About this file
This document is a 100% Construction Documents Addendum for a Building 1 Interior Demolition and Abatement project at the Captain James A. Lovell Federal Health Care Center in North Chicago, IL. The project involves a comprehensive interior demolition and abatement of a three-story brick building constructed in 1925, approximately 57,000 gross square feet, with a goal of performing a full gut renovation that removes HVAC, electrical, plumbing, and telecom systems while leaving fire protection intact.
The environmental design narrative details comprehensive asbestos and lead inspection results, revealing 92 asbestos-containing material samples across 32 homogeneous material groups, including various floor tiles, mastics, cove bases, lab countertops, and other building materials. The inspection was conducted by Riverfront Safety & Health, LC, a Service-Disabled Veteran Owned Small Business, with personnel accredited under the Asbestos Hazard Emergency Response Act. The project also identified significant moisture intrusion issues throughout the building's first floor, with visible mold growth on thermal system insulation, walls, and ceilings, necessitating potential additional remediation and investigation.

PROJECT NAME Building 1 Interior Demolition and Abatement at Captain James A. Lovell FHCC
SUBMITTAL 100% Construction Documents – Addendum 2
SUBJECT Environmental Design Narrative
Project Overview and Background
Riverfront Safety & Health, LC (Riverfront), a Service-Disabled Veteran Owned Small Business, has provided a limited asbestos and or lead inspection summary and abatement plan in support of the Building 1 Interior Demolition and Abatement Project (VA Project 556-24-102) proposed at the Captain James A. Lovell Federal Health Care Center in North Chicago, IL (The Project).
Field Investigation Objectives
Project objectives are as follows:
Provide comprehensive design and construction documents for Building 1 interior demolition and abatement at Captain James A. Lovell Federal Health Care Center, as further described under this Scope of Work. Building 1 is a brick-clad, three-story structure with an attic, sub-basement and crawlspaces used for administration, research, and education. The building was constructed in 1925 and is approximately 57,000 GSF. The building was partially occupied at the time of inspection. The goal of this project is to perform a full gut with the removal of HVAC, electrical, plumbing, and telecom, leaving fire protection intact.
Riverfront was provided Tropical Environmental Inc.’s 2011 Asbestos Condition Assessment Report and historic as-builts from Browne-Morse Company dated 1976 and from Balluff and Balluff dated 1980 and 1982 (Appendix E). No information on historic or current lead or mold conditions was provided.
Field Investigation Summary
Riverfront provided inspection personnel accredited under the Asbestos Hazard Emergency Response Act (AHERA), U.S. Environmental Protection Agency Model Accreditation Program for Asbestos Building Inspectors and U.S. Environmental Protection Agency accredited Lead Inspectors to perform the following:
Verify the presence, location, quantity, appearance, and condition of all applicable building materials exhibiting asbestos, lead and mold containing materials within areas of scheduled work at Building 1, identified by the VA as likely to be impacted during this project.
The project specific limited asbestos survey was performed by Mr. Charles Davis, Mr. Jamison Bear, and Mr. Travis Carter on July 8, 9, 10, 11, and 12, 2024. Mr. Justin R. Rhyneer and Mr. Davis gathered as-built record document on September 9, 2024. Mr. Davis and Mr. Bear return for an additional survey on November 11 and 12, 2024. All inspectors are accredited under the Asbestos Hazard Emergency Response Act (AHERA), U.S. Environmental Protection Agency Model Accreditation Program for Asbestos Building Inspectors. Furthermore, Mr. Davis is an Illinois Department of Public Health (IDPH) licensed Asbestos Building Inspector and Lead Risk Assessor. All work was performed under the supervision of a Certified Industrial Hygienist (CIH), Mr. Justin R. Rhyneer, to ensure standard EPA-NESHAP/AHERA/OSHA field inspection and sampling methods were employed for representative project inspection. Principal contacts for this project were Sean Brewner of the Captain James A. Lovell Federal Health Care Center and Andrew Stevens from Spur Design. The project inspection was performed with the following assumptions and limitations:
• The VA intends to vacate the building prior to construction, but it will still be partially occupied at the time of this design inspection effort, which requires selectively destructive sampling means. As such, inspection efforts must include more deliberate and cautious means and methods (which would not otherwise be necessary for unoccupied building) to ensure the safety of building occupants during performance of destructive sampling efforts needed to identify concealed hazardous materials components within scope of work. With this in mind, It must be stated that all areas within the scope of work were not able to be completely assessed due to both active office
Captain James A. Lovell FHCC work in areas of the building as well as active construction work in other areas. Trends and historical information have been applied where necessary. Due to the ongoing nature of construction in this building, materials conditions may defer by time of construction for The Project
• The VA confirms that the end-product of this project is a building gutted down to bare concrete/substrate on floors, walls, and ceilings construction.
• The VA does not have a full set of historical abatement inspection records or closeout records to provide.
• The VA directs the AE to assess current conditions in relation to provided 12/12/2011 Tropical Environmental, Inc. Asbestos Report.
• The limited mold assessment was based on visual observations only.
• There was an active and ongoing construction project installing new windows to Building 1 during the time of the inspections. The dynamic nature of inspecting during an active construction site means that conditions may change rapidly, potentially exposing or concealing hazardous materials.
As such, our design is based on observations and sampling from our last current inspection.
Asbestos Containing Materials – Assessment & Inspection Results
Riverfront personnel collected a total of 442 representative samples of suspected asbestos containing materials related to The Project. The suspect materials, which were similar in general appearance and age of construction, were grouped into homogeneous material sets and sampled. Standard industry practices were used in collecting samples to ensure the safety of all personnel. All samples were transferred via written chain-of -custody, to EMSL, an AIHA & NVLAP accredited laboratory, for analysis.
A minimum of three samples were collected of all suspect asbestos containing building materials. All samples were analyzed using polarized light microscopy (PLM) analysis with dispersion staining (DS) according to U.S. Environmental Protection Agency (EPA) Method 600 IR-93/116 which is presently recommended by the EPA.
The EPA defines asbestos-containing materials (ACM) as having greater than 1% asbestos as measured by polarized light microscopy (PLM) methods. Out of the four hundred forty-two (442) suspect asbestos samples collected, ninety-two (92) samples were identified as asbestos-containing materials by PLM analysis. These ninety-two (92) ACM samples represent thirty-two (32) homogeneous material groups and are as follows:

Addendum 1 Update – After the completion of the project, additional details were added to the environmental drawing set, prompted by pre-bid RFI’s provided by the contractor. These additions included small amounts of additional floor tile and/or asbestos mastic that will need to be removed. In addition, the exact items to be abated were changed in some areas. The overall cost impact of these changes is negligible. On sheet HA102, areas CO-2-01, 226A-1, and 200-1 were changed from asbestos floor tile and asbestos black mastic abatement to non-asbestos floor tile and asbestos black mastic abatement. Asbestos floor tile and black mastic removal was also added to room 208C-1. On sheet HA101, asbestos floor tile and asbestos black mastic removal was added to room 117B-1.

Lead Containing Materials – Assessment & Inspection Results
A targeted lead inspection was performed on representative indoor building components to identify lead-based paint on primary substrates likely to be impacted by The Project.
Riverfront personnel Charles Davis collected lead readings from targeted surfaces to identify the presence of lead-based paint (LBP), which is defined by the EPA as greater than or equal to 1.0 mg/cm2 lead by surface loading (as tested by XRF instrument) or greater than 0.5% (5,000 parts per million lead by dry weight) from paint chip samples. EPA/HUD compliant XRF (X-Ray Fluorescence) and paint chip sampling methods were employed for this project assessment.
It is stressed that this sampling was intended to identify the presence of lead-based paint and/or lead glazed tiles within readily accessible areas of the buildings which may become disturbed during future construction and activities and associated with this particular project design, and not necessarily an EPA- HUD type “surface-by- surface” inspection. An EPA-HUD trained Lead Inspector/Risk Assessor collected all readings.
The RSH inspection team gathered a total of 229 readings including instrument calibration readings and 20 paint chip samples. Of these, 65 readings (36 materials) were above the 1.0 mg/cm2 threshold. Of the 20 paint chip samples, 3 paint chip samples (2 materials) were above 0.5% percent by weight threshold. Materials showing elevated levels included:

