analytical report and sample chain of custody contained in Appendix A and B.

2.0 Scope of Work

Riverfront provided an AHERA/EPA trained Asbestos and Lead Inspector/Risk Assessor to perform the following:

- Perform an initial non-destructive survey to identify the presence, location, quantity, appearance, and condition of all suspect building materials within the VAJC Medical Center Building 1 in St. Louis, MO identified by the VA as likely to be impacted during removal and reinstallation of approximately 1,050 exterior/perimeter window units. Standard EPA/AHERA field sampling methods were employed to ensure representative sample collection during this initial assessment effort.

- Identify principal locations where ceiling grid will be removed to allow for project work and access hazardous material potential.

- Identify key windows for destructive inspection purposes to access potential presence of additional/concealed asbestos containing caulking, waterproofing, and lead containing materials inaccessible during the initial survey.

- Provide a Missouri Licensed Abatement Contractor (Talbert ICS, Inc.) to demolish and remediate window systems within a negative pressure mini containment (built by VA for use by A/E) to expose potentially concealed original window framing systems in A & B Wings (oldest wings with original window systems likely concealed). Windows selected for removal by the VA and Riverfront for this Pilot Study included one window each in Rooms A904 and B015, considered to be representative of all window systems/units within A & B Wings, respectively. During this Pilot Study, the inspector was to obtain a representative number of samples of concealed suspect building materials, if any, from A & B wing window units likely to be disturbed by future window replacement. Following removal, using manual wet methods only, ensure the Abatement Contractor shall properly dispose of window component waste as asbestos containing materials. Thoroughly clean containments that will be subject to asbestos (PCM) and lead air clearance sampling. Once clearance inspection and air sampling has been conducted and passed using EPA/MDNR/VA thresholds, the VA was to tear down containments.

3.0 Limited Inspection Results

Asbestos Containing Materials - Inspection Results

All suspect asbestos materials sampled were classified as a miscellaneous (e.g., adhesives, caulks, seals, and glazing) or surfacing material in the case of plaster skim coat wrap (unique to windows in stairwells). After classifying the suspect material, homogeneous materials (areas of uniform color, texture, and general appearance) were delineated. The number of samples collected from each area varied based on the classification of the material and increased as the potential for a non-uniform mixture of asbestos in the material increased. Typically, three samples per homogeneous area were collected, with less collected in the event of limited access to homogenous material areas. Sampling locations were chosen to be representative of the accessible materials and collected randomly from all accessible areas of the building where material was available without destructive collection procedures. All sampling procedures were conducted using guidelines established by the EPA. Samples were analyzed using polarized light microscopy (PLM) analysis with dispersion staining (DS) according to U.S. Environmental

Protection Agency (EPA) Method 600 IR-93/116 which is presently recommended by the EPA.

Asbestos Containing Materials are defined by the EPA as having greater than 1% asbestos as measured by PLM methods.

Riverfront collected a total of 177 representative samples of suspected asbestos-containing materials related to proposed removal and reinstallation of windows in Building 1, A-E Wings.

All samples were transferred via written chain-of-custody, to EMSL, an AIHA & NVLAP accredited laboratory, for analysis. Sampled indoor and outdoor building materials included plaster wrap skim coat, sill adhesive, window seals, caulks, expansion caulks, glazing, and frame seals. The suspect materials, which were similar in general appearance and age of construction, were grouped into homogeneous material sets and sampled.

Eleven material groups tested positive as asbestos-containing materials (ACM) during this survey and will be affected by the project scope. The material groups are as follows:

• Black Textured Interior Window Caulk (exposed) bulk sample results were positive, containing 3-6% Chrysotile asbestos. The estimated quantity of known affected material was approximately 4,860 lineal feet (~ 18 LF per window x 270 windows). This black texture caulk was in good pliable condition and located at interior seams of some perimeter windows (wood to metal; wood to plaster; metal to metal).

The black textured caulk was used on numerous windows throughout (A.1, B.1, A.2, &, B.2 types) but appeared to be limited to Wings A and B. This window caulk is predominately located in Wing B with limited locations in Wing A, and not observed in Wings C-E whatsoever. Similar black textured caulk (newer black textured caulking) was sampled in Wing C and was “none-detected” (i.e. negative) for asbestos and should not be confused with that of A & B Wing as it had less texture and exhibited more of a “shiny finish” than the asbestos-containing homogenous material. An additional caulk is existing or co-existing in some locations and is recognized as shiny, pliable, nontextured black caulk that is non-detected for asbestos throughout.

The number of window locations where asbestos-containing black textured asbestos caulk exists is yet unknown. Many rooms with exterior windows were unavailable for inspection due to patient restrictions or covered by paint. It is unknown if this black textured caulk exists on wood window mouldings behind drywall surrounds in remolded areas (window types C & D) and/or orphaned windows behind new walls in the A & B wings, and cannot be reliably determined without room-by-room destructive inspections.

• Tan Exterior Caulk (concealed original construction) bulk sample results were positive, containing 6% Chrysotile asbestos. The estimated quantity of likely affected material is approximately 4,860 lineal feet (~ 18 LF per window x 270 windows). This concealed tan exterior caulk on window seams was discovered during the Pilot Study window removal process as being in very poor condition (due to historical weathering) and located at exterior seams (where wood/brick meet) of what should be presumed on all perimeter windows located in B Wing.

• Black/Brown Exterior Caulk on Aluminum Frame bulk sample results were positive, containing 3-5% Chrysotile asbestos. The estimated quantity of known affected material was approximately 4,860 lineal feet (~ 18 LF per window x 270 windows). This black exterior caulk on aluminum frames was discovered during the Pilot Study window removal process and was generally observed in good condition and located along the exterior aluminum/brick seams of what should be assumed on all perimeter windows located in B Wing.

• Brown Interior Caulk on Bottom Aluminum Trim (concealed original construction) bulk sample results were positive, containing 4% Chrysotile asbestos. The estimated quantity of known affected material was approximately 990 lineal feet (44” per window x 270 windows). This concealed brown interior caulk on bottom aluminum trim was discovered during the Pilot Study window removal processes in good condition and located along the aluminum to wood window sill interior seam and should be assumed present on all perimeter windows located in B Wing.

• Brown Interior Caulk on Aluminum Frame (concealed) bulk sample results were positive, containing 5% Chrysotile asbestos. The estimated quantity of known affected material was approximately 4,860 lineal feet (~ 18 LF per window x 270 windows). This brown interior frame caulk on aluminum framing was generally in good condition and located along the aluminum to wood interior seams of what should be assumed on all perimeter windows located in B Wing. The concealed brown interior frame caulk on aluminum framing was discovered during Pilot Study window removal processes.

• White Frame Caulk on Original Wood Window Framing bulk sample results were positive, containing 9% Chrysotile asbestos. The estimated quantity of known affected material was approximately 4,860 lineal feet (~ 18 LF per window x 270 windows). This white frame caulk on window was in good condition and located along the wood/brick interior seams of what should be assumed on all perimeter windows located in B Wing.

The concealed white frame caulk was discovered during window removal processes.

• Off-White Frame Caulk at Bottom Sill bulk sample results were positive, containing 6% Chrysotile asbestos. The estimated quantity of known affected material was approximately 990 lineal feet (44” per window x 270 windows). This off-white frame caulk was in good condition and located at the bottom of the window sill interior seams of what should be assumed on all perimeter windows located B Wing. The concealed off-white frame caulk was discovered during window removal processes.

• Beige Exterior Caulk (outer seam) bulk sample results were positive, containing 6-7%

11,700 lineal feet (18 LF per window x 650 units). This beige exterior caulk was in poor condition and located along the wood/brick exterior seams of what should be assumed on all perimeter windows located in A Wing. The concealed beige exterior caulk was discovered during window removal processes.

• Beige Exterior Caulk, Inner Bead, bulk sample results were positive, containing 7%

2,384 lineal feet (44” per window sill x 650 units). This beige exterior caulk, inner bead, was in poor condition and located along the original wood to stone window sill seams of what should be assumed on all perimeter windows located in A Wing. The concealed beige exterior caulk, inner bead, was discovered during window removal processes.

• Black Caulk Interior Wood Framing bulk sample results were positive, containing 6%

11,700 lineal feet (18 LF per window x 650 units). This black caulk on the interior wood sill was in generally good condition and located at window sill interior seams of what should be assumed on all perimeter windows located in A Wing. The concealed black caulk on the interior wood sill was discovered during window removal processes.

• Brown/Black Exterior Caulk bulk sample results were positive, containing 8% Chrysotile asbestos. The estimated quantity of known affected material was approximately 11,700 lineal feet (18 LF per window x 650 units). This brown exterior caulk was in poor condition and located along the aluminum to brick seams of what should be assumed on all perimeter windows located in A Wing. The brown/black exterior caulk was discovered during window removal processes.

Ceiling Tiles, brick, brick mortar, drywall, drywall joint compound, and general wall plaster were not sampled as a significant number of historic building samples from 1994 Roth Asbestos and Environmental Consultants, 2008 Altec Environmental Consultants & 2018 Riverfront Safety and Health Campus Surveys have shown these materials to be non-detect for asbestos.

It should also be noted that Riverfront utilized previous inspections, including the 2018 Riverfront Safety and Health Facility Re-Inspection, the 2008 Altec Asbestos Assessment as well as 1994 Roth Asbestos and Environmental Consultants Asbestos Survey, to identify ACM Thermal System Insulation above potentially impacted ceiling grids.

A summary of sampling efforts, including sample numbers, descriptions, locations, quantities, category (friable or non-friable) and analytical results are presented in the following Table 1.

The full laboratory analytical reports are included in Attachment A.