Mold Impacted Buildings Materials – Assessment & Inspection Results
An inspection targeting visible mold growth (VMG) was also performed alongside the lead and asbestos surveys in support of The Project design. Currently, no licensing requirements exist for performing mold inspections.
The inspection of Building 1 revealed significant evidence of moisture intrusion throughout the first floor.
This moisture presence was observed manifesting in various forms including visible water staining on various flooring systems. Water staining was noted on multiple flooring types, suggesting prolonged exposure to moisture which can lead to the deterioration of flooring materials and introduce potential safety hazards. VMG was observed on thermal system insulation (TSI). The presence of VMG on TSI indicates that moisture is condensing on these surfaces. This can compromise the thermal efficiency and promote mold growth throughout the rest of the buildings HVAC system. VMG was also observed on drywall and plaster wall systems and plaster ceiling systems throughout the first floor. Plaster wall failures were observed in the forms of blisters, efflorescence, cracking and debonding. Blistering indicates moisture trapped beneath the plaster surface, efflorescence suggests moisture migration through the plaster, and cracking and debonding suggests structural integrity issues due to moisture infiltration and the expansion/contraction cycle. An active project was underway during our inspection in which the windows in Building 1 were actively being removed and replaced with temporary plywood sheets covering the openings. Observations suggested that gaps around the windows were allowing outdoor environmental conditions to affect the building's interior, exacerbating moisture intrusion issues throughout the building but more noticeable throughout the first floor. The presence of VMG poses health risks to building occupants and ongoing moisture intrusion can lead to further structural damage, compromising building integrity. In summary, observations strongly suggest significant and ongoing moisture intrusion issues are affecting Building 1’s interior. Addressing the moisture intrusion in Building 1 is critical to preserving the integrity of the structure and ensuring a safe environment for occupants. Photo documentation of specific observations are included in Appendix C.

Miscellaneous Hazardous Materials
A visual inspection throughout the subject property took place to identify miscellaneous hazardous materials. Observations have identified or suggested the following miscellaneous hazardous materials at the Subject Property that shall require special handling and final disposition:
• Approximately 16 fluorescent light ballasts not specifically labeled "No PCBs"
• Approximately 1227 fluorescent light bulbs not green tipped (presumed Hg)
• Approximately six mercury-containing (presumed) regulated thermostats
• Approximately 21 emergency exit sign assemblies
Recommended Response Actions
ACM Materials
• ACM materials that will be impacted must be removed by a licensed qualified IDPH contractor.
• A comprehensive asbestos remediation design specification shall be developed to govern the proper repair, handling and/or removal of ACM. This specification shall be prepared in accordance with current EPA/OSHA/State and Local project design guidelines and shall be written to account for the potential disturbance of any ACM, directly or indirectly, during the course of construction-related activities.
• Should any identified ACM components require disturbance and/or removal as part of planned renovation plans, such components should be removed in whole or protected in place, as deemed most appropriate by construction needs and/or asbestos remediation project design specifications.
• All ACM removal activity is to be performed by professional asbestos abatement contractors and workers that are licensed in the State of Illinois for such and trained/accredited in accordance with applicable EPA-AHERA training requirements.
Lead-Based Paint / Lead Containing Materials
• A comprehensive lead paint remediation design specification shall be developed to govern the proper repair, handling and/or removal of lead containing materials likely to be disturbed. This specification should be prepared in accordance with current EPA/OSHA and/or State of Illinois project design guidelines and should be written to account for the potential disturbance of any lead
- based paint, directly or indirectly, during construction related activities.
• All lead containing material impact or removal is to be performed by professional lead abatement contractors and workers that are licensed in the State of Illinois for such and trained/accredited in accordance with applicable EPA training requirements.
Mold Impacted Building Materials
• Consider initiating immediate investigations into building envelope integrity to ensure that all possible sources of water intrusion associated with the building envelope system have been thoroughly investigated and mitigated. This may require a more thorough (i.e. remove drywall paneling, remove roof insulation, or direct observation) assessment of conditions within wall cavities and above ceilings. For obvious reasons, all water intrusion concerns should be resolved prior to initiation of mold/moisture impacted building materials, as outlined below.
• Consider conducting an aggressive and more invasive investigation approach in order to further
Captain James A. Lovell FHCC delineate the nature and extent of excessive mold growth conditions present within The Site, particularly within otherwise concealed locations such as wall cavities and above ceiling spaces, so that impacted building materials may be removed and replaced. Two distinctly different approaches can be pursued, each with their own advantages and disadvantages, as follows:
o Have an Illinois-licensed abatement contractor perform exploratory abatement of mold impacted ceiling and walls under controlled conditions (i.e. construction of negative pressure enclosure) for the purpose of removing damaged materials posing a risk to occupants, as well as to facilitate visual examination of the condition of concealed surfaces (i.e. backside of material not otherwise accessible). Any surfaces exhibiting suspect staining/debris/growths would be subject to abatement/removal followed by a microbial visual assessment and final air clearance after abatement activities. This approach offers an immediate response action but presents less certainty with respect to eventual abatement related costs. A mold/moisture abatement plan should be prepared by a Certified Industrial Hygienist (CIH) for use by selected abatement contractor to ensure a safe, methodic approach to this process.
o Have an Industrial Hygiene consultant perform a more detailed, more invasive investigation effort in order to more clearly delineate the full nature and extent of water intrusion and/or mold growth related conditions, followed by development of a mold abatement plan by a Certified Industrial Hygienist (CIH) offering a clear scope of work for subsequent efforts to remove all mold/water damaged building components from The Site. This approach offers more certainty with respect to eventual abatement related costs, but is a more deliberate, time-consuming approach.
Removal of Miscellaneous Hazardous Materials
• Miscellaneous Hazardous Material (MHM) building components presenting special regulatory risks and requirements must, by law, be properly removed, manifested and disposed/recycled prior to building demolition activities.
Riverfront appreciates the opportunity to assist with this project. Please feel free to contact us with any questions or concerns.
Sincerely, December 20, 2024 Justin R. Rhyneer, CIH Date Principal