Lead Based Painted Window Components & Potential Associated Impacted Material Results

A targeted lead inspection was performed on representative indoor window assemblies to identify lead-based painted components and/or lead-based glazing on primary substrates potentially impacted by project work. Typical window trim, where existing, included sills, wood moulding, and quarter round. The typical window pieces had aluminum covering (black anodized) over stiles, rails, jambs, and sills. Primary adjacent substrates included painted plaster, drywall, historic metal moulding, glazed ceramic tiles, glazed ceramic blocks, and/or brick.

Window treatments which included window films on glass and valances were not surveyed.

Riverfront collected lead readings from window components and targeted surfaces to identify the presence of lead-based paint (LBP), which is defined by the EPA as greater than or equal to 1.0 mg/cm2 lead by surface loading (as tested by an XRF instrument). EPA/HUD compliant XRF (X- Ray Fluorescence) sampling methods, which are non-destructive, were employed for this project assessment. Note that lead glazing finishes on ceramic tiles and blocks have been tested in the same manner as painted surfaces, with results held to the same standard of lead mass loading of

1.0 mg/cm2.

It is stressed that this sampling was intended to identify the presence of lead-based paint and/or lead glazed tiles and blocks within readily accessible areas of the building likely to become disturbed during future window removal and associated with this particular project design, and not necessarily an EPA-HUD type “surface-by- surface” inspection. Readings were collected by an EPA-HUD trained Lead Inspector/Risk Assessor.

An attempt was made to sample each paint color on indoor wood window components, where accessible, in functional areas within the building Wings. Painted window components for a given window were generally the same color with a different color presented on the adjacent wall substrate. Wood trim was not observed outside, all surfaces appeared to be black metal covering over stiles, rails, jambs, and sills and readings were not collected.

Wing A

Indoor testing locations and XRF results are contained in Table 2. The primary type or assumed to be aged (Typical) window has wood vertical moulding, header moulding, quarter round, and slanted sill (A.1, A.2, B.1, B.2) where one or all of these components contain lead-based paint.

Within areas where floor spaces have been remodeled, some window types have visible lead-based wood moulding (A.3 & B.3). It was not apparent whether underlying lead based painted wood mouldings, sills, or ceramic tiles exist behind the new Faux Drywall surrounds at C.3 and D.3 locations. In approximately 12 locations, exterior painted steel cages were not accessible however should be assumed to be have a lead-based paint. Visual observation suggested that the lead paint film, where component readings were positive for lead, likely exists beneath the current paint color as suggested by multilayer paint observed in chipped locations. A total of 390 XRF measurements, including 13 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials likely to be

These measurements represent approximately 97 windows and 179 readings were considered to be lead based paint or ceramic glaze. Paint condition on window components were noted to be intact with the exceptions of Level 10 Mechanical Room, Hall near 837, A837, and A014 where the paint is deteriorated.

Additional sampling occurred during the Pilot Study window removal process that took place in Room A904. Remediation processes exposed original painted wood window components which were then tested for lead based paint using the same procedures and guidelines previously mentioned. Additional testing proved that interior concealed wood window components are considered lead-based paint containing and should be assumed present throughout A Wing for all window types.

Wing B

Indoor testing locations and XRF results are contained in Table 2. The primary type or assumed to be aged (Typical) window had wood vertical moulding, header moulding, quarter round, and slanted sill (A.1, A.2, B.1, B.2) where one or all of these components contain lead-based paint. Within areas where floor spaces have been remodeled, some window types have visible lead-based wood moulding (A.3 & B.3). It was not apparent whether underlying lead based painted wood mouldings, sills, or ceramic tiles exist behind the new Faux Drywall surrounds at C.3 and D.3 locations. A total of 187 XRF measurements, including 18 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. These measurements represent approximately 44 windows and 95 readings were considered to be lead based paint or ceramic glaze.

Additional sampling occurred during the Pilot Study window removal process that took place in Room B015. Remediation processes exposed original painted wood window components which were then tested for lead based paint using the same procedures and guidelines previously mentioned. Additional testing proved that interior concealed wood window components are considered lead-based paint containing and should be assumed present throughout B Wing for all window types.

Wing C

Indoor testing locations and XRF results are contained in Table 2. The primary type or assumed to be aged (Typical) window was not observed in C Wing at the time of the inspection.

Limitations may have inhibited the observation of such typical windows that could contain lead based paint components. It was not apparent whether underlying lead based painted wood mouldings, sills, or ceramic tiles exist behind the new Faux Drywall surrounds at J.3, L.3, M.3 and N.3 locations. A total of 49 XRF measurements, including 15 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. These measurements represent approximately 11 windows and 0 readings were considered to be lead based paint or ceramic glaze.

It was not apparent whether underlying lead based painted wood mouldings, sills, or ceramic tiles exist behind the new Faux Drywall surrounds at L.3, M.3 and H.3 locations. A total of 38 XRF measurements, including 15 measurements for QA/QC purposes were collected from painted surfaces and/or suspect lead-containing materials likely to be impacted by project work. These measurements represent approximately 8 windows and 0 readings were considered to be lead based paint or ceramic glaze.