TRADITIONAL ASBESTOS ABATEMENT
PART 1 - GENERAL
1.1 SUMMARY OF THE WORK
A. Contract Documents and Related Requirements: Drawings, general provisions of the contract, including general and supplementary conditions and other Division 01 specifications, shall apply to the work of this section. The contract documents show or describe the work to be done under the contract and related requirements and conditions impacting the project. Related requirements and conditions include applicable codes and regulations, notices and permits, existing site conditions and restrictions on use of the site, requirements for partial owner occupancy during the work, coordination with other work and the phasing of the work. In the event the Asbestos Abatement
Contractor discovers a conflict in the contract documents and/or requirements or codes, the conflict must be brought to the immediate attention of the Contracting Officer for resolution. Whenever there is a conflict or overlap in the requirements, the most stringent shall apply. Any actions taken by the Contractor without obtaining guidance from the Contracting Officer shall become the sole risk and responsibility of the Asbestos Abatement Contractor. All costs incurred due to such action are also the responsibility of the Asbestos
Abatement Contractor.
B. Extent of Work:

D. Tasks:
1. The work tasks are summarized briefly as follows:
a. Pre-abatement activities including pre-abatement meeting(s), inspection(s), notifications, permits, submittal approvals, regulated area preparations, emergency procedures arrangements, and standard operating procedures for asbestos abatement work.
b. Abatement activities including removal, encapsulation, enclosure, clean-up and disposal of ACM waste, recordkeeping, security, monitoring, and inspections.
c. Cleaning and decontamination activities including final visual inspection, air monitoring and certification of decontamination.
E. Contractors Use of Premises:
2. The Contractor and Contractor's personnel shall cooperate fully with the VA representative/consultant to facilitate efficient use of buildings and areas within buildings. The Contractor shall perform the work in accordance with the VA specifications, drawings, phasing plan and in compliance with any/all applicable Federal, State and
Local regulations and requirements.
3. The Contractor shall use the existing facilities in the building strictly within the limits indicated in contract documents as well as the approved VA Design and Construction Procedures. VA Design and
Construction Procedures drawings of partially occupied buildings will show the limits of regulated areas; the placement of decontamination facilities; the temporary location of bagged waste
ACM; the path of transport to outside the building; and the temporary waste storage area for each building/regulated area. Any variation from the arrangements shown on drawings shall be secured in writing from the VA representative through the pre-abatement plan of action. The following limitations of use shall apply to existing facilities shown on drawings:
1.2 VARIATIONS IN QUANTITY
A. The quantities and locations of ACM as indicated on the drawings and the extent of work included in this section are estimated which are limited by the physical constraints imposed by occupancy of the buildings and accessibility to ACM. Accordingly, minor variations (+/-

02 82 11 - 5 considered as having no impact on contract price and time requirements of this contract. Where additional work is required beyond the above variation, the contractor shall provide unit prices for newly discovered ACM and those prices shall be used for additional work required under the contractor.
1.3 STOP ASBESTOS REMOVAL
A. If the Contracting Officer; their field representative; the facility
Safety Officer/Manager or their designee, or the VA Professional
Industrial Hygienist/Certified Industrial Hygienist (VPIH/CIH) presents a verbal Stop Asbestos Removal Order, the Contractor/Personnel shall immediately stop all asbestos removal and maintain HEPA filtered negative pressure air flow in the containment and adequately wet any exposed ACM. If a verbal Stop Asbestos Removal Order is issued, the VA shall follow-up with a written order to the Contractor as soon as it is practicable. The Contractor shall not resume any asbestos removal activity until authorized to do so in writing by the VA Contracting
Officer. A stop asbestos removal order may be issued at any time the VA
Contracting Officer determines abatement conditions/activities are not within VA specification, regulatory requirements or that an imminent hazard exists to human health or the environment. Work stoppage will continue until conditions have been corrected to the satisfaction of the VA. Standby time and costs for corrective actions will be borne by the Contractor, including the VPIH/CIH time.
